[Suspicion of Down's syndrome based on patients obstetrical history and clinical data (author's transl)].

At the university women's clinic Zurich between 1963 and 1974, 29 babies in a total of 32255 babies were born with Down's syndrome. 11 of these had a cardiac vitium and 2 an intestinal malformation. 5 babies died during the first year of life. This high morbidity demands an analysis of the criteria in the obstetrical history and clinical picture in order to reach a diagnosis in those cases which would otherwise not be diagnosed if one used the normal criteric for amniocentesis in early pregnancy. Maternal age was the only relevant factor to emerge. In 6 pregnancies a tentative diagnosis for poor intrauterine growth was made in which only one case showed a retarded growth of the bipariental diameter. In all 7 cases in which cephalometry was performed after the 24 week of pregnancy, the concurrent hormonal parameters showed significantly lowered oestriol levels with normal HPL levels. From this information we draw the conclusion that transabdominal amniocentesis should be performed in in pregnancies with permanently low oestriol levels and normal HPL levels; especially in cases which clinically appear "small for dates" and for which there is no other explanation such as anencephaly for the low maternal oestriol levels.

[On the metabolic heterogenity of the cartilage tissue (author's transl)].

Autoradiografic studies with 3H-thymidine, 3H-proline and 35S-sulfate in different cartilage tissues from rabbits and rats (articular and epiphyseal cartilage of the knee; costal, xiphoid and tracheal cartilage) were performed. The following results were obtained: The proliferative zone of the epiphyseal plate of young and adult animals is limited to the superior part of the columnar cartilage. The articular cartilage shows no proliferative zones but single S-phase-nuclei in all layers seldom were found. In the adult animals a decrease of DNA-synthetising chondrocytes in all investigated cartilage tissues and a narrowing of the proliferative zone could be detected. 3H-proline and 35S-sulfate in proliferating chondrocytes of the bone cartilage border line and in chondrocytes of the subperichondrial area increasingly were incorporated. Possible causes of the regional differences in the uptake of the used precursors were discussed and the significance of the metabolic heterogenity of the cartilage tissue for aging and pathological processes was pointed to.

[Comparative morphometric investigations for the determination of the degree of stenosis of the femora artery (author's transl)].

Planimetry, point-counting-technique and gravimetry were tested for their suitability for determining the degree of stenosis on the same material, the arteria femoralis dextra out of 103 unselected obduction cases. Pilot studies on the circle showed the planimetric method to be most accurate. On transverse sections of the femoral arteries the results of the point-counting-technique deviate significantly stronger from the planimetric test results in comparison with those of the gravimtric method. The point-counting-technique shows the smallest, the gravimetric method the highest time expenditure. Attention is drawn to the necessity of taking into account the plane of the intact intima when determining the degree of stenosis. Also in the case of high-grade stenoses there are frequently no clinically conspicous disturbances of the blood flow.

[First experiences with amniocentesis from genetic indications].

Genetically indicated amniocentesis was performed in 93 patients in the first half of pregnancy and in 36 in the second half. To the common and recognized indications several obstetric indications were added. Six (4.7%) fetal abnormalities were diagnosed by karyotyping and determination of alpha1-fetoprotein (AFP), i.e. a child with trisomy 21 and a boy with XXY Klinefelter syndrome, both in mothers aged over 40, a partial trisomy 3 in a pregnancy with a balanced anomaly, 2 boys with X-linked hereditary disease and an anencephalic. In 4 cases (3.1) abortion was performed at the parents' request. Three pregnancies ended in spontaneous abortion. In 54 cases in which amniocentesis was performed in the first half of pregnancy the babies have now been born. One was found to have a congenital heart defect and the remainder were healthy. In the cases which had undergone genetic screening during the second half of pregnancy 3 neonates displayed anomalies not detectable by karyotyping or AFP determination, one of which was hydrocephalus. The abortion rate was 0.8%.

[Expermental contribution on the genesis of arteriosclerosis caused by hypertension].

Autoradiographic tests carried out on rats with renal hypertension using 3H-proline resulted in an acclerated collagen synthesis by media cells of aorta and coronary arteries. Electronmicroscopically an increased content of collagen fibers and an enrichment of ruthenium-red-positive substances in the extracellular space were found. The 35S-sulfate-incorporation in aorta and coronary arteries of animals with hypertension is also increased. These changes in the extracellular space of the vascular wall have an atherosclerosis promoting effect, probably caused by a distrubance of the permeability.

[A case of amniotic fluid embolism (author's transl)].

A case of lethal amniotic fluid embolism is reported. The microscopic findings are documented and the patho-physiologic course of events is reconstructed. The unsuccessful treatment is compared with recommendations in the literature and critically evaluated.

[A-like effect of a proteinpolysaccharide containing extract from bovine cartilage].

In regard to the undecided immunological role of protein polysaccharides (of the glycosaminoglycan type) for pathogenesis of connective tissue diseases a protein polysaccharide-containing extract from bovine nasal cartilage was tested by the absorption method, precipitation and immunization. An A-like effect of the extract was found.

[Investigations on the influence of Rh immunoglobulin prophylaxis on the immune response to postpartum rubella vaccination (author's transl)].

A preliminary investigation showed that patients with rubella HHT antibody titres of 1:8 or greater did not show a significant rise in the antibody titre following rubella vaccination. The rubella antibody titre was determined in 651 obstetric patients. Of these, 43 (6.6%) had no significant antibodies to rubella (HHT less than 1:8) and were included in the present investigation. Patients in Group A received 0.5 ml. of the rubella vaccine Meruvax on the fifth postpartum day. Patients of Group B(Rh negative and Rh positive) received 250 mug anti-D in a 16% gammaglobulin solution intra-muscularly 48 hours postpartum and the rubella vaccination 3 days later. Three weeks following the rubella vaccination the mean geometric rubella antibody titre had risen +/- 1 Standard deviation to 19.6 +/- 7.7 in Group A (17 patients) and to 18.0 +/- 6.3 in Group B (12 patients). Six weeks following the rubella vaccinations Group A (19 patients) showed titres of 61.7 +/- 2.9 and Group B (14 patients) showed titres of 70.0 +/- 2.6. There was no statistically significant difference (greater than 0.5). The conversion rate in both groups was 100%. Patients can therefore be vaccinated against rubella in the postpartum period even though they will receive a concomitant prophylaxis with Rh immunoglobulin.

[Alpha-fetoprotein: its significance in the monitoring of "at risk" pregnancies (author's transl)].

Having plotted a curve showing the normal values of Alpha-Foeto-protein (AFP) concentration in maternal serum and amniotic fluid, its significance in the monitoring of "at risk" pregnancies was examined and critically judged in the light of statements found in the literature. According to the results gained AFP determination in maternal serum or amniotic fluid can only be used with reservation as a monitoring test in "at risk" pregnancies. In particular in Rhesus Incompatibility it was impossible to establish the degree of foetal Haemolysis or to determine the best time for premature induction. In addition an intra-uterine death could not be predicted with certainty.
