Light microscopical demonstration and zonal distribution of parasinusoidal cells (Ito cells) in normal human liver.

The parasinusoidal cells of the liver (Ito cells) were demonstrated light microscopically in autopsy specimens fixed in formalin and stained with Oil red O after dichromate treatment. The method allows examination of large samples containing numerous acini. Quantitative assessment showed a zonal gradient with 6.3 and 7.7 parasinusoidal cells per 62.5 X 10(3) micron2 in zone 1 and 3, respectively.

Perivenous and perisinusoidal collagen content in the acinar zone 3 in the "normal" liver. A light microscopical study.

The thickness of the terminal hepatic vein wall (TTHV) and the content of perisinusoidal collagen in 989 zone 3 areas in Picro-Sirius-stained sections from 117 liver biopsies with normal morphology or slight non-specific changes were analyzed. TTHV varied from 0.4 micron to 21.3 microns (median 3.8 microns). A positive correlation was found between TTHV and the diameter of the terminal hepatic vein. The average TTHV of the individual biopsies was not correlated to age, sex or liver morphology. The average zone 3 collagen content did not exceed that of zones 2 and 1, with the exception of biopsies with slight steatosis, where a minimal increase in zone 3 collagen was found. The normal variability of TTHV and the correlation to the diameter of THV must be considered in the evaluation of perivenular fibrosis. Even a slight increase in perisinusoidal collagen content may be suggestive of a pathological condition.

The occurrence and significance of fibronectin in livers from chronic alcoholics. An immunohistochemical study of early alcoholic liver injury.

The occurrence and distribution of fibronectin (FN) was assessed by an immunoperoxidase technique in liver biopsies from alcoholics without and with acinar zone 3 fibrosis of varying degrees. Increased amounts of FN was found diffusely in zone 3 areas with a perisinusoidal and pericellular localization. FN was closely correlating to the pattern of fibrosis but increased amounts of FN could also be seen in biopsies without fibrosis as visualized in Picro-Sirius stained sections. There was no topographical relationship to liver cells with fatty changes, Mallory bodies or to alcoholic hepatitis. It is made probable that FN is of significance in the development of early liver fibrosis in alcoholics and that FN may act as a chemotactic factor for collagen producing cells and as a skeleton for the new collagen formation.

Megamitochondria as a diagnostic marker for alcohol induced centrilobular and periportal fibrosis in the liver.

One hundred and five biopsies with centrilobular and/or portal/periportal fibrosis of different aetiology were examined by light microscopy for the presence of megamitochondria (MM). Sixty eight patients had some daily alcohol intake, 37 did not. The incidence of MM in the group with daily alcohol consumption was 49% and in the group without only 5%. Two types of MM were identified. Type I MM (round to oval) were located mainly in zone 3 and type II (needle-shaped) mainly in zone 1. Both types were related to alcoholic liver disease.

Psychological effect of detailed preanesthetic information.

Eighty-one patients admitted for minor surgery were followed with questionnaires and self-rating scales in the pre- and post-anesthetic period to evaluate the effect of giving either routine or detailed information. The patients were randomly allocated to two groups and received either routinely given information by the anesthetist for about 5 min or more detailed information for at least 20 min. The patients' experience of the effect of the preanesthetic visit was tranquillizing and adequate in both groups. The most significant difference with detailed information was a smaller number of side-effects like slow cerebration, nausea and a general feeling of discomfort compared to the routinely informed patients. Repetitive ratings on Spielberger's State of Anxiety Scale showed that the patients who had had previous anesthetic experience were less influenced by the degree of information given. In view of the considerable numbers of parameters investigated, there were relatively few significant differences between the groups, and it was concluded that there was no convincing benefit from expanding routine to detailed information.

Detection of early zone 3 liver fibrosis in chronic alcoholics. A comparison of four connective tissue staining methods.

Fibrous deposits in the perisinusoidal space in the acinar zone 3 are early and prognostically important features of the alcoholic liver lesion. Four connective tissue staining methods: van Gieson's Picro-Fuchsin (VG), Gordon & Sweets' reticulin (GS), Chromotrope Aniline Blue (CAB), and Picro-Sirius (PS) were evaluated concerning their relative sensitivity in the detection of early zone 3 fibrosis. A selected area in 60 liver biopsies with preserved architecture, obtained from chronic alcoholics, were studied. A semiquantitative registration of the degree of fibrosis was made. A comparison between the scores of the various staining methods by means of Pratt's test revealed PS to be significantly the most sensitive method. GS was insignificantly more sensitive than CAB while VG (which in contrast to the other three methods leaves reticulin unstained) was the least sensitive. In five biopsies which only revealed fibrosis in PS, perisinusoidal collagenization was confirmed by transmission electron microscopy. In the histological detection of early zone 3 fibrosis, PS is therefore recommended.

Early alcoholic liver injury. Activation of lipocytes in acinar zone 3 and correlation to degree of collagen formation in the Disse space.

