Platelet monoamine oxidase and clinical phenomenology of schizophrenia.

Platelet monoamine oxidase activity (MAO) was determined in 39 unmedicated chronic schizophrenic patients and 88 normal control subjects. Platelet MAO activity did not distinguish paranoid from nonparanoid patients or patients who met Taylor and Abrams criteria for narrowly defined schizophrenia from other schizophrenics. Enzyme activity was not related to either prognostic scores or age at onset of illness. MAO activity was decreased in patients compared to controls, and was lower in males than in females. Our findings indicate that clinical phenomenology, as defined in the present study, is of limited use in identifying biological subtypes of schizophrenia with deviant platelet MAO activity.

EEG sleep of normal healthy children. Part I: Findings using standard measurement methods.

Despite the increasing application of all-night electroencephalographic (EEG) sleep studies to children for clinical as well as for research purposes, readily available normal EEG sleep standards for the period of childhood have remained sparse and, at present, reflect data on only approximately 100 children 6 to 16 years of age. As part of a large scale study examining various aspects of EEG sleep among children, findings derived using standard recording and scoring methods are reported for a new sample of nearly 100 normal, healthy children and are compared with existing standards. Data obtained add substantially to the existing database and generally confirm findings of previous normative reports on children in this age range.

Conversion of Blastomyces dermatitidis to the yeast form at 37 degrees C and 26 degrees C.

A partially defined agar medium, KT, has been developed and compared with brain heart infusion agar for the conversion of Blastomyces dermatitidis to the yeast form. On the KT medium, the mold form converted to a yeast form within 72 h of incubation at 37 degrees C or after 3 weeks at 26 degrees C. A nutritionally dependent dimorphism in B. dermatitidis was observed.

Is loxapine more effective than chlorpromazine in paranoid schizophrenia?

The authors compared loxapine with chlorpromazine in inpatients with paranoid schizophrenia and found no difference in clinical efficacy. Thus a previous finding, based on retrospective analyses, that loxapine was superior to other neuroleptics in the treatment of paranoid schizophrenia was not verified in this prospective study.

Platelet monoamine oxidase activity and genetic vulnerability to schizophrenia.

Platelet monoamine oxidase (MAO) activity, determined in 102 patients with chronic schizophrenia, 223 first-degree relatives, and 88 normal control subjects, was shown to be a heritable and stable trait and was significantly lower in patients than in normal control subjects. Within families, MAO activity distinguished ill from well relatives. However, the considerable overlap in enzyme activity between affected and unaffected individuals limits the usefulness of low MAO activity as a major risk factor in schizophrenia.

Blastomycosis: a new endemic focus in Canada.

A survey of the 38 patients resident in Ontario from whose sputum, body fluids or tissues Blastomyces dermatitidis was cultured by our laboratory between 1970 and 1981 revealed a new endemic focus to the north and east of Lake Superior, where 20 of 27 traceable patients lived. Direct microscopy revealed B. dermatitidis in 90% of the cases. A lack of clinical awareness, however, had often resulted in a delay (average 15 weeks) in diagnosis. In two cases the disease was identified only at autopsy. About 80% of the patients survived.

Malaria in the emergency department.

Eleven patients with malaria seen over five years in one emergency department demonstrate a trend toward more efficient diagnosis and outpatient treatment. Ten patients were diagnosed using a routine Wright's stained peripheral blood smear (three fortuitously). Because treatment of hospitalized patients consists of no more than oral chloroquine, the current regimen allows for outpatient management. Determination of species and extent of drug resistance, as well as eradication of Plasmodium vivax and P ovale, are referred to a consultant after 2 days of oral chloroquine therapy.

Pulmonary caseating granulomas associated with Mycoplasma pneumoniae pneumonia.

Severe Mycoplasma pneumoniae pneumonia, complicated by autoimmune hemolytic anemia, monarticular arthritis, and persistent upper lobe consolidation, clinically simulating pulmonary tuberculosis, occurred in a young girl. A specimen taken at open lung biopsy grew Mycoplasma pneumoniae and microscopically showed caseating granulomas, a finding never previously associated with M pneumoniae infection. Although it is possible that another infectious or immunologic granulomatous lung disease may have been responsible for the unusual clinical, roentgenographic, and pathologic findings, no other etiologic agent was identified.

Local nasal immunotherapy for ragweed-allergic rhinitis. III. A second year of treatment.

