A spatial power spectrum analysis of the electroencephalogram.

The method of spatial power-spectrum analysis has been applied to measurements of the distribution of rms alpha-band potential on the scalp. Data was recorded using the 31-Electrode System and spatial power-spectrum estimates (PSEs) were obtained from Mercator projections of the potential interpolated using the triangular method. PSEs were calculated using the Lim and Malik algorithm for maximum-entropy power-spectrum estimation. In order to investigate the utility of spatial power-spectrum analysis, PSEs were obtained from subjects in two conditions; resting with eyes closed (EC) and resting with eyes open and fixed on a single point (EO). A stepwise discriminant analysis was performed with features from the PSEs and the resultant discriminant function was applied to data not considered in the formulation of the function. Over 92% of test data was correctly classified. The features used in the discriminant function identify spatial waves which are most useful in separating data. The results demonstrate that waves oriented along front-back and right-left lines are most important in separating data into EC and EO groups.

Code teams and the review of cardiac arrests.

A new system for responding to Codes, or resuscitations of patients with cardiopulmonary arrest, was implemented at St Luke's Medical Center, Milwaukee. A teaching program was begun to establish the Advanced Cardiac Life Support (ACLS) guidelines as the standard of care. A task force defined the composition of the Code team and was then designated the Code Review Committee, meeting monthly to review all Codes. Indicators for review were established. Code review is followed by a letter to the physician Code team leader and/or registered nurse. Data for 18 months, still preliminary, show improved compliance with indicators for documentation and for adherence to ACLS guidelines.

Prediction of 24-h energy expenditure and its components from physical characteristics and body composition in normal-weight humans.

The applicability of body composition as estimated by the bioimpedance method to predict energy expenditure (EE) was studied. Ten healthy subjects underwent measurement of body composition and 24-h energy expenditure (24-h EE) twice in a respiration chamber on a fixed program. The 24-h EE and its components, sleeping EE (SEE), basal EE (BEE), and daytime EE, for an individual were very reproducible (coefficient of variation 2.3%, 1.4%, 5.0%, & 3.1%, respectively). The variability of 24-h EE among subjects was 11.4% but only 4.1% when adjusted for differences in lean body mass (LBM). LBM was the best determinant of 24-h EE, BEE, and SEE and accounted for 91-93% of the interindividual variance of EE. The prediction equations were 24EE (kcal/d) = 390 + 33.3 LBM (r2 = 0.93, P = 0.000001), SEE (kcal/h) = 9.8 + 1.1 LBM (r2 = 0.92, P = 0.000001), and BEE (kcal/h) = -3.1 + 1.35 LBM (r2 = 0.91, P = 0.000002). In conclusion, 24EE, BEE, and SEE can be predicted with a high degree of precision from LBM as estimated by bioimpedance in normal-weight subjects.

[Moped accidents. A prospective study on the causes of accidents, injury patterns and sequelae].

In a one year prospective investigation, the circumstances and sequelae of 75 moped accidents were registered. The patients were aged mainly between 14 and 18 years and the accidents happened mainly in the afternoon hours. The majority of the injuries were on the head and the extremities. Twenty-one of the patients were admitted to hospital for median 4.8 days, 50 were treated as outpatients and four were dead on admission. The patients were contacted one year after the accident and one had severe sequelae and five had minor sequelae.

Pulse radiolysis study of the reactivity of Trolox C phenoxyl radical with superoxide anion.

The reaction between the phenoxyl radical of Trolox C, a water-soluble vitamin E analogue, and superoxide anion radical was examined by using the pulse radiolysis technique. The results indicate that the Trolox C phenoxyl radical may undergo a rapid one-electron transfer from superoxide radical [k = (4.5 +/- 0.5) x 10(8) M-1.S-1] to its reduced form. This finding indicates that superoxide radical might play a role in the repair of vitamin e phenoxyl radical.

Abbrevatio: a new operation for chronic mallet finger.

An old, but hitherto unpublished, method for surgical correction of chronic mallet finger was carried out in 42 patients. In the abbrevatio operation, the elongated tendon is transected and resutured, after which the developing fibrous scar tissue usually causes adequate shrinkage for correcting the drop-deformity. The D.I.P. joint is transfixed with a K wire in slight hyperextension for six weeks. Forty patients were studied. The result was excellent in 16 patients (40%), good in 20%, fair in 17.5% and poor in 22.5%. On average, the extension defect was decreased from 38.2% to 11.8%. The method is a suitable alternative in patients with annoying chronic mallet finger who refuse arthrodesis or further conservative treatment.

N-(heterocyclic alkyl)pyrido[2,1-b]quinazoline-8-carboxamides as orally active antiallergy agents.

