Evaluation of canines for accelerant detection at fire scenes.

In recent years, canines have been successfully used in fire investigations to detect accelerant residues. We set out to determine the lower limits at which canines could reliably detect potential accelerants. Measured amounts ranging from 10 to as little as 0.01 microL of gasoline, kerosene, and isopars were applied to preselected spots along a continuous sample path (25 to 40 feet long) made out of burned and unburned wood or nylon carpeting strips at the testing site. Two canines were led past this sample path at least three times and positive alerts and negative responses were recorded. Both dogs were generally able to alert on spots containing 0.01 microL or more of all three accelerants, at or beyond the purge and trap recovery and gas chromatographic detection method employed. The canines did alert occasionally on background, especially that containing traces of styrene residues, either purposely added in specific amounts or formed upon partial pyrolysis of carpeting material. The dogs alerted on sites containing 0.1 to 1.0 microL of freshly applied gasoline or kerosene placed at actual heavily damaged fire scenes, but were less successful on samples containing smaller amounts.

[Therapy of choice in complicated diverticulitis?].

Between 1988 and 1992 we treated in our hospital 79 patients with a complicated diverticular disease. We analysed and compared two groups (group A 42 cases January 1988 to February 1991/retrospectively, group B 37 cases March 1991 to December 1992/prospectively). We studied our operative procedure in respect to the postoperative morbidity and lethality, the number of performed and persistent stomata and the mean length of stay. The complications of diverticular disease in both groups were similar. Since March 1991 we performed primary resection with primary anastomosis in 89% of the patients, three-stage resection or primary resection with an anastomosis and a proximal colostomy was no more done. The Hartmann procedure was rarely chosen (only in 4 of 37 patients). In spite of forcing the primary resection with a primary anastomosis in group B, the complication rate (local and general) decreased from 35.7% to 29.7%, the lethality rate from 9.5% to 2.7%. The number of performed stomata showed a great difference with 22 in group A against 4 in group B. The mean length of stay was 31 versus 22 days. Therefore we consider primary resection with an anastomosis for the best therapy in complicated diverticular disease.

[Laparoscopic diagnosis in suspected acute appendicitis].

Since introduction of laparoscopic appendectomy we have the possibility to examine the whole abdominal cavity and not only the ileocaecal region and right adnex. The aim of the study is to find out if there is an advantage in laparoscopic diagnostics compared to conventional laparotomy for suspected acute appendicities. We compared prospectively all patients who underwent laparoscopy for acute right lower abdominal pain between August 1991 and March 1993 with a comparable group retrospectively analyzed who underwent conventional appendectomy in 1989. In both groups 20% of patients had a normal appendix. In 1% of the laparoscopically operated patients we couldn't find any pathological findings, in 12% of the conventionally operated group we couldn't find an accurate diagnosis. The average operation time of laparoscopically operated patients without acute appendicities was 20 minutes shorter compared to conventionally operated patients. We found identically results for the duration of hospital stay. The median hospital stay for conventionally operated patients was 6.6 days, for laparoscopically operated patients 4.7 days. The complications were in both groups 1-2%. We conclude that in laparoscopy the diagnostics are more reliable, and with a diagnostic accuracy of almost 100% the unnecessary appendectomy with a higher morbidity would not be necessary in 12% of patients.

[Ambulatory surgery--a sensible future perspective].

Increasing costs in health system, reduction of bed capacities and lack of nurses force to search for solution. To prove that day surgery may be a kind of solution, a pilot project was analysed during 8 months. This project was fully integrated in normal clinical management. Operated patients (n = 100; hernias, varicosis, proctological and other indications), preoperatively selected by specific criterias, and the family doctors were questioned retrospectively of their experiences. The results were very encouraging. Moreover we stated that day surgery is cost-covering. So day surgery is to consider as an important factor to cost-reduction.

Topography and source analysis of brain activity associated with selective spatial attention and memory search.

We investigated the topographical aspects of the ERP reflections of visual spatial attention and memory search. Spatial attention was found to enhance the amplitudes of the P1 and N1 deflections. The brain activity in the P1-N1 latency range could be modeled with a single moving equivalent dipole, or alternatively with two stationary dipoles in a spatio-temporal dipole model. The dipoles were located in mesial and lateral posterior brain regions. Similar dipole solutions were obtained for ERPs to attended and unattended stimuli. Increasing the memory search requirements of the task resulted in an increase of late negativity, which was topographically distinguishable from the P3 component.

Speed reduction on through roads in Nordrhein-Westfalen.

