The fate of loop ureterostomy and severe contracted bladder.

Bladder contracture following cutaneous loop ureterostomy is a known but sometimes neglected serious complication. Recently we treated a boy successfully by effecting an increase in bladder capacity by long-term hydraulic dilatation prior to performing colonic augmentation. The severity of the contracture seems to be the worst ever published in the literature.

Importance of serum amyloid A (SAA) level in monitoring disease activity and response to therapy in patients with prostate cancer.

Serum amyloid A (SAA) and acid phosphatase (AcP) levels were determined in serial serum samples of 35 patients in different stages of dissemination and correlated with activity of carcinoma of the prostate. Up to 500-fold increases in SAA level were detected during active periods of cancer with a decrease towards the normal range in remission, in comparison with a 10-fold increase of AcP. The correlation between these two parameters was highly significant (P less than 0.001), but while SAA shows 100% sensitivity during the active stage, AcP shows only 85% sensitivity. It is suggested that although SAA is not a specific marker for any particular illness, due to its characteristic pattern of change in malignant diseases and its high sensitivity, it represents a useful biochemical parameter for the assessment of the activity of the disease to monitor response to therapy during follow-up.

Congenital anterior urethral diverticulum.

Congenital anterior urethral diverticulum (CAUD) was diagnosed in 7 male subjects, of whom 3 cases were neonates, 3 cases were school aged and 1 case was adolescent. Varying degrees of urinary obstruction, often associated with urinary tract infection, were the main presenting symptoms. Urinary ascites, a rare manifestation of CAUD, was presented in a neonate associated with severe upper urinary tract changes. In 6 cases the diverticulum was of the wide-mouthed saccular type, and in 1 case of the narrow neck globular type. The clinical presentations, methods of diagnosis and the treatment modalities are discussed.

Conformational analysis of cytokinins and analogs.

The conformational energy surfaces of 12 active cytokinins and analogs are studied with the aid of PCILO quantum mechanical calculations. The resulting conformational energy maps indicate that cytokinin activity is associated with the ability of the above molecules to attain a specific conformation, presumably related to their conformation at the active site of cytokinin receptor(s). The calculations locate the conformational energy minima and describe the flexibility of the studied molecules in terms of conformational barriers and transition paths. An approximate relation is found between cytokinin activity and the values of energy barriers to transitions between certain local minima. According to this relation, active compounds should have rotational barriers within 4-12 kcal/mol, besides the known hitherto constitutional requirements for high physiological activity.

Malignant fibrous histiocytoma of the spermatic cord.

We report the seventh case of malignant fibrous histiocytoma arising from the spermatic cord. The followup of our patient supports the generally accepted approach of radical inguinal orchiectomy along with wide en bloc local resection of the adjacent soft tissue.

Quantum mechanical conformational analysis of heterocyclic analogues of norephedrine.

Studies on the conformation of several structural analogues of norephedrine, thiophene, carbazole and furan, were carried out using the differential PCILO method. The erythro-forms of these compounds possess minima on the conformation map corresponding to a gauche conformation with synclinal H-atoms. This result is in good agreement with the proton-proton coupling constants found in previous NMR-studies. H-NMR-studies suggest for the threo-isomers of the studied molecules an equilibrium between the trans- and gache-conformations of the ethanolamine chain. Present calculations agree fairly well with this result. All the studied molecules possess conformational minima corresponding to the folded form of the side chain believed responsible for the physiological activity of norephedrine. The distances between "N" and 'O' atoms in this preferred conformation correspond to the model proposed by Kier and Pullman for alpha-adrenergic receptors.

Paratesticular rhabdomyosarcoma in an adult.

Embryonal rhabdomyosarcoma is a common tumor of children, rarely appearing in the scrotum from tunica vaginalis or paratesticular tissues. In adults it is rare and carries a particularly bad prognosis. A case of paratesticular rhabdomyosarcoma occurring in a seventy-three-year-old man who survived a total of five and one-half years from the time of his original operation, is presented.

Spontaneous perirenal hematoma: a complication of polyarteritis nodosa.

Polyarteritis nodosa is a rare cause of perirenal hematoma owing to rupture of 1 of the aneurysms. Eleven cases have been reported previously in the English literature, including 5 with a preoperative angiographic diagnosis. Our case, which was diagnosed angiographically before a successful conservative operation, is the first report of a patient who survived 3 years. The pathology is discussed.

Diverticula of Meckel's diverticulum.

Diverticula, or multiple sacculations, of a Meckel's diverticulum are a relatively unknown and undocumented pathologic entity. A case is reported, and another case recalled. In both cases, the sacculations were in the fundus. There was partial or complete absence of muscular layers in the saccular wall. A congenital origin of these muscular defects is more likely than is an origin from acquired hydrostatic pressure.
