[Correlation of expression of progesterone receptors with histopathological type and grade of malignancy of cerebral neoplasms].

Steroid receptors play an important role in homeostasis and in proliferation of neoplastic cells as well. Biological effect of steroid hormone on tissues depends on intracellular concentration of its receptor. The binding hormone with its receptor activates adequate genes in cellular nuclei and cell proliferation. The mutation of genes codifying of receptor protein may cause its structural modification or changes in its activity and lead to neoplastic transformation. The aim of our work was the estimation of progesterone receptors (PR) concentration in cerebral neoplasma and correlation with histopathological type and grade of malignancy of cerebral tumours. We examined 89 patients, among them 28 with meningioma, 43 with glioma and 18 with metastatic tumour. The concentration of PR was evaluated using radioreceptor assay. Our results show statistically significant correlation between expression of PR and type and malignancy of neoplastic tumours. It was especially seen in meningioma and the highest concentration of PR were in I grade tumours.

[Syringohydromyelia--coexistence or complication following removal of meningioma of forth ventricle].

Numerous theories have been presented to explain the pathogenesis of syringohydromyelia. Among these is Gardner's hydrodynamic theory with later modifications. Syrinx is usually located in cervical cord and is often associated with Arnold-Chiari, Dandy-Walker malformation or other anomalies of cranio-cervical region. Other theories suggest that syringohydromyelia may be secondary to arachnoiditis, neoplasms or spinal cord trauma. The distal location of the syrinx is very often associated with occult spinal dysraphic lesion. The authors present the case of 28 years old woman operated on meningioma of fourth ventricle. One and a half year later syrinx on Th11-Th12 and coexistent arachnoiditis were recognized. They stress diagnostic and therapeutic difficulties in this case and try to explain of etiology of syringohydromyelia.

[The impact of changes in levels of calcium, phosphate and magnesium during hemodialysis on autonomic system reactivity as measured by heart rate variability analysis].

The aim of the study was to analyze the response of autonomic nervous system to dialysis related systemic volume reduction using heart rate variability (HRV) analysis. The possible relationship between changes in serum levels of calcium-phosphate balance parameters and HRV measurements results was also analyzed. The study was performed in 32 patients (20 men, 12 women) aged between 27 and 71 years (mean 44 years) treated with maintenance dialysis due to end-stage renal disease from 3 months to 15 years (mean 4.4 years). All parameters mentioned above were analyzed during 4-hour dialysis session. Mean value of LF/HF ratio at the beginning of the procedure was 5.36, with continuous increase in consecutive measurements performed 30-minute intervals to the maximal value 8.2 in 120th minute of HD session (p < 0.05). In the next measurements continuous decrease in the mentioned parameter was noticed, to the mean value 6.99 in minute 240. The values of LF/HF ratio were also lower for the whole HD session in the group of patients with initial predialytic concentration of calcium lower than 2.35 mmol/l as compared to those with initial calcium concentration higher than 2.35 mmol/l. Higher values of LF/HF ratio and bigger oscillation amplitude of this parameter were also noted in those patients, in whom the percentage reduction of magnesium level during dialysis exceeded 20%. In addition, statistically significant relationship between percentage reduction in magnesium ion concentration and LF/HF ratio during HD was found. Obtained results let us to conclude, that hemodialysis leads to important change in the activity of both components of autonomic nervous system. The factors which may adversely influence the quality of this response may be, among others, low total calcium ion concentration as well as low percentage reduction in magnesium level during hemodialysis session.

Diagnostic value of recruitment order in routine EMG.

In this report a new technique of recording, employed for the first time in routine clinical electromyography (EMG) is described. Conventional analysis is extended to the order of recruitment pattern, and relates the new electrophysiological features to current data about anatomical reorganization of all active motor units (MUs) and their functional effectiveness in neuromuscular disorders. This new concept was tested in four groups of patients (two different myopathic disorders), and (two neuropathies), which were compared to normal subjects. The data obtained confirmed that in normal subjects isometric muscle contraction from weak to maximal effort, was characterized by recruitment of MUs in an orderly pattern according to the "size principle". In polymyositis patients representing the myopathy model a nearly normal range of functioning MUs was found, but their sizes were reduced to a greater extent in the low threshold population. Irrespective of their increased firing rate and variability, the orderly recruitment pattern was still maintained. A different picture was seen in patients with Duchenne dystrophy where the range of functioning MUs and their sizes were reduced with the characteristic lack of high amplitude MUs, (probably high threshold). In the neurogenic groups the range of functioning MUs was reduced but increased in size, and the pattern of recruitment evidently became disorderly. We also found a difference in the manner of disorderly recruitment between motor neuron range disease (MND) and polyneuropathy patients. In the MND group the surviving MUs create very narrow and probably only high threshold MUs with a tendency to their synchronization seen evidently during maximal effort. In the peripheral neuropathy group, the functioning MUs represented wider range of very different sizes, and they were often recruited asynchronously during maximal effort, giving lower amplitudes and rather higher density of record not proportional to the number of MUs lost.

