Intussusception of the appendix with a calcified fecalith.

We treated a patient with a complete invagination of the appendix which contained one large laminated calcified fecalithy. Colonofiberscopy showed a dimpling submucosal tumor, which was palpated as a bony hard tumor at laparotomy. This finding suggested that the fecalith caused an intussusception of the appendix and that such an intussusception should be suspected when there is a bony hard dimpling submucosal tumor in the cecum.

Studies on new antibiotics SF2415. II. The structural elucidation.

Structures of new antibiotics SF2415A1, A2, A3, B1, B2 and B3 were deduced from spectroscopic analyses and degradation studies. The structure of SF2415A3, which has the most antibacterial activity, was proposed to be 3,4a-dichloro-9-diazo-3,4, 4a,10a-tetrahydro-6-hydroxy-2,2,7-trimethyl-10a-[(E)-3,7-dimethyl- 2,6-octadienyl]-(2H,9H)naphtho-[2,3-b]pyran-5,8,10-trione. All of antibiotics SF2415 have a semi-naphthoquinone structure.

[Distribution and constitutional changes of edema fluid in cytotoxic brain edema analyzed by electron microscopy and differential scanning calorimetry].

To understand the pathogenesis of brain edema, we studied distribution and constitutional changes of brain-tissue water by morphological and thermoanalytical methods in cytotoxic brain edema induced by 6-aminonicotinamide (6-ANA). Ninety-two Wistar rats were divided into three groups; Group I rats receiving physiological salt solution intraperitoneally served as controls. Group II and III animals were intraperitoneally given 120 mg/kg and 36 mg/kg of 6-ANA respectively. All animals were starved after injection of drugs to exclude differences in water intake. Then they were decapitated at 3, 6, 12, 24, 36 or 48 hours to measure specific gravity (SG) of large brain tissue (1.1-1.5 g), and to evaluate water content (WC) and free water ratio (FWR) of small brain-tissue samples (15-35 mg) taken from the frontal cortex; WC was measured by a drying-weighing method, and FWR was analyzed with a differential scanning calorimeter. Moreover morphological changes of the frontal cortex of the brain were studied in Group II (n = 12) at 3, 6, 12, 24 and 48 hours with an electron microscope. Neurological status of animals administered 6-ANA (Group II and III) deteriorated with time. Morphological studies showed that perivascular astrocytes and astrocytic processes in the cerebral cortex were swollen most remarkably at 48 hours. However neuronal and endothelial cells were almost intact. The FWR of Group I decreased significantly about four per cent (p less than 0.001) after being starved for 48 hours. But the SG and WC of the group showed little change.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of a selective thrombin inhibitor MCI-9038 on fibrinolysis in vitro and in vivo.

The effect of a selective thrombin inhibitor, (2R, 4R)-4-methyl-1-[N2-[(3-methyl-1,2,3,4-tetrahydro-8-quinolinyl)sulfonyl]- L-arginyl]-2-piperidinecarboxylic acid (MCI-9038), on the fibrinolysis induced by t-PA and u-PA was studied in vitro and in vivo. MCI-9038 remarkably reduced the lysis time of the plasma clot generated by the addition of calcium chloride to the plasma at the concentration ranging from 0.01 to 0.3 microM. Heparin also reduced the plasma clot lysis time with a lower effect than MCI-9038. The fibrin crosslinkage in the plasma clot was inhibited by MCI-9038 or heparin. MCI-9038 potently inhibited the factor XIIIa generation from factor XIII by thrombin. The effect on the in vivo thrombolysis was studied on the arterial thrombosis generated by the endothelial cell injury of the rabbit carotid artery by acetic acid. t-PA dissolved the thrombi with the infusion at 0.96 mg/kg over 2 h without a significant activation of a systemic fibrinolysis. u-PA dissolved the thrombi with the infusion at 180,000 and 360,000 IU/kg over 2 h. At a dose of 0.48 mg/kg t-PA or 90,000 IU/kg u-PA, the thrombi were not dissolved, but the combined use of MCI-9038 at 1.2 mg/kg over 2 h effectively dissolved the thrombi. Thus, combination of MCI-9038 with plasminogen activators accelerated thrombolysis of an experimental thrombosis in rabbits.

Studies on a new antibiotic SF-2330. II. The structural elucidation.

The structure of antibiotic SF-2330 has been elucidated to be 11-hydroxy-5-methyl-2-(2,2'-bioxiran-2-yl)-4H-anthra[1,2-b] pyran-4,7,12-trione by spectral analyses. The olefinic side chain at C-2 of alpha-indomycinone is replaced by a bioxiran-2-yl group in SF-2330.

[A fibrin clot containing of anticancer drug for intra-arterial chemo-embolization therapy. (1) Experimental study on basic characteristics in dogs].