In 1979, pre-seasonal local nasal immunotherapy (LNIT) was found to be an effective treatment for ragweed hay fever. In 1980, this study was continued to evaluate the clinical and immunologic responses of a second year of LNIT. Patients received either pre-seasonal treatment with an unmodified ragweed extract (RW) or a polymerized ragweed extract (PRW), or no treatment. The results of the second year of treatment were the same as the first year. Adverse reactions were significantly higher in the RW-treated group than in the PRW-treated group (P less than 0.001). Symptom/medication scores (SMS) in the RW-treated group were significantly lower than in the control group (P less than 0.005). Although SMS in the PRW-treated group were lower than in the control group, this difference was not significant. The immunologic response was evaluated by measurements of serum (S) RW-specific IgE and IgG and nasal secretory (NS) RW-specific IgE, IgG, and IgA. After treatment, serum IgE titres and secretory IgA titres rose in the RW-treated patients. Nasal secretory-IgG and NS-IgA titres increased with PRW treatment. The only immunologic response observed in the control group was a rise in S-IgE titres after the ragweed season. There was no substantial difference in immunologic measurements observed in the 1979 and 1980 seasons, except that the pre-treatment NS-IgE level was higher in 1980 (P less than 0.02). No significant correlations were found between antibody response and SMS. This study supports the efficacy of LNIT but does not support the protective role for NS-ragweed-specific IgA or IgG.

Age-of-onset in schizophrenia and schizotypal disorders. Clinical and genetic implications.

Age-of-onset data were gathered on 93 chronic schizophrenic probands and 57 affected (mainly schizotypal) siblings. 55% of affected individuals were ill before age 20 and 14% had their onset before age 14. The risk period for schizophrenia and schizotypal personality disorders terminated at age 40. Age-of-onset did not distinguish paranoid from nonparanoid schizophrenics, or definite from probable schizotypal personalities. Schizophrenic and schizotypal subjects were similar in their age-of-onset patterns. Sex effect on age-of-onset was not present. A square-root normal distribution gave the best fit to the data. The implications of these findings for schizophrenia research were discussed.

Familial relatedness of schizophrenia and schizotypal states.

The types and expectancy of mental disorders in the siblings of 74 probands with chronic schizophrenia were examined. The siblings were classified according to whether 1) both parents had schizotypal personality disorder, 2) one parent had the disorder and one was normal, or 3) both parents were normal. Siblings whose parents both had the disorder were at significantly greater risk for schizophrenia and schizotypal personality disorder than siblings with at least one normal parent. Similarly, the expectancy of schizotypal personality disorder alone and combined with schizophrenia was higher among siblings with one parent with the disorder than in siblings with two normal parents. The data suggest that schizotypal traits may be genetically related to schizophrenia.

Neuroleptic drug effect on platelet monoamine oxidase and plasma amine oxidase in schizophrenia.

Activity levels of platelet monoamine oxidase (MAO) and plasma amine oxidase (PAO) were determined in eight chronic schizophrenic patients who had been treated with neuroleptic drugs for 3 months. The mean reduction in platelet MAO activity was 18.6%. The extent of decrease was statistically significant. The reduction in enzyme activity was unrelated to serum iron levels. PAO activity was unaltered. The implications for schizophrenia research are discussed.

Schizoaffective illness, schizophrenia and affective disorders: morbidity risk and genetic transmission.

Data on schizoaffective illness, schizophrenia and affective disorders were gathered on first-degree relatives of schizoaffective probands and matched controls (bipolars, unipolars and schizophrenics). The familial pattern of affective and schizophrenic subtypes of schizoaffective disorder resembled the familial pattern of affective and schizophrenic probands, respectively. The overall risk for the spectrum of schizoaffective and affective disorders was higher among relatives of schizoaffective-manic as compared to relatives of schizoaffective-depressive probands, although the difference fell short of significance. When tested for consistency with multiple threshold hypotheses of genetic transmission, schizoaffective illness did not qualify as either a more extreme form of affective illness nor as a disorder that occupies an intermediate position between bipolar and unipolar disorders or is genetically milder than affective disorder. The implications of diagnostic subtyping for genetic research in the major psychoses were discussed.

Microsporum equinum in North America.

Microsporum equinum was isolated in Ontario, Canada, from five human and two equine cases of ringworm infection. This dermatophyte was previously recovered from North American horses on several occasions, but was considered to be M. canis. We regard M. equinum as distinct from M. canis. It can be differentiated from M. canis by the smaller size of its macroconidia, its failure to perforate hair in vitro, its poor growth and sporulation on bromocresol purple casein dextrose agar, and its incompatibility with Nannizzia otae, the telemorph of M. canis.
