Digital subtraction angiography in the diagnosis of arterial complications after renal transplantation.

Digital subtraction angiography (DSA) was used to study arterial complications following renal transplantation in 33 patients. The results were compared with clinical follow-up and in three cases with conventional angiography. In 9 per cent using DSA (3 cases) we experienced inadequate visualization of the renal arteries of the graft; in 91 per cent, the visualization was diagnostically sufficient. In 53 per cent, we discovered an arterial stenosis of the main artery or segmental artery and in one case an AV-fistula. No morbidity resulted during the procedure. We consider DSA to be the best non-invasive method in the evaluation of patients with suspected renal artery stenosis following kidney transplantation.

[Advances in diagnosis and their significance for radiological practice].

Many changes have taken place in x-ray diagnosis from the early introductory days of tomography and the introduction and development of the image intensifier. Ultrasound marked a big step forward. There are now about 12 000 ultrasound units in the FRG. In general practice, ultrasound is mainly used for screening examinations of the abdominal organs, whereas in hospitals special examinations are of great importance, such as neonatal skull, thyroid gland, mammary gland, scrotal sonography, and the valuation of blunt traumas. In nuclear medicine, an important advance was the application of spect in the diagnosis and localisation of disturbances of regional myocardial perfusion in coronary heart disease. In the last years CT has revolutionised radiology. In the FRG, the number of CT units, and all kinds of CT investigations in hospitals and in general practice, have been increasing rapidly. In 1983, there were 361 CT scanners in the FRG. The interdisciplinary character of radiology emerges especially in those areas which are known as interventional radiology. The most important methods are percutaneous transluminal angioplasty, percutaneous transhepatic cholangiography, puncture and drainage of abscesses, extraction of intraluminal foreign bodies, as well as therapeutical occlusion of vessels. Digital subtraction angiography is a new imaging method which is now available at all major departments in Western Germany. The method is well established by transvenous contrast medium injection for examination of the carotids, the renal arteries, the aorta, and the larger vessels of the extremities. Another new imaging method, magnetic resonance, has produced excitement in medical imaging.(ABSTRACT TRUNCATED AT 250 WORDS)

[Comparison of mammography and automated sonography in 700 patients].

700 patients underwent routinely x-ray mammography and breast ultrasound with an automated grey-scale breast scanner. The parenchymal pattern of the ultrasound B-scan was morphological identical to the x-ray film. The echogram gave further informations about the topographic localisation of pathologic lesions to the chest wall including the pectoral muscles. The sonogram was of no value in 10 percent due to diffuse reflection of the breast. In cystic disease the diagnostic accuracy of the ultrasound was with about 100 percent superior to that of x-ray mammography. On the contrary the accuracy of the x-ray mammography was higher than that of ultrasound in the diagnosis of benign or malignant solids. Benign adenoma smaller than 1 cm could not be definetively recognised as carcinoma of this size. The diagnostic accuracy of the sonogram was 70 percent by 40 histologically proved carcinomas. The only use of the automated scanner in the diagnostics of the breast seams to be not justified, even not as a screening method, especially because small tumors with good prognosis are not recognized, however, the ultrasound gives additional important informations in the differential diagnosis of benign and malignant lesions of the female breast.

[Control of reconstructive vascular surgery using digital subtraction angiography (DSA)].

100 patients were evaluated by DSA after vessel reconstructions by surgery or percutaneous transluminal angioplasty (PTA). The visualization of the proved vessels was very good or good in 92%, sufficient in 6% and without diagnostic value in 2%. Patency of grafts and postoperative vascular abnormalities like stenoses, occlusions or aneurysms were well demonstrated. According to our results, DSA is an excellent semi-invasive method with a high accuracy for postoperative evaluation of vascular reconstructions.

[Digital subtraction phlebography].

The usefulness of digital subtraction angiography of the venous system, which we have termed "digital subtraction phlebography (DSP)" was demonstrated in the region of the vena cava, the pelvic veins and the veins of the arms. DSP is especially of diagnostic value in the region of the vena cava and pelvic veins and offers no advantage in the veins of the lower extremities compared to conventional phlebography. DSP is a time--and cost--saving method, furthermore minor concentrations of contrast media are needed.