[Surgical treatment of secondary hyperparathyroidism in patients treated with renal replacement].

Between January 1990 and July 1997, in 15 maintenance dialyzed patients and in 3 patients after renal transplantation manifesting hyperparathyroidisms surgical treatment was performed. The diagnosis was based both on the estimation of serum PTH level, total and ionised calcium, phosphates, alkaline phosphatase and imaging procedures: ultrasonography and 99-mTc subtraction scintigraphy. Indications to surgical treatment included ailments like generalized prurigo, bone and joint pains and muscular weakness with no response to pharmacological treatment. The commonly used procedure was subtotal parathyroidectomy (94%), its extent, however, was in each case determined during surgery, depending on the quantity and size of parathyroid glands found. In all cases the immediate intraoperative histopathological examination of the resected tissues was performed. In 10 patients resection of the thyroid gland tissues was carried out because of goiter (56%), among them in 1 case occult papillar carcinoma was found in histopathological examination. In the postoperative period 4 patients (22%) manifested transient hypocalcemia with good response to pharmacological treatment. Good results of surgical treatment reflected by both ailment relief and normalization of serum PTH and phosphates were obtained in 16 patients (89%). In 2 patients (11%) the ailments subsided but did not completely disappear. Surgical treatment of secondary hyperparathyroidism by subtotal parathyroidectomy is efficacious and entails a low risk of complications.

[Use of urea kinetic modeling for evaluating the efficacy of reusing capillary dialyzers].

The aim of the study was to evaluate the influence of follow fiber dialyzers reprocessing procedure on urea kinetic modeling parameters, dialysis effectiveness for small molecules and effective clearance/predicted clearance ratio. Fifty patients (27 F, 23 M) aged 18-61 years (mean 47) being on maintenance hemodialysis due to end stage renal failure for 4-136 months were included into the study. Patients were treated 3 times a week with blood flow 160-290 ml/min and dialysate flow 500 ml/min. The dialysis time was calculated individually based on urea kinetic modeling parameters. Reprocessing of dialyzers was performed using Renatron RS 8300 and Renalin as sterilized agent. Dialysers with ppr below 80% were excluded from the use. Mean values of dialysis index Kt/V, TAC and dialysis effectiveness (Ct/C0) for urea, creatinine, uric acid and kalium as well as values of effective/predicted clearance ratio for consecutive reprocessing procedures were compared with the results obtained for the new dialyzers. There was no significant influence of reprocessing procedure on Kt/V, TAC and Ct/C0 for tested substances and studied clearances. The obtained results indicates that dialysers reprocessing procedure performed according to the described protocol was safe and didn't decrease dialysis effectiveness.

[Incidence of rubella antibodies and the results of vaccination against rubella in young women].

The incidence and the titer of rubella virus antibodies in sera of 936 girls (average age 21 year) from 5 medical school were examined. The antibodies were determined in HI test (Rubenosticon, Organon Teknika). Serum samples were collected in the period of I.1990-IX.1994, and the antibody titer > or = 1:16 was accepted as positive. In 80.3 percent of the examined girls antibodies were found. The mean titer was 1:34.5. Out of 184 seronegative girls, 95 were immunized with Rudivax vaccine. Seroconversion occurred in all, but one person, the mean titer of antibodies was 1:54.6. In 2 vaccinated girls only mild side-effect after immunization occurred.

[Use of urea kinetic modeling in patients on maintenance hemodialysis].

The effect of urea kinetic modeling (u.k.m.) application on dialysis efficiency and metabolic status was evaluated in 50 maintenance dialyzed patients. U.k.m. sessions were performed once a month based on the self-developed computer program to control dialysis. The dialysis index (Kt/V), the time averaged concentration (TAC), protein catabolic rate (pcr) and dialysis effectiveness (Ct/Co) were evaluated and the results obtained at the beginning and after 2, 4, and 8 months of the study were compared. Kt/V had risen significantly in the modeled patients from 1.04 to 1.24 and was accompanied by 12% Ct/Co increase of urea removal after 8 months. The tendency of the moderate (non significant) decrease of TAC from 54.57 to 52.48 mg% BUN was observed during the study. According to the NCDS criteria the percentage of adequately dialyzed patients increased from 42% at the beginning to 64% after 80 months; underdialyzed patients decreased from 16% to 6% and malnourished also from 16% to 6%, respectively, after u.k.m. application. Dialysis effectiveness for creatinine and uric acid described by Ct/Co for the above after 4 and 8 months was significantly increased when compared with the results obtained at the beginning of the study. These results indicate that u.k.m. application allowed to take control over uremic toxemia and improved dialysis adequacy in patients on maintenance dialysis. Protein catabolic rate in studied patients increased from 1.18 to 1.24 g/kg/per day during the study and it was accompanied by a total blood protein and serum albumin increase. This could indicate improvement of the nutritional status of dialyzed subjects.

