Value of high-frequency US for preoperative assessment of skin tumors.

The purpose of this study was to evaluate the accuracy of high-frequency ultrasound (US) in the preoperative assessment of skin tumors. A US scanner with a 20-MHz probe was used to visualize and evaluate 70 skin lesions (38 clinically suspected melanomas and 32 suspected basilar cell carcinomas [BCCs]) before surgical resection. A US morphologic study and a Doppler analysis of vascularity were performed for each tumor. Of the 70 tumors, 62 were clearly visualized, including 19 melanomas, 12 nonmalignant nevi, and 31 BCCs. Most lesions were hypoechoic. In 13 of 19 proved melanomas, the difference between the histologic and US measurements was equal to or less than 0.2 mm. Vessels were visualized in melanomas with thicknesses greater than 3 mm. All BCCs were visualized, and in 29% of cases of BCC, tumor size at US was greater than that at clinical examination. High-frequency, high-resolution US is a simple, reliable, noninvasive method for accurate preoperative assessment of skin tumor dimensions. This technique allows surgical planning to be adapted and reexcision to be avoided. However, its role is limited in the differential diagnosis of malignant and benign skin lesions.

[Islet cell tumor with diarrhea and diabetes (glucagonoma?) associated with hyperparathyroidism. Long-term results of local-regional treatment with streptozotocin].

A case report of endocrine pancreatic tumour associated with diabetes and hyperparathyroidism is presented. Some arguments are in favour of a glucagonoma. Despite the absence of beta-cells in tumour, an exceptional result was obtained by use of Streptozotocin. This case brings up the relations between endocrine pancreatic tumours and parathyroid, and may be included in polyglandular adenoma syndrome.

[Surgical treatment of sequelae of Basedow's orbitopathy].

Sequellae of Graves' orbitopathy often need a functional or cosmetic correction. The different procedures are reviewed with special commentary (orbital decompression, surgery of oculomotor disturbances, lid retractions and orbital fat protrusion). A modified procedure for levator lengthening is described.

[Evaluation of 242 laparotomies for hepatic metastasis performed at the Institut Gustave Roussy over 5 years].

Two hundred and forty two laparotomies for single or multiple isolated liver metastases (LM) have been performed in 5 years. One hundred and seventy three were performed for LM from colo-rectal cancers and after abdominal exploration it has been carried out 46 hepatectomies, 92 implantations of catheter for intra-arterial hepatic chemotherapy (IAHC) and 35 abstentions. Sixty nine were performed for LM from miscellaneous origins and it has been carried out 32 hepatectomies, 18 implantations of catheter for IAHC and 19 abstentions. The main conclusions of this study are: 1) every preoperatively estimated unresectable LM has never been resectable at abdominal exploration. 2) When it was anticipated to perform an hepatectomy, it was possible to do it only in two-third of the cases, the main reason of non resectable LM was intra or extra hepatic diffusion. 3) When the LM has been preoperatively estimated as un-resectable, the metastatic disease was isolated in the liver in two third of the cases and it was almost always technically possible to place an arterial catheter (108 instances in 110 cases). 4) In 3% of the cases, pre-operative images of LM have not been confirmed at surgery and the diagnosis has been made on biopsies with frozen sections which, must always preceded local hepatic treatment.

Fine needle aspiration of the liver and pancreas with ultrasound guidance.

The experience of the Institut Gustave-Roussy in the diagnosis of hepatic and pancreatic lesions by fine needle aspiration (FNA) is reported. Totals of 116 consecutive percutaneous ultrasound-guided FNAs of the liver and 27 of the pancreas were performed without complication in patients with ultrasonically suspected neoplastic lesions. In 12 cases, the material was not suitable for diagnosis. In the liver, 97 cases were correctly diagnosed and confirmed by follow-up. Immunohistologic studies were helpful in distinguishing primary liver tumors from other malignancies. One false-positive result was reported. In the pancreas, malignancy was detected in 17 cases. Cytology alone provided the correct tumor diagnosis in 15 cases: 10 primary carcinomas, 2 endocrine tumors and 3 metastases. The sensitivities of FNA in this study were 87.6% for the liver and 85% for the pancreas, similar to those reported in other series.

[A case of regressive Riedel's thyroiditis (author's transl)].

Riedel's struma is one among the locations of an invasive fibroinflammatory and sometimes multifocal process, of unknown etiology. This disease is extremely rare. The authors report a further case in a 41 year old man who had been developing a woody goiter for 3 months. Laboratory investigations showed normal inflammation tests, high rates of antithyroid antibodies and mild hypothyroidism. A surgical operation only consisted in a biopsy, which revealed typical histological features. The only treatment used was a suppletive therapy. During the evolution, antithyroid antibodies progressively decreased, hypothyroidism moderately increased and, chiefly, the goiter completely disappeared after 18 months. To our knowledge, regressive evolution was reported in very few cases after corticoid therapy, but never spontaneously.