Double-contrast small-bowel examination with barium and methylcellulose.

A modification of the small-bowel enema with barium and methylcellulose is described and findings in 300 consecutive examinations compared with the clinical diagnoses. The radiograph was positive in 37% of cases and the clinical examination in 29%. Diseases and abnormalities of the small bowel, including motility disorders, can be diagnosed with high sensitivity and specificity using this technique.

[Comparison of conventional radiology, ultrasound and computer tomography in the preoperative localization of intraocular foreign bodies].

Three methods complementing each other are available for the preoperative intraocular localisation of foreign bodies. Basing on the results obtained, it is recommended, first of all, to localise the site of the foreign body by means of conventional x-ray film, followed by fine diagnosis via ultrasound and, finally, in problematic or doubtful cases, by means of computed tomography.

[Results of serial CT in the evaluation of the sellar region].

The article reports on the methodology and results of serial CT in differential diagnosis of intrasellar and parasellar structures, based on 32 examinations. Whereas the normal pituitary gland displays an approximately equal contrast medium performance as the pituitary adenomas and recurrent tumours, different types of tissue such as those of tumours, vascular structures and postoperative scans can be well differentiated from each other by means of serial CT. Aneurysms can be safely differentiated from intracranial tumours, such as meningiomas, only via serial CT.

[Therapeutic results of malignant goiter 1960-1980. Significance of percutaneous radiotherapy in well differentiated carcinomas].

The therapeutic approach in malignant goitre is fairly uniform, apart from use of percutaneous irradiation. The influence of irradiation on survival rate in differentiated carcinomas transgressing organ boundaries was assessed in the evaluation of treatment results in 621 patients (two patient groups: 1960-1980). Early stages of tumour (T0-T1) were seen in only 18% of cases; 24% of all cases had metastases, mainly of the lungs and skeleton. Males died 1.5 times more frequently due to differentiated tumours than did females The 10-year-survival rate was 90% in patients below the age of 50 years in contrast to 60% in older patients. Papillary carcinomas had such a survival rate in 78% and follicular carcinomas in 66%. Stages T0-T2 had a survival of 87% in contrast to 58% in stage T3. Occurrence of distant metastases diminished survival to 48%. 167 patients with irradiation (I) and 73 without (II), but an otherwise equal treatment, with differentiated carcinoma stage T3 could be compared. Group I showed a 5- and 8-year survival of 88% and 75%, group II 68% and 38% (P less than 0.001). Local recurrence was the same in both groups, however metastases occurred more frequently and earlier in group II.

[Video-densitometric demonstration of abnormal movement of the left ventricle compared with results of levocardiography (author's transl)].

Video-densitograms of the heart of 46 patients were obtained in three projections; these patients had clinically and angiocardiographically confirmed coronary disease of varying severity, or suffered from cardiomyopathy or were normal. The results were compared with levocardiography. The purpose of the investigation was to determine to what extent abnormalities of movement, seen on levocardiography, can also be demonstrated by video-densitometry. Our results show that video-densitometry, compared with levocardiography, has a sensitivity of 97%, a specificity of 44% and an accuracy of 86%. Video-densitometry provides definite proof of abnormal cardiac movement. The differentiation of hypokinesia from akinesia by the two methods is discussed.

Computed tomography for localizing enlarged parathyroid glands in primary hyperparathyroidism.

Computed tomography (CT) has been performed in 21 patients with primary hyperparathyroidism (HPT) in an attempt to localize preoperatively the parathyroid tumors. Eighteen of these patients also had ultrasound examinations. The CT examinations included the neck and mediastinum and were performed before and after contrast medium administration. At subsequent surgery, 24 enlarged glands were found, 20 in the neck and 4 in the mediastinum. In the neck, two were locally recurrent carcinomas. The true positive diagnostic rate for ultrasound was 60% with a false positive rate of 15% and a false negative rate of 25%. The CT examinations resulted in 70.9% true positive, 8.3% false positive, and 20.8% false negative diagnoses. The combination of both methods in the 18 patients so studied resulted in 80% true positive, 10% false positive, and 10% false negative findings. The CT examination procedure and its interpretation and limitations in primary HPT are described.
