Prophylaxis against postoperative pulmonary embolism and deep-vein thrombosis by low-dose heparin.

The prophylactic effect of low-dose heparin on postoperative fatal and on clinically apparent but non-fatal thromboembolic complications was studied in a double-blind, prospective, randomised study comprising 1296 patients. 16 out of 653 patients in the placebo group had such complications within the treatment period of 1 week, compared with 4 out of 643 in the heparin group. This difference was statistically significant (P less than 0.05). 4 cases in the placebo group and 1 in the heparin group were fatal. After prophylactic treatment had been stopped at the end of the first postoperative week, the rate of thromboembolic complications was equal for the 2 groups. Low-dose heparin prophylaxis is thus effective and should be given routinely in patients aged over 40 years; it should also be given for more than 1 week in patients not ambulant by then.

Pulmonary embolism associated with elective surgery, detected by ventilation-perfusion scintigraphy.

The frequency of clinical and sub-clinical postoperative pulmonary embolism, demonstrated by combined pulmonary ventilation-perfusion scanning, and the prophylactic effect of low-dose heparin, were assessed in a randomised double-blind study in 43 patients aged over 40 years. Pulmonary embolism was demonstrated using combined ventilation-perfusion scanning in 4 out of 43 patients (9%). The incidence was 1 out of 19 patients in the heparin-treated group and 3 out of 24 patients in the placebo group. This difference was not significant. If post-operative perfusion scanning alone had been utilised for diagnosing pulmonary embolism, the frequency of pulmonary emboli would have been 33%, with 21% in the heparin group and 46% in the placebo group. This difference is not significant. It is concluded that perfusion scanning over-estimates the frequency of post-operative pulmonary embolism. The use of the more specific combined ventilation-perfusion scanning shows that asymptomatic postoperative pulmonary embolism does occur, but that the frequency is considerably lower than indicated by perfusion scanning alone. Combined ventilations-perfusion scanning must be performed in studies assessing the effects of various treatment regimes on the frequency of pulmonary embolism postoperatively. The small number of patients included in the study do not permit any conclusions concerning any prophylactic effect of low-dose heparin against pulmonary embolism.

Keloids treated with topical injections of triamcinolone acetonide (kenalog). Immediate and long-term results.

In a prospective trial, the results of local steroid treatment of keloids have been studied. 52 patients were treated with Kenalog (triamcinolone acetonide) injections alone, whilst in 15 patients, steroid therapy was combined with excision of the keloid. The combined treatment gave no better results than injection therapy alone. The early result of treatment was the complete flattening of the lesions and cessation of itching in the majority of cases. However in one-third of the cases partial recurrence was found after one year, and after 5 years, the recurrence rate was 50%.

Peritoneal lavage in the diagnosis of intraperitoneal bleeding.

The ability of peritoneal lavage with normal saline to disclose intraperitoneal haemorrhage requiring surgical intervention was assessed in a prospektive study of 30 patients admitted after blunt abdominal trauma or on suspicioun of ruptured ectopic pregnancy. The results were compared with the results of a meticulous clinical examination including simple laboratory tests, and were found significantly more reliable in the management of these patients.

Lung function and morphology after bilateral hilar stripping in dogs.

Acute changes of lung function and morphology were studied in 3 groups of dogs: after bilateral hilar stripping by ipsilateral approach (group 1), after right-side hilar stripping and left pneumonectomy by left-sided approach only (group 2) and in a control group. All dogs of group 1 showed a syndrome corresponding to functional hilar stripping with absent Hering-Breuer and cough reflex, slow respiration with increased tidal volume and discharge of fluid from the airways. Alveolar ventilation and oxygen tension, dead space/tidal volume ratio and pulmonary shunting did not change. Cardiac output and arterial oxygen tension were decreased. The lungs were heavy and histological examination showed lymphostasis and interstitial edema. The dogs of group 2 did not show significant changes in comparison to the control group.

A study of protein synthesis in cells cultured from involved psoriatic skin.

Using histochemical techniques an abnormal programme of epidermal differentiation has been well documented in psoriasis. In order to characterise further the biochemistry of this process we have cultured dermal fibroblasts and epidermal keratinocytes from involved psoriatic skin. This has facilitated metabolic radiolabelling of skin cells and analysis of protein synthesis by two-dimensional polyacrylamide gel electrophoresis. The expression of keratin and differentiation markers was identical to that of normal keratinocytes, suggesting that psoriatic epidermal differentiation is not truncated in vitro as has been postulated to be the case in vivo. Low molecular mass components (5-8.5 kDa), previously shown to be upregulated in suprabasal keratinocytes, were detected in epidermal fractions from psoriatic skin enriched for basal cells. Of special interest was a component of 26 kDa, pI 5.9, which was highly upregulated in psoriatic as compared to normal cultured keratinocytes and was not detected in fibroblasts. These findings are in accord with a qualitatively abnormal pattern of differentiation for keratinocytes in the involved psoriatic epidermis.