A new embolus consisting of a fibrin clot containing an anticancer drug (mitomycin-C) was devised. An experimental study was then performed using the new embolus in dogs and several interesting characteristics were observed. The fibrin clot containing mitomycin-C was in a fluid from when infused and its action as an embolus did not occur until it reached a target artery. The fibrin clot containing mitomycin-C formed a mold within of the arteries in which it was embolized and was soluble with urokinase during a ten-hour period. The release of mitomycin-C from the fibrin clot was sustained over sixty hours. The intraarterial occlusion formed by fibrin clot was temporary, and recanalization was observed over forty-eight by hours by both angiography and pathological examination.

[A fibrin clot containing of anticancer drug for intra-arterial chemo-embolization therapy. (2) Chemotherapeutic effects on a model hepatoma using VX-2 carcinoma in rabbits].

The chemotherapeutic and ischemic effects of a fibrin clot containing mitomycin-C were observed on a model of primary hepatoma using VX-2 carcinoma inoculated in to the subcapsule of the liver of rabbits. The results of antitumor effect suppression of pulmonary and intrahepatic metastases and survival time using the fibrin clot containing mitomycin-C were better than with other treatments, namely a one-shot infusion of MMC or embolization with fibrin clot only. A synergistic antitumor effect was obtained using embolization with the fibrin clot incorporating mitomycin-C. Using direct subcapsular inoculation with a VX-2 carcinoma block a solitary hepatic tumor was obtained which was hypervascular and resembled human primary hepatoma. These results suggested that a fibrin clot containing an anticancer drug is safe a and effective modality for hepatic or other arterial embolization therapies.

[A long survival case of carcinoma of the colon with hepatic metastasis following intra-arterial infusion chemotherapy].

A 61-year-old man with double colon cancers and a solitary hepatic metastasis underwent right hemicolectomy and catheterization of the proper hepatic artery in April 1979. Intraarterial infusion chemotherapy of 5-FU, to a total of 8,000 mg, and Mitomycin C, to a total of 40 mg, was performed for thirty-two days. During chemotherapy, calcification appeared and occupied the metastatic lesion. The patient is presently in good health and has been disease-free for five years and nine months. In this case, it was supposed that obstruction of the peripheral tumor vessels was the most important causative factor of tumor calcification. This case was clinically valuable because tumor calcification proved to be a marker of tendency toward cure and complete cure of cancer.

[Evaluations of regional cerebral blood flow (rCBF) by 133Xenon i.v. method in patients with occlusions of bilateral internal carotid arteries].

The studies of changes in regional blood flow (rCBF) have been reported on different clinical courses in the five cases (mean 57.4 y.o.) showing bilateral occlusion or stenosis of internal carotid arteries. Values of rCBF using Fg (fast component in gray matter) were closely correlated with their clinical courses. The three patients (2 males and 1 female) showed bilateral focal decreased patterns of rCBF and their uneventful clinical courses except for mild attacks of transient cerebral ischemia. However, the rest two male patients showed bilateral decreased pattern of rCBF and moderate hemiparesis including attacks of loss of consciousness. The one died suddenly because of the thalamic hemorrhage and the other died also suddenly from the unknown etiology. Such differences between the classification of clinical course and hemodynamics evaluated by rCBF was discriminated more clearly by Fg (fast component in gray matter) than by F mean (mean rCBF). The authors concluded that the evaluation by Fg may be valuable to estimate prognosis of patients with bilateral occlusion of internal carotid artery. There are two patterns of decreased rCBF in our study. The patients with bilateral diffuse decreased patterns of rCBF should be followed up more carefully because of the sudden death from cerebral hemorrhage.

Neonatal type of argininosuccinate synthetase deficiency. Report of two cases with autopsy findings.

Two autopsy cases of neonatal argininosuccinate synthetase (ASS) deficiency demonstrating the particular histological changes of the liver are presented. Case 1 was a female infant with elevated blood ammonia and citrulline. The patient died of sepsis at nineteen days after birth. Autopsy revealed hematomas in bilateral cerebral hemispheres and a yellow liver parenchyma. Histologically, the cerebrum showed diffuse astrogliosis with Alzheimer type II cell and swollen cytoplasm. Status spongiosus and gliosis were observed in the subthalamic and pontine nuclei. The liver demonstrated fatty degeneration and wide portal space with bile duct proliferation and inflammatory cell infiltration. The ASS activities in the liver and the kidneys were not detected. Case 2 was a female infant who died of respiratory distress twenty-four days after birth. Autopsy revealed hematomas in the bilateral cerebral ventricles and a liver with yellow parenchyma. Histologically, there was destruction of arrangement in the cerebrum and cerebellum together with marked decrease of nerve cells with gliosis and phagocytosis. The liver showed mild fatty degeneration and wide portal space accompanied by bile duct proliferation and inflammatory cell infiltration. The ASS activities in the liver and kidneys were not detected.
