Retroperitoneal fibrosis following treatment for Hodgkin's disease.

With improvement and refinement of therapy, the majority of Hodgkin's disease patients are alive and free of disease at 5 years. As these patients continue to be observed, a variety of late complications have been reported. We describe herein three patients who developed retroperitoneal fibrosis following definitive therapy for Hodgkin's disease. The incidence approaches that seen with methysergide.

Refining personality assessments by combining MCMI high-point profiles and MMPI codes, Part V: MMPI code 78/87.

The present investigation shows that the addition of Millon Clinical Multiaxial Inventory (MCMI) high-point code data to the Minnesota Multiphasic Personality Inventory (MMPI) 78/87 code type may be used to clarify contradictory MMPI descriptors and produces three distinct clusters. These MMPI/MCMI clusters were designated as an interpersonally acting-in group, an emotionally acting-out group, and an emotionally acting-in group. In addition to the identification and elaboration of these three subgroups, we outline several clinical uses of this objective test battery approach. The results of this study support the efficacy of combining two objective assessment inventories for the purpose of multiaxial diagnosis as prescribed by the DSM-III.

Chemotherapy of malignant thymoma. Case report and review of the literature.

The authors report the case of a patient with malignant thymoma unresponsive to combination chemotherapy (cyclophosphamide, doxorubicin, vincristine, and prednisone [CHOP] and cisplatin/VP-16) who subsequently achieved clinical response to continuous daily prednisone. A review of the literature indicates that prednisone and cisplatin are the most active agents in the treatment of malignant thymoma.

Contrast-enhanced MRI of periarticular soft-tissue changes in experimental arthritis of the rat.

This study was to determine if manipulation of magnetic resonance signal intensity by means of an intravenously injected paramagnetic contrast agent is useful for the detection and characterization of periarticular inflammation. Arthritis was induced in 20 rats by means of intradermal injection of Freund's complete adjuvant. MR imaging was performed with a resistive magnet operating at 0.35 T. A double spin-echo technique with TE's of 28 and 56 ms and TR's of 0.5 and 2.0 s was used. The hindpaws of the adjuvant-injected rats were imaged on Day 8, Day 11, or Day 15 following injection of the adjuvant. The images were obtained in the transverse plane before and after intravenous injection of gadolinium-DTPA (0.2 mmol/kg). Because of their long T2 relaxation time, inflammatory lesions were characterized by high MR signal intensity on precontrast images obtained with long TR and long TE (T2-weighted images). On the other hand, because of their long T1 relaxation time, the inflammatory lesions were of relatively low intensity and not easily recognized on precontrast images obtained with short TR and short TE (T1-weighted images). Postcontrast T1-weighted images were also sensitive in detecting periarticular inflammation as a result of T1 shortening by the gadolinium-DTPA. However, in our particular model, the data did not indicate any greater MR sensitivity for detecting arthritis by means of gadolinium-DTPA enhancement.

Changes in proton relaxation times of the rat spleen during adjuvant-induced immunologic reaction.

Possible changes in magnetic resonance (MR) relaxation times resulting from an immune reaction were evaluated in the rat spleen by means of in vitro proton spectroscopy and an established model of immune disease. Heat-killed Mycobacterium butyricum in Freund's adjuvant was injected intradermally into the tail of 28 rats. Ten normal rats served as controls. At different times following the injection of adjuvant (day 7, day 10, day 12, day 14, or day 22), the spleens were removed and weighed, and proton T1 and T2 relaxation times were measured with a 0.25-T spectrometer (10.7 MHz). Water content was also determined. Specimens of each spleen were histologically examined; this included Prussian-blue staining for detection of hemosiderin. In 22 of the 28 adjuvant-injected rats, spleen T1 values increased significantly, from a mean value of 451 msec +/- 23 S.D. (control rats) to a maximum mean value of 571 +/- 15 S.D., which occurred on day 12; this T1 increase paralleled an increase in weight and water content and a histologic appearance of granulomatous inflammation. In these rats, T2 changes were not significant. In the other six adjuvant-injected rats, spleen T2 relaxation time significantly decreased from a mean value of 42 msec +/- 5 S.D. to a mean value of 32 msec +/- 1 S.D. Histologic evaluation indicated that the origin of this T2 decrease was probably the presence of hemosiderin in the spleens of only these six rats. Mean T1 of these spleens decreased slightly, but not significantly.

