Surgical treatment of psoriasis.

24 patients with plaque psoriasis were treated by serial dermatome shaving of the affected skin. Depth of excision was to the middle of the reticular dermis. Postoperatively the wound was treated as for a skin-graft donor site and healed satisfactorily in all cases. Follow-up extended from 3 to 36 months. In 17 patients, the shaved areas healed to give normal skin and appendages and remained free from psoriasis during the period of observation. 7 patients had minor recurrences along the margin of the shaved area; in 1 a further shaving was followed by recurrence-free healing of the area. In plaque psoriasis, therefore, excision of the superficial skin layers appears to be an effective and easy alternative to medical treatment.

Absorbable suture in hernia repair.

In a randomized, double-blind study, comparison was made between absorbable polyglycolic acid sutures (Dexon) and non-absorbable sutures (Dacron) in inguinal and femoral herniorrhaphies. The study comprised 58 patients with 61 hernias. Bassini repair was done in inguinal hernia and McVay repair in femoral hernia. The patients were evaluated at a follow-up examination six months after the operation. There was no wound dehiscence or infection during the primary admission. In one patient from each group there was recurrence of hernia during the observation period. No suture granuloma had developed in that time. These results warrant the use of absorbable sutures in inguinal and in femoral herniorrhaphy.

Hypospadias repair: with construction of a fossa navicularis.

Acceptable results can be achieved in the treatment of hypospadias by the use of Denis Browne's method. However, the condition is not completely normalized, as the external orifice of the urethra is most frequently situated in the neighbourhood of the coronal sulcus and the orifice itself open. The distance to the normal urethral mucosa is thus short with the consequent increased risk of contamination. In order to ensure normalization of the condition, Denis Browne's method was supplemented by the construction of a fossa navicularis in 74 patients with hypospadias. The results of this construction were satisfactory in the great majority of cases.

The use of skin flaps in the treatment of malignant melanomas in the head and neck region.

The use of skin flaps in the treatment of malignant melanomas in the head and neck is compared to the application of free skin grafts. In 13 patients the wounds were closed directly, in 58 the defects were covered by free skin grafts, and in 80 patients the defects were covered by flap plasties. The distribution of tumours with various degrees of invasion was similar within the groups. Local recurrences or local skin metastases occurred in 18 patients. Eight were found in the skin graft group. In the group with direct closure only 1 patient had a recurrence. Nine were found in patients treated with skin flaps. The use of skin flaps is often a great advantage from an aesthetic point of view and this survey does not indicate that it is contraindicated for oncologic reasons.

Urokinase or heparin in the management of patients with deep vein thrombosis?

Twenty patients with clinical signs of deep vein thrombosis of a duration not exceeding 72 hours, and with the condition confirmed phlebographically, were randomly allocated to one of two groups in a double-blind study. In group 1 the patients received urokinase in a low-dose regimen of 200 000 Ploug units during the first 24 hours, followed by infusion of heparin, 40 000 units daily during the next 5 days. Patients in group 2 received heparin only, 40 000 units daily for 6 days. The clinical course was assessed daily. When the infusion period was completed, the phlebography was repeated, and the results of the two examinations were compared with respect to extent of filling defects and the degree of non-filling of the deep veins. We found no superiority in the regimen consisting of urokinase preceding heparin infusion, compared with that of heparin infusion alone. Most of the patients improved clinically during the 6-day infusion period, but the degree of thrombosis, evaluated phlebographically, was unaltered or even deteriorated during the period in all patients except two. Overt bleeding was noted in 6 patients.

X-ray examination and/or endoscopy in the diagnosis of gastroduodenal ulcer and cancer.

The diagnostic value of barium meal and endoscopy for the diagnosis of gastric ulcer and cancer was studied in 173 unselected patients with upper abdominal dyspepsia. The predictive value of both positive and negative results was higher for endoscopy than for barium meal. This was also found in earlier studies comprising hospitalized patients, with a consequent higher prevalence of disease, but this study shows that the superiority of endoscopy is maintained in a low-prevalence outpatient population. The diagnostic efficiency of both endoscopy and X-ray was independent of the order of examination. Knowledge of the result of the first examination thus did not improve the efficiency of the second one. Parallel use of endoscopy and barium meal results in a maximum of diagnostic yield but requires the highcost of double examination and supplementary examinations in cases of discrepancy between the two results. Routine use of barium meal with selective use of endoscopy in X-ray-positive cases requires a smaller number of examinations but results in the loss of an unacceptable number of diagnoses. When both costs and diagnostic yield are considered, endoscopy seems to be the examination of choice, with X-ray examination reserved for exceptional cases when endoscopy for one reason or another cannot be carried out. Hospital facilities should be planned with the aim of developing endoscopic services capable of taking over the traditional leading role of X-ray examination in the gastroduodenal field.