Acinar zone 3 areas in liver biopsy specimens from 23 alcoholics and 47 non-alcoholics were investigated by light microscopy and transmission electron microscopy to asses fibrosis in the perisinusoidal space and to evaluate the role of the lipocytes. Quantitative analysis by light microscopy on toluidine blue-stained sections showed a significant reduction in number of lipocytes--median values of 2.7 and 1.2 lipocytes per 100 hepatocytes in biopsies from chronic alcoholics showing no or varying degrees of zone 3 fibrosis, respectively, as compared to 3.6 lipocytes per 100 hepatocytes in non-alcoholic livers. By transmission electron microscopy, the reduction in number of lipocytes was related to a corresponding increase in number of cells rich in rough endoplasmic reticulum and microfilaments (activated lipocytes). The occurrence of activated cells was significantly correlated to fibrosis of the perisinusoidal space. Activation of lipocytes and collagenization of the perisinusoidal space appeared before light microscopic evidence of fibrosis and were topographically not related to Mallory bodies or alcoholic hepatitis.

Endoscann cell sampling for cytological assessment of endometrial pathology.

Cytological samples of the endometrium obtained by Endoscann were compared with histological preparations from conventional curettage of the uterine cavity (D & C) in 275 women. Indications of D & C were irregular menstrual bleeding, postmenopausal bleeding, or suspected ovarian or cervical neoplasia in women of 50 years or more. The women were consecutively enrolled in the study and had an average age of 60.2 years (range 50-87). The cytological investigation detected all 16 cases of endometrial cancer but none of 6 cases of atypical hyperplasia. Insertion of the Endoscann cell sampler was easily performed in 87% of the patients in whom cervical dilatation was unnecessary. The cytological technique had 11 cases of insufficient material (4%). By D & C, sufficient material for histological evaluation could not be obtained in 32 cases (12%). The predictive value of a positive test with respect to endometrial carcinoma was 0.38, the corresponding sensitivity was 1.00, and the specificity 0.90. A positive cytology should lead to immediate further investigation.

Early alcoholic liver injury: changes of the Disse space in acinar zone 3.

One hundred and twelve consecutive liver biopsies were studied by transmission electron microscopy. In 18 out of 23 biopsies from chronic alcoholics showing preserved architecture by light microscopy, basal laminas were found in acinar zone 3 applied to endothelial cells as well as to hepatocytes. Basal laminas occurred independently of Mallory bodies and lipogranulomas, and were observed even in cases where light microscopy showed no fibrosis in zone 3. In addition to basal laminas, deposition of collagen and defenestration of the endothelial cells were observed. Basal laminas were seen only rarely in biopsies from non-alcoholics. We suggest that basal lamina formation in acinar zone 3 is initiated by changes in the perisinusoidal space and that it may contribute to impaired liver cell function and possibly initiate liver cell necrosis.

The predictive value of three diagnostic procedures in the evaluation of palpable breast tumours.

In a prospective study of 200 palpable breast tumours of 200 women, the diagnostic specificity and sensitivity of clinical examination, mammography, and fine needle aspiration were compared, alone and in combinations. All tumours were excised and examined histologically, and 38 were malignant. The diagnostic sensitivity of clinical examination was high, 98% (91-99), whereas the specificity was rather low, 48% (36-60). No statistically significant differences were found between the sensitivities or the specificities of the three examinations. When the methods were combined the sensitivity was 100% (96-100) and the specificity was 37% (27-46). Statistically, no improvement of the diagnostic value could be shown. It is concluded that the possibility of distinguishing between malignant and benign palpable breast tumours is not increased by adding mammography and/or fine needle aspiration, but the discovery of two non-palpable malignant tumours by mammography underscores the importance of this method in disclosing subclinical malignant lesions. The study disclosed a statistical possibility of overlooking a few malignant tumours when using these three procedures. Therefore, we advise that all palpable breast tumours should be excised.

Low Ca2+ impedes cross-bridge detachment in chemically skinned Taenia coli.

Muscle force is generated by cycling cross-bridges between actin and myosin filaments. In smooth muscle, cyclic attachment and detachment of cross-bridges is thought to be induced by a Ca2+- and calmodulin-dependent myosin light chain kinase which phosphorylates myosin. The relaxation that occurs after Ca2+ removal is usually ascribed to dephosphorylation of myosin by a phosphatase as non-phosphorylated myosin is unable to form force-generating criss-bridges. Recently, Dillon et al. claimed, however, that dephosphorylation of attached cross-bridges may impede cross-bridge detachment, thus forming so-called 'latch bridges'. Here we present evidence that after a Ca2+- and calmodulin-induced contraction of chemically skinned guinea pig Taenia coli, the rapid removal of Ca2+ impedes the detachment of the myosin cross-bridges from the actin filament; force can then be maintained without energy consumption. The extremely slowly detaching cross-bridges which maintain the force after Ca2+ removal may indeed correspond to the 'latch bridges' mentioned above.

[Double optic papilla in eye coloboma (author's transl)].

The article reports on a 4-year-old girl with multiple congenital malformations. Ophthalmological examination revealed microphthalmia, bilateral colobomas of iris, leukoma of the right eye, convergent strabismus in the left eye. An oval salmon-coloured area was seen at the fundus of the left eye where the retinal vessesl proceed into the depth. There was no vascular infundibulum. A papilla-like cavity was seen at the bottom left of the coloboma (possibly a circulus zinnii, also called circulus vasculosus nervi optici, may be present). Both papillae are connected with each other by means of a vein. This patient, therefore, presented an extremely rare case of malformation with two papillae in one coloboma.
