[Experimental investigations on the regeneration of aortic endothelial cells. Automatic and visual evaluation of autoradiograms (author's transl)].

Autoradiographic studies on plane preparations using 3H-thymidine and pulse cytophotometric investigations on scrape preparations of aortic endothelium from 34 rabbits have brought the following results: By means of multiple 3H-thymidine-injections the number of labelled endothelial cells is higher in young growing animals than in older rabbits; pulse cytophotometrically we did not find any indication of polyploidization processes; from that we conclude a decrease of the growth fraction with advancing age. In comparison to visual counting the automatic image analysis is a highly effective method for the evaluation of autoradiograms. After multiple 3H-thymidine applications the mean blacking intensity of the labelled cell nuclei is lower in 2 months old animals than in 12 months old rabbits; that may mean an increase in the S-phase duration of the proliferating endothelial cells of the older animals. In the area with branching sites there are more 3H-thymidine-labelled endothelial cells than in the neighbouring vascular segments. The significance of these findings for the preferential incidence of arteriosclerosis in this area is discussed.

[Clinical experiences with prostaglandin E 2 and F 2 alpha in the termination of pregnancy and labor induction in intrauterine fetal death].

In 34 cases of therapeutic termination of pregnancy in the second trimester and in 26 cases of fetal death in utero up to the 43rd week of pregnancy, expulsion of the uterine contents was achieved by means of transabdominal-intraamniotic and intravenous administration of prostaglandin (PG) E2 and PGF2lalpha. The average expulsion time was 14 1/2-16h. The following respective doses were needed: for intraamniotic use, 5-25 mg PGE2 and 40-120 mg PGF2alpha, and for intravenous use 5.8-7.3 mg PGE2 and 43.3-52.3 mg PGF2alpha. Two patients required treatment for side effects. These results are compared with existing data from the literature and the clinical application of various prostaglandins is discussed.

[Stream mechanics and endothel regeneration (author's transl)].

The regeneration of rabbit aortic endothelium was studied by autoradiography using 3H-thymidine. A decrease in the 3H-thymidine labelling index and in the mitotic rate with advancing age were found but no significant differences in the endothelial cell turnover between aortic segments with and without flow-mechanical stresses.

[Contribution to the treatment of threatened abortion].

460 cases of threatened abortion were divided into 3 different treatment categories. Group A was treated with a combined antihistamine and antispasmodic, and Group B with tranquilizers and synthetic gestagens. Group C consisted of patients who refused therapy. The percentage of delayed abortions in group A was 79.3%, in group B 84.7% and in group C 54.8%. According to the x2-test, the difference between the treated and untreated groups is statistically significant. The mode of action of the prescribed drugs is substantiated and their effectiveness was compared with published data on other therapies. Complications of pregnancy, at delivery and during the puerperium were not more prevalent than in normal pregnancies. However, the percentage of premature births (22.9%) and of perinatal mortality (5.4%) was strikingly high. Accordingly, successfully treated threatened abortions are to be followed up as "risk" pregnancies and closely supervised as such. It is especially important to avoid a premature birth. The frequency of congenital defects and functional disturbances was 11.9%; their existence is independent of the type of therapy. Since the defects are usually of minor consequence and reversible, we advocate treatment of the threatened abortion.

[Comparative study on the question of systemic chemoprophylaxis following gynecological surgery].

The incidence of delayed wound healing and urinary tract infections was studied in 2 randomized groups of post-operative gynecological patients, one of which was given systemic antibiotic prophylaxis and the other not. 143 women were given Bactrim in a standard dose of 2 x 2 tablets or ampoules daily, commencing preoperatively during and continuing until the eighth postoperative day. The control group received placebos parenterally and orally for the same period. The incidence of complications in wound healing, of subfebrile and febrile states, urinary tract infections and micturition difficulties following removal of the catheter was significantly lower in the group who had undergone antibiotic therapy. 83% of this group, compared with 68% in the control group, were discharged after the normal hospital stay. In 51% of the control group a chemotherapy or antibiotic course had to be given secondarily to combat infection, the commonest indications being urinary tract infections or pyrexia for several days. The drawbacks were side effects of the systemic chemoprophylaxis, such as skin eczema (6%) and gastrointestinal disturbances, as well as a tendency to infections with problem bacteria requiring treatment with reserve antibiotics. It is concluded that routine postoperative antibiotic prophylaxis, despite its favourable effect on infections morbidity as a compromise between optimum treatment for the individual and general medical interest, must be prescribed on an individual basis.

HCG, HPL, oestradiol, progesterone and AFP in serum in patients with threatened abortion.

The predictive value of various biochemical methods for monitoring early risk pregnancies has been compared in 65 cases of threatened abortion. Estimation of human chorionic gonadotropin (HCG), human placental lactogen (HPL), progesterone, oestradiol and alpha-fetoprotein (AFP) in serum were made by radioimmunoassays. Values below the normal range predicted abortion in 79, 81, 89, 92 and 38 per cent of patients, while normal values confirmed continuation of pregnancy with an accuracy of 71, 61, 60, 68 and 30 per cent respectively. Thus predictions from oestradiol and progesterone were at least as reliable as those from the protein hormones, while AFP proved to be unsuitable for this purpose. Combination of two variables gave even more reliable results. Due to individual and diurnal variation, however, abortion in the third and fourth month could not be definitely assumed at values above 5 IU HCG/ml, 5 ng progesterone/ml or 200 pg oestradiol/ml.

Ultrasound and biochemical findings in intrauterine growth retardation.

An attempt was made to make an early diagnosis of intrauterine growth retardation due to chronic placental insufficiency using ultrasound cephalometry and biochemical tests of placental function. We examined 83 hospitalised patients in whom there was a risk or suspicion of chronic placental insufficiency. For each patient an average of 4 determinations were made of head diameter, 10 of estriol and 3 of pregnandiol urinary excretion, 6 serum HPL and 7 of heat stable alkaline phosphatase (HSAP). Fetal growth retardation was assumed if the last 2 determined head diameters were below the normal curve with the same tendency and with term being well predicted. Biochemical parameters were considered to indicate pathological changes if they were 95% below normal values, with 2 below this range or 3 continuously falling below the normal level. Infants were assessed after birth both neurologically and somatically according to DUBOWITZ. Small for date infants were those whose birth weight was below the 10th percentile (LUBCHENKO). This was found to be the case in 15 out of 83 newborns five of whom were younger than 37 weeks. These 15 could be diagnosed before birth with various degrees of certainty. For estriol and HPL this was 67% or 53% and these two parameters were found to be the most valuable. Cephalometry was found to be less valuable with 20%, pregnandiol levels with 9% and placental HSAP with 33%. Hence it is recommended to perform serial determinations of estriol and HPL in the third trimester of pregnancy in all patients with histories indicative of fetal growth retardation and in those in whom the uterus appears small for date.