The need for replantation centers in view of the incidence of traumatic amputations.

During one year a total of 451 amputations in 360 patients were registered by the Danish National Accident Insurance to which all such accidents must be reported, all except 6 of these were finger amputations. An attempt is made to estimate the candidates for replantation surgery in Denmark. Following a somewhat restricted policy the number may be no more than twenty-five to thirty-five per year. This calls for limitation of the numbers of centers which deal with such accidents.

Ultrasound and clinical diagnosis of deep vein thrombosis of the leg.

The diagnostic precision of clinical evaluation and of ultrasound examination in the diagnosis of deep vein thrombosis was obtained by ascending phlebography. The diagnostic specificity and diagnostic sensitivity of clinical examination was 0.72 and 0.78 and with ultrasound 0.73 and 0.70. With no other examination than phlebography is it possible to demonstrate major central thrombosis in the leg with certainty.

Combined ventilation-perfusion scintigraphy for demonstratio of pulmonary embolism.

In 34 patients with suggested pulmonary emboli, ventilation scintigraphy with 133Xe and perfusion scintigraphy with 99Tcm-labelled albumin spheres were carried out. The combined ventilation-perfusion scintigraphy had a significantly higher diagnostic specificity (1.0, confidence limit 0.69-1.0) than perfusion scintigraphy alone (0.48, 0.26-0.70). Both methods had a diagnostic sensitivity of 1.0.

Postoperative deep vein thrombosis of the lower limb and prophylactic value of heparin evaluated by phlebography.

The incidence of postoperative thrombosis of the leg at phlebography, and the prophylactic effect of low-dose heparin, was assessed in a double-blind investigation comprising 50 patients. Thrombosis occurred in 17 per cent in the heparin group and in 23 per cent in the placebo group. This difference is not significant. No prophylactic effect of low-dose heparin was demonstrated.

Biopsy and brush cytology in the diagnosis of gastric cancer.

In a prospective study the diagnostic value of biopsy and that of brush cytology in the diagnosis of gastric cancer were compared. The patients included in the study were endoscopied because of upper abdominal dyspepsia. When the endoscopic appearance of the mucosa suggested malignancy, and in all cases in which ulcerations were seen, brush cytological smear and at least five biopsies were obtained. The cytology and the biopsy specimens were assessed by two different pathologists in a mutual blind fashion. If the result of the examination suggested or showed the presence of cancer, it was considered positive. The results were evaluated by means of sequential analysis. When 195 examinations had been included in the study, disagreement had occurred in 8 cases, in all of which the biopsy was verified as correct. This difference is significant. In 187 cases the result of the 2 tests were in agreement, but in 3 cases the result of both tests were falsely negative. It was concluded that no diagnostic advantage was gained by the addition of brush cytology to endoscopic biopsy.

The role of liver passage for conversion of pteroylmonoglutamate and pteroyltriglutamate to active folate coenzyme.

In an attempt to clarify the possible site of conversion of folates to active methylated coenzyme forms mono- and triglutamate was injected rapidly into the portal vein of dogs. Samples of blood were removed at various intervals from catheters placed in the portal vein, the hepatic vein and the femoral artery for assay carried out both with L. casei, S. Faecalis and liquid scintillating counting. Unreduced monoglutamates were extracted in the liver, immediately converted to methyltetrahydrofolate and released again to plasma from which it was slowly taken up by peripheral tissue. Reduced folates were refused by the liver. Triglutamates were extracted exclusively in the systemic circulation, converted to unreduced monoglutamate later to be reduced in the liver. No tissue displacement of folate was demonstrated. Only small amounts of monoglutamates, mainly reduced forms, were lost in the urine during the experiments. The kinetics of unreduced monoglutamate suggest a one compartment system, while the kinetics of methyltetrahydrofolate and triglutamate may implicate a two compartment system.