The paper describes a program of experiments for speed reduction on 28 through roads in Nordrhein-Westfalen. In the process, different measures of construction, design, and traffic control were examined to determine their usefulness to reduce speed and to improve road safety. The need for systematic evaluation of a catalogue of measures partly resulted in unsatisfactory design conceptions for the through roads. The evaluation of the effects of the program has been carried out by use of before-and-after-studies of large extent. The speed measurements show that a distinct speed reduction can be obtained only from selected physical improvements. Measures based on optical effects and traffic signs were found to have very little effect on vehicle speeds. Furthermore, in light of the relatively small number of accidents that occur on the selected facilities, it was not possible to establish the beneficial impact of each specific control measure with statistical certainty. However, the results of the experiment have had an influence on the current definition of new design standards for main urban streets in Germany.

[Alcohol-induced hypoglycemia in type I diabetic patients].

The hypoglycemic effect of alcohol, probably caused by inhibition of gluconeogenesis in the liver, is well known. However, accurate data on the effect of usual amounts of alcohol on the blood sugar in type 1 diabetics are not available. We therefore investigated the effect of an evening dose of alcohol on the blood sugar in 23 male type 1 diabetics (age 32.8 [20 to 64] years, duration of diabetes 11.5 [0.5 to 38] years, Broca index 0.98 [0.83 to 1.34] HbAlc 8.4 [5.8 to 12.2]%). On one day, the diabetics were given 0.56 g ethanol per kg body weight to drink in the evening, corresponding to about one liter of diabetic beer, on another day, the same volume of mineral water. In addition, eleven of the 23 patients received half the amount of alcohol on a third evening. Between 7 and 11 o'clock in the morning after alcohol consumption, blood glucose levels were significantly lower. The number of hypoglycemic episodes in the morning of the day following alcohol administration was appreciably greater. Usual amounts of alcohol consumed after the evening meal thus increase the risk of hypoglycemia the following morning. Diabetics should be instructed accordingly.

Specific immuno-detection of benzoate-para-hydroxylase with antibodies raised against synthetic peptides.

As a model system for the industrial use of fungal cells in the enzymatic conversion of chemicals, the parahydroxylation of benzoate was studied. To increase the amount of benzoate-para-hydroxylase (BPH, EC 1.14.13.12.) in the cell the gene coding for the enzyme (bphA) was cloned and expressed in Aspergillus niger. Detection of the enzymatic activity of the protein was not reproducible. It was decided to raise an antiserum for immuno-detection purposes. Sufficient benzoate-para-hydroxylase for immunization could not be obtained; therefore the synthetic-peptide strategy was used. We demonstrate that synthesis of antigenic determinants, can be useful in the production of highly specific reagents for the detection of proteins. The availability of monospecific polyclonal sera opens new possibilities in functional studies and purification of benzoate-para-hydroxylase.

[En bloc esophagectomy--when should the digestive tract be reconstructed?].

In a prospective study direct reconstruction of the esophagus was compared to reconstruction 48-72 h after esophagectomy. In both groups (26/24) transthoracic en-bloc esophagectomy was performed. During the same time period of the study another group of 45 patients had transmediastinal esophagectomy and direct reconstruction and this group was also used as comparison. There were no differences concerning postoperative complications (26.9%; 29.1%; 22.2%), postoperative 30-days mortality (0%; 4.1%; 2.1%), and hospital mortality (3.2%; 4.1%; 4.2%). Thus reconstruction with delayed urgency does not lead to a further decrease of risk; on the other hand there is also no increase of risk and therefore it can be included in the spectrum of procedures of esophageal surgery.

Isolation and molecular characterisation of the benzoate-para-hydroxylase gene (bphA) of Aspergillus niger: a member of a new gene family of the cytochrome P450 superfamily.

The gene coding for benzoate-para-hydroxylase (bphA) of Aspergillus niger was cloned using differential hybridisation techniques and complementation of mutants deficient in this enzyme activity. The nucleotide sequence of the gene was determined, the presence of two introns was shown and the transcription start and termination sites were determined. The structure of the mRNA upstream from the long open reading frame (ORF) is unusual. It contains two small, overlapping ORFs whose function is unknown. Comparison of the deduced amino acid sequence of the protein with the sequences present in the databanks, indicated a significant similarity of BPH to the superfamily of cytochrome P450 enzymes. Further analysis revealed that this protein is a member of a new P450 gene family designated P450LIII. The gene is designated CYP53. To increase the BPH activity of A. niger, multiple copies of the bphA gene were introduced into the genome of a recipient strain by transformation. Although increased intracellular levels of the BPH protein could be detected, the BPH enzyme activity was decreased, suggesting titration of another essential component.
