Primary squamous cell carcinoma of the endometrium.

A case of squamous cell carcinoma of the endometrium which fulfills Fluhmann's criteria is presented. This case is reviewed together with additional 25 cases which had been already reported in various papers in the literature. The pathogenesis and the etiologic factors associated with primary squamous cell carcinoma of the endometrium are discussed. A review of the literature indicates that this rare malignancy is found in older patients, is highly malignant and carries a poor prognosis as myometrial invasion, and local extension or metastasis is found in 80% of the patients.

Evolution of malignant lymphoma in agnogenic myeloid metaplasia.

Two young arab patients are described in whom malignant lymphoma developed within less than 1 year of the diagnosis of agnogenic myeloid metaplasia. Both patients showed a satisfactory response to combined chemotherapy. One of them died of hepatitis B at 10 months and the other is alive and in clinical remission 25 months after initial diagnosis. These observations demonstrate the close relation between myeloproliferative and lymphoproliferative syndromes and illustrate the diversity of malignant lymphoproliferative disorders into which agnogenic myeloid metaplasia may evolve in the course of disease. Our experience also demonstrates the ease with which some patients with an 'end stage' myeloproliferative disorder may respond to standard chemotherapy designed for the treatment of malignant lymphoma.

Colchicine does not interfere with the antiphosphaturic effect of 25(OH) vitamin D3 in the rat.

The effect of colchicine pretreatment on the antiphosphaturic action of 25(OH)vitamin D3 (25[OH]D3) was examined in parathyroidectomized (PTX), parathyroid-hormone (PTH)-infused rats by acute clearance studies. The following groups were studied: group 1, colchicine-pretreated PTX rats; group 2, colchicine-pretreated PTX rats receiving PTH infusion; group 3, colchicine-pretreated PTH-infused PTX rats receiving intravenous 25(OH)D3 with pretreated PTH-infused PTX rats serving as controls. The effect of 25(OH)D3 on PTH-activated renal adenylate cyclase [AC] from colchicine-pretreated rats was studied in vitro. In group 1, fractional excretion of phosphate (Cp/CIn) after colchicine was 0.102 +/- 0.011 vs. 0.029 +/- 0.005 in controls (p less than 0.001). In group 2, PTH increased Cp/CIn from 0.074 +/- 0.026 to 0.184 +/- 0.028 (p less than 0.05) in colchicine-pretreated animals and from 0.018 +/- 0.006 to 0.213 +/- 0.034 in controls (p less than 0.0025). In group 3, 25(OH)D3 decreased Cp/CIn from 0.344 +/- 0.022 to 0.235 +/- 0.026 (p less than 0.005) and UcAMP from 103.7 +/- 12.4 to 69.0 +/- 9.6 (p less than 0.025). In vitro PTH activation of AC was blunted in colchicine-pretreated rats; however, the response to 25(OH)D3 was intact. Histological changes compatible with microtubule disruption were found in proximal tubules of kidneys from colchicine-pretreated rats. These results show that colchicine alters the response to PTH but it does not affect the antiphosphaturic action of 25(OH)D3, suggesting that this action is not mediated via cytoplasmic microtubules.

IgM nephropathy: morphological study related to clinical findings.

This study describes 10 cases of IgM nephropathy in whom the main morphological findings consisted of diffuse mesangial deposition of IgM and varying degrees of mesangial cell proliferation. In addition, focal segmental sclerosis was present in 1 patient and global sclerosis in another. An ill-defined electron-dense deposit was seen within the mesangial area in 1 case. Except for 1 patient, who had hematuria only, all suffered from nephrotic syndrome without deterioration of renal function. In view of the constant and characteristic finding of a diffuse mesangial IgM deposition, it is suggested that this form of nephropathy constitutes an entity separate from focal glomerulosclerosis or minimal change disease.

Granular cell tumor (myoblastoma) of the eyebrow.

Granular cell tumor (Myoblastoma) is a tumor of controversial nature that rarely appears in the ocular adnexa. A 37-year-old man with such a tumor in his right eyebrow is reported, and the relevant literature is reviewed.

Renal biopsy in Fabry's disease eight years after successful renal transplantation.

Late graft histology after renal transplantation for Fabry's disease has only once been previously reported. Clinical data and kidney biopsy findings in a case of Fabry's disease before and eight years after successful kidney transplantation are presented. The graft maintains normal function. Graft histology in light microscopy showed no abnormalities reminiscent of the diseased native kidney. Electron microscopy revealed occasional small myelin figures which were present only in the vascular endothelium. Their significance and a review of conflicting reports and opinions from the literature are discussed.

Artificially induced endometrial cycles and establishment of pregnancies in the absence of ovaries.