A series of N-(heterocyclic alkyl)pyrido[2,1-b]quinazoline-8-carboxamides were evaluated for their ability to antagonize slow-reacting substance of anaphylaxis (SRS-A) induced contractions of guinea pig ilea and to inhibit thromboxane synthase in vitro. The results indicated that those pyrido[2,1-b]quinazoline-8-carboxamides bearing a branched-chain alkyl moiety in the 2-position and a four to six atom linear chain between a 3- or 4-substituted pyridine or a 1-substituted imidazole ring and the carboxamide nitrogen atom showed the best combination of potencies in the two assays. Several of these compounds were found to be orally active inhibitors of LTE4-induced bronchoconstriction in the guinea pig and LTE4-induced skin wheal formation in the rat. One of the most potent analogues, 2-(1-methyl-ethyl)-N-(1H-imidazol-1-ylbutyl)-11-oxo-11H-pyrido [2,1-b]quinazoline-8-carboxamide (36), was selected for extensive pharmacological investigation. It was found that this compound was not a specific inhibitor of LTE4-induced symptomatology, but exhibited more general activity by inhibiting bronchospasm in guinea pigs induced by LTC4, LTD4, PAF, and histamine and skin wheal formation in rats and guinea pigs induced by LTC4, LTD4, and PAF. In addition, 36 was orally active in the passive cutaneous anaphylaxis assay, suggesting that it also exhibits mediator release inhibitory activity. On the basis of the overall pharmacological profile of 36 and its closely related analogues, it was concluded that these compounds may be useful for the treatment of asthma.

The psychological impact of diagnostic ultrasound.

The psychological impact of ultrasound scanning in pregnancy was examined in low- and high-risk pregnancies. Women in each category were assigned consecutively to a condition of either low or high feedback. In the latter condition, the women received extensive verbal and visual feedback, whereas in the former, subjects were denied visual access to the monitor. Psychological changes were measured using the State Anxiety Inventory and the Subjective Stress Scale. When a male partner attended, he was included in the assessment. The emotional impact of ultrasound was influenced by the level of feedback provided, with those in the high-feedback condition indicating significantly less anxiety and more positive emotional experiences during the scan, compared with those who received less feedback.

Elevated serum transaminase levels during ritodrine administration.

Two cases are presented of the occurrence of elevated serum transaminase levels during ritodrine administration for the treatment of premature labor. The clinical course of these cases suggests that elevated transaminase levels per se do not require the discontinuation of beta-sympathomimetic tocolytic treatment.

Ifosfamide and etoposide as salvage therapy for non-Hodgkin's lymphoma.

The result of a combination consisting of ifosfamide and etoposide was evaluated in 57 patients suffering from non-Hodgkin's lymphoma (NHL) who had relapsed or were resistant to adriamycin-containing combinations. Some of the patients (35%) also received methotrexate. 80% of the patients belonged to the histology group 'high-grade' malignant NHL. 11 of the 57 patients (19%) obtained a remission, 5 of these were complete and 6 partial. The remission predominantly occurred in the group of patients with 'high-grade' malignant histopathology who were treated at the first relapse after complete remission or when front-line treatment induced a partial remission and ifosfamide and etoposide were added in direct connection with front-line treatment. It is concluded that the use of the combination of ifosfamide and etoposide should be limited to such patients.

The rib cage in normal and emphysematous subjects: a roentgenographic approach.

The configuration and motion of the bony rib cage were studied from lateral chest roentgenograms in 10 young normal subjects (YN), 12 elderly normal subjects, and 12 hyperinflated emphysematous patients [chronic obstructive pulmonary disease subjects (COPD), mean total lung capacity (TLC) 133% of predicted]. The acute angles formed by the fourth through seventh ribs with an axial reference plane were measured at residual volume, functional residual capacity, and TLC in both supine and standing positions and correlated with corresponding lung volumes. both rib angles (RA) and changes in RA with lung volume were greatest with the fourth rib and decreased progressively going down (caudad) the chest. At TLC the RA of upper ribs was significantly less in EN and significantly greater in COPD than in YN. RA's were greater supine than standing. When RA information was used together with autopsy data on the angles formed by intercostal muscles with adjacent ribs, intercostal muscle lengths in hyperinflation could be calculated. Computed intercostal muscle length data suggested that hyperinflation should not be associated with degrees of intercostal muscle shortening or overstretching, that would interfere seriously with tension generation.

Obstetrical complications in a patient with the Marfan syndrome.

A case history of a pregnant patient with the Marfan syndrome is presented. During pregnancy regular echocardiographic examinations were performed, showing a slight increase in the aortic root diameter 1 month after delivery. The hydroxyproline content of the amniotic fluid appeared to be significantly elevated; however, the baby did not seem to be affected by the Marfan syndrome. After delivery we noted a recto-vaginal perforation, which has not been described in deliveries of Marfan patients before. Pregnancy in a Marfan patient is a hazardous situation, with a 10-40% fatal outcome according to the literature. Regular echocardiographic control is necessary to evaluate the cardiac risk.
