In 5 patients with respiratory tract infections, concentrations of cefotetan (CTT) in serum and sputum after 1 hour intravenous administration of 2 g of CTT to each patient were investigated, and following results were obtained. Serum levels of CTT The mean peak level of CTT was 232.0 +/- 16.2 micrograms/ml immediately after the 1 hour drip infusion, then declined gradually and was 48.0 +/- 8.8 micrograms/ml after 6 hours. Sputum levels of CTT Mean sputum levels of CTT were 2.15 +/- 0.19 micrograms/ml from 2 to 4 hours after the administration and 2.19 +/- 0.18 micrograms/ml from 4 to 6 hours. Clinical evaluation of CTT Clinical effects were excellent in 1 case, good in 3 cases and poor in 1 case. No side effects were observed.

Pituitary samples were obtained from female rats at various stages of the estrous cycle, and from intact male and gonadectomized rats with and without estradiol treatment. The pituitary extracts with 60% EtOH pH 9.5, were fractionated by preparative isoelectric focusing (IEF), and immunoreactive prolactin (IR-PRL) was measured by RIA. Three types of IR-PRL molecular species were found in these IEF profiles. The first type (species A) was consistently found in an area of pH 4.5-5.4, and consisted of two main subspecies with pls 5.0 (Al) and 5.25 (A2). Species A occupied most part of pituitary IR-PRL in males, gonadectomized animals, and in females in a basal state such as diestrus (D) II 17:00. Species A was also found exclusively in the serum at proestrus (PE) 19:00. The amounts of species A decreased notably when the secretion became active from PE 15:00 to 22:00, then increased at estrus (E) 6:00 and 10:00 when the second type (species B), which was found in the area of pH 5.4-6.8 only in trace amounts at basal states, increased markedly. Species B decreased again at E 17:00, while species A fully recovered. Species B also increased when PRL biosynthesis was stimulated by estradiol in intact male and gonadectomized rats. These findings indicate that species A must be the storage and secretory type of IR-PR, and that species B must be IR-PRL in the biosynthetic process which is to be finally converted into species A. A third type (species C) was found in a region of pH 3.5-4.5 in the IEF profiles of gonadectomized animals. This species is possibly IR-PRL molecules under degradation. When the pituitary was extracted serially with 0.25 M ammonium sulfate pH 5.5 (fraction AMS) first, then with 60% EtOH pH 9.5 (fraction ET), fraction AMS contained mostly species B and C, while fraction ET contained species A almost exclusively. The results obtained with this differential extraction roughly coincided with IEF data, though some disagreements were observed.

In 11 patients with respiratory tract infections, the concentrations of cefmenoxime (CMX) in serum and sputum after 1 hour intravenous administration of 2 g of CMX were investigated. The peak serum level of CMX was 102.2 +/- 11.4 micrograms/ml after 1 hour drip infusion, then declined and was 3.51 +/- 0.55 micrograms/ml after 5 hours. Sputum level of CMX was lower than serum level but it was able to cover enough for MIC of Haemophilus influenzae, Klebsiella pneumoniae, Escherichia coli, Streptococcus pneumoniae and the peak sputum level of CMX was 0.77 +/- 0.17 micrograms/g after from 2 hours to 4 hours. Among the 11 patients with respiratory tract infections, 2 patients showed excellent, 6 patients good, 3 patients poor results (isolated organisms of sputum were normal flora) and no side effects observed.
A 24-year-old woman suffering from left trigeminal neuralgia presented with the familial occurrence of skeletal anomalies simulating Hajdu-Cheney syndrome. She was of a short stature and had low-set ears, protrusion of the bilateral temporal and occipital bones, platybasia, basilar impression, and dental anomaly. Vertebral angiography disclosed marked displacement to the left and dorsally of the basilar artery, which was confirmed during the operation to have compressed the left trigeminal nerve in the cerebellopontine angle narrowed by the unusually protruded bony structure. Cases of Hajdu-Cheney syndrome have occasionally been reported, a few combined with facial spasm, but Hajdu-Cheney syndrome with trigeminal neuralgia is very rare. The patient's family members were surveyed. The possible cause of the neuralgia with its relation to the osteodysplasia is briefly discussed.

A 12-year-old Japanese girl with polyostotic fibrous dysplasia and endocrine concomitants, was treated with elcatonin, a synthetic eel calcitonin analogue, 10 MRC unit/twice a week given by intramuscular injection. Significant decreases in 24 hr urinary content of hydroxyproline and other amino acids from bone collagen were observed during the course of treatment over 5 months. This biochemical result suggests that the synthetic eel calcitonin analogue exhibits the therapeutic effect in patients with polyostotic fibrous dysplasia by inhibiting bone resorption.