Gallbladder wall thickening in acute hepatitis.

Massive gallbladder wall thickening in association with hepatitis was diagnosed in a 13-year-old hemophiliac boy. The gallbladder returned to normal with improvement of clinical symptoms, illustrating the nonsurgical nature of this problem in pediatrics.

Cystic fibrosis in adults: delayed diagnosis in three siblings.

Cystic fibrosis (CF), which is transmitted as an autosomal recessive trait, is the most common lethal genetic disease in the United States. Median survival age for patients followed up at CF centers in the US is now 21 years. While the disease is diagnosed in most patients before the age of 5 years, in 10% of cases the diagnosis is not confirmed until after age 12. We report an unusual family in which the diagnosis of CF was first established in three siblings at the ages of 36, 40, and 44 years. We describe the clinical features of the patients, as well as issues relating to the diagnosis of CF in adults. This unusual pedigree supports the concept of genetic heterogeneity in CF.

Refining personality assessments by combining MCMI high-point profiles and MMPI codes, Part IV: MMPI 89/98.

This study shows that the addition of MCMI high-point code data to the MMPI 89/98 code type serves to resolve contradictory MMPI descriptors and produces three distinct clusters. These clusters include an interpersonally acting-out group, an interpersonally grandiose group, and an emotionally acting-out group. The results of this study lend further support to the efficacy of combining two objective assessment inventories for the purposes of multiaxial diagnosis as prescribed by the DSM-III.

Telecommunications and the hospital.

Telecommunications is a rapidly changing industry which currently offers the hospital administrator an opportunity to cut costs in the short run while providing for future needs. Like any important resource, this technology must be managed by skilled personnel who can recommend solutions consistent with the hospital's business philosophy and strategic plan. Three specific actions were recommended: first, consider modernizing your PBX; second, train an inhouse person or hire a telecommunications analyst to conduct the review of available systems and options and to supervise installation and operation; and third, when replacing computer systems and PBXs, develop a plan which will provide for current needs and future growth.

Comparative prevalence of Babesia microti and Borrelia burgdorferi in four populations of Ixodes dammini in eastern Massachusetts.

We determined the prevalence of Babesia microti and Borrelia burgdorferi in four populations of Ixodes dammini in eastern Massachusetts. The Feulgen's reaction was more sensitive than the Giemsa method for detecting salivarian Babesia. A combination of darkfield and direct-fluorescent-antibody examination proved more sensitive than either method alone for detecting spirochetal infection. The prevalence of spirochetes was greater than the prevalence of Babesia in each of the tick populations studied. Overall, 24% of nymphs and 47% of adults examined were infected with spirochetes; in contrast, 11% of nymphs and 14% of adults were infected with Babesia. The difference between the spirochetal and babesial prevalence was greatest in the most recently infested site. The rising incidence of Lyme disease, as compared to the stable incidence of human babesiosis, may result from the relatively greater abundance of Bo. burgdorferi infected I. dammini ticks in newly infested locations.

The use of a computerized system in defining the therapeutic spectrum of antidepressants.

Few substantial improvements have been made in the development of detailed and logical methodologies and techniques for analysing, documenting and presenting information acquired during clinical trials on psychotherapeutic agents. This results in a loss of information and makes it impossible to compare or combine data derived from different clinical trials, even on the same drug. We believe that a more extensive utilization of the large amount of data collected in clinical trials may be derived through the use of computerized systems for analysing and documenting such data. With the purpose of studying computer-based data documentation techniques, at the Institute of Clinical Psychiatry of the University of Pisa there has been jointly established by the US National Institute of Mental Health and the University of Pisa a Centre for Clinical Psychopharmacology Data Documentation (CCPDD). The major objective of the CCPDD is to enhance the quality of clinical trials of psychotherapeutic agents by improving computer-based data documentation techniques which allow more understandable, comprehensive, exhaustive and standardized data analysis and data presentation. The CCPDD operates the Biometric Laboratory Information Processing System/Data Bank for Psychopharmacology (BLIPS/DBP), a fully computerized system designed to process any type of assessment data in standardized ways providing the basic documentation for each clinical trial.(ABSTRACT TRUNCATED AT 250 WORDS)
