Mycetoma caused by Nocardia madurae.

Actinomycotic mycetoma was diagnosed in a woman from Jamaica living in Ontario. This is the first case reported in Canada in which the infection was caused by Nocardia madurae. Despite oral therapy with trimethoprim-sulfamethoxazole, local excision of newly appearing nodules was required periodically for clinical improvement. Laboratory procedures were modified to aid in identification of pathogenic actinomycetes.

Are prophylactic antiparkinson drugs necessary? A controlled study of procyclidine withdrawal.

Of 55 aftercare patients receiving long-term treatment with antipsychotic and antiparkinson (AP) drugs, 37 were switched to being given placebo, and 18 remained on a regimen of procyclidine hydrochloride. The dose of antipsychotic was kept constant. After three weeks extrapyramidal side effects (EPS) developed in 54% of those patients receiving placebo and in none of those receiving procyclidine (P less than .002): Twenty-seven percent of the placebo group had EPS without akinesia, and in the same percentage akinesia developed (P = .003). We believe the risk-benefit ratio favors the routine use of AP drugs for prophylaxis and maintenance so as to avoid misdiagnosing as psychopathology, unspontaneity due to akinesia, and to reduce unreliable pill-taking due to EPS.

Long-acting oral vs injectable antipsychotic drugs in schizophrenics: a one-year double-blind comparison in multiple episode schizophrenics.

Sixty patients meeting the criteria established for schizophrenia who attained a clinical plateau following hospital discharge were randomized to receive for one year either penfluridol, 20 to 160 mg orally once each week, or fluphenazine decanoate, 0.5 to 4 ml every two weeks. The relapse rate for both treatments was low and equal. The rate of recurrence of psychosis for patients receiving penfluridol was 7% and for those receiving fluphenazine decanoate 10%. A retrospective comparison of the penfluridol group was made to a similar group of patients assigned to placebo in an earlier study. Placebo-treated patients had a relapse rate of 68%. Penfluridol patients had statistically fewer psychotic relapses. Questions about the possible carcinogenicity of penfluridol in animals will have to be resolved before it can be widely used. This study demonstrates the feasibility of using an oral, long-acting antipsychotic agent. It would be a useful psychopharmacologic addition in the treatment of outpatient schizophrenics.

Early detection and identification of Trichophyton verrucosum.

A new medium for the early detection and identification of Trichophyton verrucosum has been formulated. The key ingredients of the medium are 4% casein and 0.5% yeast extract. T. verrucosum is recognized by its early hydrolysis of casein and very slow growth. Microconidia were produced by 19 out of 35 isolates (54%), and macroconidia were produced by 8 out of 35 isolates (23%). All isolates formed chains of chlamydospores at 37 degrees C, and 24 out of 35 isolates formed chains at 28 degrees C. Nutritional requirements of all 35 strains of T. verrucosum were confirmed. The medium was evaluated by isolating 570 suspected T. verrucosum from skin scrapings. The early detection of hydrolysis, formation of characteristic chains of chlamydospores, and restricted slow growth of this dermatophyte differentiate it from T. schoenleinii.

Comparison of the incidence and severity of extrapyramidal side effects with fluphenazine enanthate and fluphenazine decanoate.

Forty-nine schizophrenic outpatients stabilized on oral antipsychotic medication and procyclidine received 12.5 mg or 18.75 mg of fluphenazine enanthate or fluphenazine decanoate and were examined for extrapyramidal side effects one and two weeks later. Extrapyramidal side effects were present in 30 patients (61%) but were clinically significant in only 11 (22%). Fluphenazine enanthate produced more clinically significant extrapyramidal symptoms, particularly akathisia, than did fluphenazine decanoate.

Prophylactic effect of lithium and imipramine in unipolar and bipolar II patients: a preliminary report.

Relapse rates of the first 35 unipolar and bipolar II manic-depressive patients who entered the controlled phase of a continuing evaluation of the prophylactic value of lithium alone or in combination with imipramine were analyzed. Lithium had a prophylactic effect in unipolar patients and possibly in bipolar II patients. Imipramine did not have a prophylactic effect in either group of patients.

Extrapyramidal side effects with lithium treatment.

The authors neurologically examined 38 patients who had received lithium carbonate for from 3 months to 5 years to determine the presence of extrapyramidal side effects. No patient had received neuroleptics for 3 months before examination. Definite cogwheeling was found in 2 patients. The results confirm previously reported occurrence of cogwheeling in lithium-treated patients.

The comparison of two agar media for germ tube and chlamydospore production by Candida albicans.

A simple (2% oxgall) agar medium is described for the production of both germ tubes and chlamydospores by over 500 clinical isolates of Candida albicans. In comparison studies between the 2% oxgall agar and the more complex "cream of rice" infusion-oxgall-Tween 80 agar (RIOT), germ tubes were formed by 478 isolates in both media, by 9 isolates in only the oxgall medium and by 11 isolates in only the RIOT medium. Chlamydospores were formed by 481 isolates in both media, by 2 isolates in only the oxgall medium and by 9 isolates in only the RIOT medium. The data also show that both the germ tubes and chlamydospores were needed for the presumptive clinical identification of C. albicans.

Modified indirect fluorescent-antibody test for aspergillosis.

Diffuse fluorescence in an indirect fluorescent-antibody test for antibody to Aspergillus fumigatus was eliminated by using germlings, rather than hyphae, as antigen. The modified method yielded diagnostic titers larger than or equal to 1:32 in 24 of 25 known cases, was highly specific, and showed good correlation with clinical status and results of immunodiffusion. This technique, which uses the germling antigen, inactivation of test serum at 59 +/- 1 degrees C, and anti-immunoglobulin G conjugate, is a sensitive, specific, and reliable quantitative indicator of aspergillosis and is useful in following the course of the disease.

Hemoglobin Tak in a newborn Malay.

The trait condition for hemoglobin Tak, was found in a 4-day old newborn Malay who suffered from severe neonatal jaundice. The beta chain of the abnormal hemoglobin was elongated by 11 residues at the C-terminus and had the same structure as reported for Hb Tak. The mother was heterozygous for this abnormal hemoglobin, the father was normal. The mother and child, 4 years later, did not show clinical or hematological symptoms except definitely increased resistance of their erythrocytes to hypotonic saline solutions and slight anisopoikilocytosis. The abnormal gene in the two reported Thai families and in our Malay family may have the same origin.
