[Effect of etomidate on cerebral blood flow and oxygen metabolism in man].

The effects of Etomidate on cerebral blood flow (C.B.F.) and cerebral oxygen consumption (C.02 C.) were studied in 13 patients in a neuroradiology department. Induction of anaesthesia was ensured using a standard dose of 15 mg of Etomidate, followed by a constant rate infusion (2.8 mg/kg/hour). Seven subjects were considered to be normal whilst six were suffering from tumour pathology. Under the influence of Etomidate, C.B.F. in the normal subjects decreased by 34 p. 100 and C.02 C. by 46.7 p. 100. The relationship between C.B.F. and arterial PC01 was studied under the influence of Etomidate: cerebral vasoactivity to C02 was maintained. A special study was made of focal changes in flow in patients with tumour pathology. Under the influence of Etomidate, there were zones of adaptation of flow, indicative of vasoparalysis situated opposite the tumour.

Cerebral blood flow in migraine and cluster headache. Compartmental analysis and reactivity to anaesthetic depression.

CBF was studied in 15 cases of vascular headache by the 135Xe intra-arterial injection method. The mean CBF was found to be increased during the headache phase of the migraine attacks in half the cases, mainly due to an increase in the rapid component (CBFg). After the attack there could be an increase or a decrease of the slow component (CBFw). Reactivity to anaesthetic depression was studied in 8 migraine cases and on the whole it was found not to be much altered in most cases. Finally, no modifications of CBF were found in any of 3 cases of cluster headache who were studied during attacks of severe pain.

[Transverse axial computerised tomography of the brain in partial epilepsy].

Two hundred epileptics presenting with partial epileptic attacks were subjected to investigation by Acta-Scanner. Results were normal in 100 cases. Also found were 20 tumours (13 of them astrocytomas), 7 epileptogenic softenings, 41 cerebral atrophies, 8 of which were porencephalies, 5 calcifications (3 isolated, 2 multiple, connected with Bourneville's disease), 5 post-operative lesions and 22 dubious conditions. Various correlations between the results of scanning, the clinical findings and the E.E.G. are considered. The value of scanning in screening for tumours developing under the cloak of isolated epilepsy is emphasized and attention is drawn to certain difficulties of interpretation.

[Tomodensitometry in stereotaxic conditions (possibilities and limitations in the diagnosis and treatment of brain lesions)].

The stereotaxic approach of cerebral lesions for diagnostic (biopsies) and/or therapeutic purposes (interstitial radiotherapy) has been well documented. These techniques depend on an exact spatial definition of the tumoral volume, which is often difficult to obtain through conventional neuroradiology, stereoencephalography, rheography... The use of tomodensitometry in this context is obviously of upmost interest if, and only if, the scan itself is obtained in stereotaxic conditions. The authors present a stereotaxic system with which all the diagnostic data, including CT scan itself, are recorded and matched within the same stereotaxic space. This method achieves an exact mapping of lesions within the actual stereotaxic brain space. Hence any further stereotaxic gesture can be calculated and controlled with a suitable accuracy. The interest of these procedures in the management of brain lesions is demonstrated through an experience of 52 supra tentorial tumors. 13 cases of lesions under 20 mm in diameter are especially discussed.

[Effects of Alfatesine (ct 1341) on cerebral blood flow and metabolism in man. Global and regional changes].

The effects of Alfatesine on local cerebral blood flow and on cerebral oxygen consumption were studied in 5 normal subjects and in 6 patients suffering from diffuse vascular repercussion of cerebral distress. There is a 75 p. 100 fall in cerebral blood flow in the gray substance and a parallel fall in oxygen consumption. At rest, the caracteristic anaesthetic action modifies the caracteristical distribution of blood flows in the cortex. A relative hyperemia appears in the area of the central sulcus. Those focal changes could not be found in the group of pathogical patients.
