Skin lesions as a sign of subacute pentachlorophenol intoxication.

Pentachlorophenol (PCP) and its sodium salt are frequently used in wood preservatives. Little is known about the effects on man when being chronically exposed. Only vague skin symptoms, such as rashes, acne and cutaneous infections were described. We present two cases of pemphigus vulgaris with a known non-occupational chronic PCP exposure. The clinical course and the titer of pemphigus antibodies roughly correlate with the PCP levels in serum. In one case of chronic urticaria the exacerbations also run parallel to the PCP serum levels and increased anti-skin antibodies, without any manifestation of pemphigus vulgaris. The role of PCP as one of the causes provoking pemphigus vulgaris and chronic urticaria with raised anti-skin antibodies is discussed.

[Study of the visual field using automated static perimetry following panretinal photocoagulation in the diabetic].

Twelve diabetic patients presenting a preproliferative or proliferative retinopathy were the subjects of our study. We studied the modifications of the visual fields, analyzed by computerized perimetry, after panretinal photocoagulation (PRP). For every patient, two techniques of PRP were applied, one on each eye: C: conventional scattered treatment, G: cluster technique. Computerized perimetry was done within 10 days prior to PRP, and 4 to 8 weeks following it. We used the Octopus 500, programs 36 (0 to 30 degrees) and 44 (30 to 60 degrees). It appears that, before any treatment, there is a loss of retinal sensitivity excepted the more central zone. Concerning the loss of sensitivity, although the number of laser burns, is greater in group G, there is no statistical significative difference between the 2 groups. Nevertheless, it seems to be more heterogeneous in group G. However, we have not studied the visual field modifications after confluent PRP using up to 5,000 500 micron laser burns. Although there is a loss of retinal sensitivity after PRP, none of our patients complained of it. PRP seems to us to be the only treatment able to prevent the natural evolution of preproliferative or proliferative diabetic retinopathies.

[Value of the lateral chest x-ray in the diagnosis of traumatic pleural effusions].

Forty patients were admitted to a hospital emergency department after closed chest injury. Chest X-rays, a-p and lateral, with low and high voltage, were performed in a supine position. The aim of the study was to show the interest of lateral films in the diagnosis of post-traumatic pneumo- and hemothoraces. A special technique, using a double-drawer plate holder, gave a good quality radiograph in emergency conditions without danger for patients with severe trauma. In 30 of the patients, chest radiographs confirmed pleural gas or blood collection. In ten of these, the diagnosis could only be made after radiographic examination. In nine other cases, radiographs showed an absence of hemo- or pneumothorax. Lateral views were very important in confirming a hemothorax and appreciating its volume, as well as in showing up partial pneumothoraces which could not be seen on a-p views. The interest of performing a lateral film in all the cases of chest injury is pointed out.

[An improvement in the performance of emergency frontal- and lateral-view thoracic radiography].

A special technique giving good quality front and lateral chest roentgenograms in emergencies with no danger for the critically ill patient is described. The system consisted of a double-drawer plate-holder: one was passed at the back of the thorax, the other then being perpendicular to it. Placing the patient on an X-ray transparent stretcher made the technique easier. Several people were required to gently move the patient while the plate-holder was put in place.

[An aid in the diagnosis of cardiac rhythm disorders in anesthesia and intensive care].

Exact diagnosis of tachycardia was sometimes difficult to do as P waves were not easy to see on ECG. A simple device is described, with which ample P waves were recorded in patients with a central venous catheter. This catheter was used as conductor of cardiac electrical activity. It proved simpler and less aggressive for the patient in comparison with other techniques (intracardiac and oesophageal recording).

Characteristics of physician addicts.

This article attempts to characterize the physician addict and to challenge the usual sociological or psychological explanations of differences and similarities among addicts. Using available data on Quebec physicians, two types of physician addicts are described according to the "two diseases model." In its first form the disease is characterized by the search of euphoria, and as such is considered to be highly contagious and pernicious, while the other is rarely contagious and relatively less pernicious.

[Visual and tactile agnosia].

A patient presented with visual and tactile agnosia due to a spontaneous left occipito-temporal hematoma. Major memory and spatial orientation disorders were also noted, but language, gestures, auditory and olfactory perception, and interior visual imagerie were unaffected. A review of the literature since 1970 found 6 cases of an association of this type among 17 patients with visual agnosia. There appears to be no relationship between the presence of tactile agnosia and the global or partial character of the visual agnosia, the severity of memory disturbances, the presence of visuoverbal disconnection or visual imagery disorders. In contrast, these cases differed in that the lesions were more extensive and extended beyond the internal occipital regions: they were widespread in 3 cases and in the left internal occipital and parietal region in 1 case. In the patient reported in this paper there was a wide lesion of the posterior white matter of the left hemisphere.

[Aerobic capacity of 6 to 17-year-old Quebecois--20 meter shuttle run test with 1 minute stages].

Norms for the 20-m shuttle run test (with 1 min stages) for the functional and maximal aerobic power (FMAP) are given by age or academic year for boys and girls from 6 to 17 years old for the province of Quebec in May 1981. The sample, 3669 boys and 3355 girls, was selected by stratum according to the scholar population of each region in Quebec. Weight and height for both sexes are similar to those obtained in a recent Canadian study (CAHPER, 1980). The FMAP or the 20-m test results (as a function of age and sex) varies like other published FMAP indices. This supports the validity of the norms of the 20-m test, a test that various advantages in school testing (group testing, progressive protocol and valid test).

A possible new HLA-DR allele.

In routine screening of anti-HLA DR reagents, serum 901 was obtained from a woman of negroid origin ten days after delivery of a second child. The spouse was also of black origin. This serum contained polyspecific HLA A and B antibodies. After platelet absorption it reacted with the B cells of 10 out of 119 European Caucasoid panel donors (8.4%) typed for DR1 to DRW10 and for MT1, MT2. Each of these ten donors had only one recognized DR antigen, the other was "blank." Serum 901 gave negative reactions with the homozygous typing cells (HTC) DW1 to DW8. In 18 normal informative Caucasoid families, serum 901 recognized one HLA haplotype in one (or both) parents and segregated with this haplotype in one or more than one child. In one family in which both parents reacted with serum 901, two children were homozygous for this marker and reacted as HTCs. One of these HTCs typed as DW9 and was found to be identical to a DW9.HTC (8W207).