In eight women with ovarian failure, we induced histologically normal endometrial function during a preparatory cycle consisting of sequential administration of estrogen and progesterone. During a subsequent cycle, endometrial stimulation was synchronized with surrogate-embryo transfer performed on days 16 to 21. Among the eight women, two pregnancies were established by embryo transfer on days 18 and 19. In both women, ovarian tissue was absent, and these patients therefore serve as an in vivo model for the isolated effects of estrogen and progesterone on implantation and maintenance of pregnancy. Treatment with exogenous estrogen was mandatory up to the 11th week of gestation, and treatment with progesterone until the 18th to 22nd weeks. We conclude that it is biologically feasible to simulate the essential hormonal and endometrial milieu of a fertile menstrual cycle and early gestation solely by the administration of estrogen and progesterone. Days 18 to 19 of the cycle are recommended for successful embryo implantation with this treatment program.

Malignant blue nevus.

A malignant blue nevus of the dorsum of the hand and a review of the literature is reported. Histopathologic examination showed the presence of a blue nevus and a cellular blue nevus, as well as a malignant blue nevus. The cellular blue nevus elements of the lesion showed the characteristic prominent neural-type fascicles intertwined with melanocytes. There was no evidence for local invasion or regional lymph node metastases. The patient was free of disease 1 year following wide excision.

Primary fibroxanthosarcoma of the kidney presenting as fever of unknown origin.

Fibroxanthosarcoma is a tumor of soft tissues, mainly effecting the extremities, mediastinum and retroperitoneum. Primary fibroxanthosarcoma of the kidney is extremely rare and has been described in only 4 cases in the past. We present an additional patient, presenting with continuous spiking fever, with rapid deterioration of her condition due to widespread metastases and death within 4 months.

Poststreptococcal neprhitis and acute rheumatic fever in two adults.

In two adult subjects who suffered acute episodes of rheumatic fever, sever kidney involvement developed with proteinuria and renal insufficiency. The clinical and histologic picture was typical of poststreptococcal (infectious) glomerulonephritis. During a follow-up period of more than three years, no clinical or biochemical abnormalities have been detected; thus, it is likely that both patients have recovered from their renal diseases. The coexistence of rheumatic fever and acute glomerulonephritis, which is uncommon even in your age groups, may be observed in adult subjects.

Pericardial meshing: an effective method for prevention of pericardial adhesions and epicardial reaction after cardiac operations.

Cardiac reoperations, particularly for coronary revascularization, are becoming more frequent and carry increased risk of damage to the heart during resternotomy. We experimentally evaluated a pericardial meshing technique to facilitate primary pericardial closure. In 18 mongrel dogs, an 8 by 5 cm pericardial flap was fashioned through a left thoracotomy. A standardized procedure for induction of pericardial adhesions was carried out in all animals. Animals were divided into three groups of six animals each: Group I (control)--the pericardial flap was primarily resutured; Group II--the flap was meshed and then resutured; and Group III--the flap was replaced by a pericardial substitute. Animals were put to death 8 weeks postoperatively and the pericardial space was examined for adhesions and epicardial reaction. The extent of adhesions and epicardial reaction was graded as: 0--none; 1--minimal; 2--moderate; and 3--severe. Both in Group I and Group III severe pericardial adhesions (grade 2-3) and epicardial reactions (grade 2-3) were formed, which obscured the underlying coronary anatomy. In Group II pericardial adhesions and epicardial reactions were none to minimal (grade 0-1) and the underlying coronary anatomy was not obscured. The meshed pericardium was completely regenerated by normal pericardium within several weeks. This study demonstrates that pericardial meshing facilitates primary tension-free pericardial closure. Free drainage of intrapericardial blood is achieved. A complete anatomic layer between heart and sternum is restored. Pericardial meshing is superior to the pericardial substitutes examined, as adhesions and epicardial reactions are significantly reduced, and the coronary anatomy is readily identifiable.

Quinidine-induced vasculitis.

Four patients developed nonthrombocytopenic purpura two to three weeks after initiation of quinidine therapy. The skin lesions disappeared and did not recur after cessation of quinidine therapy. Histologic examination revealed leukocytoclastic vasculitis with deposition of C3, IgA, and/or IgM in the small dermal vessels. Since quinidine purpura is usually associated with thrombocytopenia, the possibility of leukocytoclastic vasculitis as an additional cause of purpura is stressed.

Malignant fibrous histiocytoma developing in irradiated sacral chordoma.

Malignant fibrous histiocytoma (MFH), arising at the site of a sacral chordoma 8 years after massive radiotherapy, is described. Initially, the patient received 7000 rad to the sacral area and, on recurrence, 5 years later, an additional 4000 rad. Two years later, a sacral mass was noted again. Biopsy then revealed MFH; chest x-ray showed multiple lung metastases. A combination chemotherapy, consisting of cyclophosphamide, vincristine, adriamycin (doxorubicin), and DTIC, resulted in a 6 month partial response. Subsequently, the patient died because of progressive metastatic disease. At autopsy, 8 years after diagnosis, both the sacral lesion and the lung metastases proved to be MFH, and no residual chordoma was found.