[Cytomegalovirus infections in infants examined in the department of virology in 1988-1992].

Active cytomegalovirus (CMV) infections were found in 91 infants (11%) examined in the Department of Virology from 1988 to 1992. Serological methods (ELISA, CFT) and virus isolation from urine were used to diagnose CMV infection. Congenital or perinatal infections were recognized in 20 infants, while 10 infants were infected by blood transfusion. In most cases the route of CMV transmission was not recognized. The occurrence of clinical symptoms in CMV-infected children was also studied. In 80% of the infants the disease was characterized by hepatosplenomegaly or liver dysfunction. CMV pneumonia was found in 41% of the cases, and gastroenterocolitis in 36% of the subjects.

[Recombinant human erythropoietin stimulates synthesis of fetal hemoglobin in patients with renal anemia treated by repeated hemodialysis].

Effect of intravenously administered recombinant human erythropoietin (rHu EPO) on haemoglobin (Hb) level, haematocrit (Ht), reticulocyte count and foetal haemoglobin (HbF) concentration was assessed in 10 patients with anaemia, treated by repeated haemodialysis due to end-stage kidney. As compared to the initial values, erythropoietin treatment brought about a significant increase in all the parameters examined. During the subsequent therapy with lower, supporting doses of erythropoietin, the elevated HbF values fell back to normal, whereas the higher level of total Hb and Ht were maintained.

[Activity of selected neutrophil enzymes of patients maintained on hemodialysis and treated with erythropoietin (rHu EPO)].

The peroxidase, alkaline phosphatase, acid phosphatase, beta-glucuronidase and N-acetyl-beta-D-glucosaminidase activity was assessed using a semiquantitative cytochemical methods in peripheral blood neutrophils from 10 maintenance haemodialysed patients treated with recombinant human erythropoietin (rHu EPO) due to severe anaemia. The examination was performed immediately prior to rHu EPO treatment, after 10 weeks and 32 weeks of therapy. A statistically significant increase in the beta-glucuronidase and N-acetyl-beta-D-glucosaminidase activity was observed after 10 weeks, while all the enzymes studied except peroxidase showed a significant elevation of their activity after 32 weeks of the treatment as compared with the values obtained prior to therapy.

Detection of plasma proteins during sequential ultrafiltration/dialysis.

Elimination of low molecular weight proteins during sequential ultrafiltration/dialysis was studied in 29 uremic patients. Beta-2-microglobulin, retinol binding protein, free light chains lambda and kappa, Zn-alpha-2-glycoprotein, hemopexin, prealbumin, hemoglobin, albumin, acid alpha-1-glycoprotein, haptoglobin, alpha-1-antichymotrypsin, ribonuclease, lysozyme, amylase, non-specific esterase, and proteolytic activity were detected in all ultrafiltrates tested. The level of total protein and ribonuclease was determined in 36 crude ultrafiltrates from 23 patients. Concentrated ultrafiltrates were used to quantitate retinol binding protein, prealbumin, albumin, lysozyme, and amylase. Other proteins identified in the ultrafiltrates are present in trace amounts. The question was discussed whether ++inextensive but systematic loss of proteins during hemofiltration in chronic RDT might be the cause of patient homeostasis disturbances.

Effect of hemodialysis on plasma and erythrocyte phenylalanine levels in end-stage kidney patients.

Effect of hemodialysis on phenylalanine distribution between the intracellular water of erythrocytes and the plasma obtained from patients with end-stage kidney was studied by fluorimetric measurements of phenylalanine concentration according to the method of Mc Caman and Robins. The measurements were performed twice: before and after 4 h of hemodialysis. It was found that the nondialyzed patients with end-stage kidney had higher than normal phenylalanine concentrations both in plasma and in the intracellular water of erythrocytes. Hemodialysis brought about a decrease in plasma phenylalanine level without influencing the erythrocyte concentration of the studied amino acid and without altering the function of the erythrocyte cell membrane, as far as the phenylalanine transport was concerned.
