Full-thickness burn injury of the scalp, with or without necrosis of underlying bone, is a complex therapeutic problem. Inability to diagnose depth of injury, controversy regarding wound closure, and a high incidence of acute morbidity are all associated with it. We reviewed the records of 17 patients who had suffered such injury. Wound closure was accomplished by split-thickness autograft alone in three patients, by bone dermabrasion and split-thickness autograft in six, by bone excision and split-thickness autograft in five, and by immediate local rotation flap in three. Systemic and/or local septic complications developed in 50% of all patients who underwent bony debridement. When feasible, early excision followed by immediate flap coverage is the procedure of choice. It avoids the multiple operative procedures required by the more conservative approach to wound closure, thereby shortening the period of primary hospitalization and virtually eliminating the risk of sepsis.
Dissolution studies using both basket and paddle methods were carried out to evaluate two prednisone standards. Results of the experiments showed that the USP prednisone calibrator is sensitive to perturbations by the basket method but not to perturbations by the paddle method. However, the National Center for Drug Analysis (NCDA) prednisone performance standard is sensitive to perturbations by the paddle method but not to perturbations by the basket method. These results suggest that no single standard can predict the suitability of the dissolution equipment by the basket and paddle methods.

To establish an enrichment system of high efficiency for recovery of Campylobacter jejuni from market chickens, the effects of the temperature, duration of incubation, and pH of the enrichment culture on the isolation of the bacterium were evaluated. Whole chickens or chicken parts in plastic bags were individually rinsed, and the washings filtered through cheesecloth. The cells were separated from the washings by centrifugation, and the pellet was inoculated into 100 mL of enrichment broth. Isolation of C. jejuni from poultry samples was significantly increased by incubating these samples in an enrichment medium at 42 degrees C as opposed to 35 degrees C; for 48 h as opposed to 24 h or 72 h; and at pH 7.0 as opposed to pH 6.0, 6.5, 7.5, or 8.0.
A 20-yr prospective study of the incidence of primary drug-resistant tuberculosis among children treated at the Kings County Medical Center was undertaken in January 1961 and extended through December 1980. There were 355 strains of Mycobacterium tuberculosis isolated during this time, 56 of which were found to be primarily resistant to one or more antituberculosis drugs, giving an overall resistance rate of 15.8%. The study was divided into five 4-yr periods. The resistance rate to isoniazid was 9.9%, varying from a peak of 15.2% in the third period of study (1969 to 1972) to 4.5% in the last period of study. The changes in the rate were not significant. The overall resistance rate for streptomycin was 9.2%. There were significant increases in the resistance rate in the second (1965 to 1968) and third (1969 to 1972) periods of study, but not in the last 2 periods. The rates for PAS (3.4%), rifampin (1%), and ethambutol (0.7%) were low. The type and severity of disease among those infected with a resistant strain were no different from those infected with a susceptible strain. Life-threatening disease was found in 10 of the 56 patients infected with a drug-resistant strain. There was one fatality in a child with meningitis who was treated early in the study. Our experience suggests that rifampin and ethambutol be included in the initial treatment regimen of all children with a life-threatening form of tuberculosis until the susceptibility pattern of the infecting strain is determined, after which the drug regimen can be modified if necessary.
The complex metabolism of rabbit lung fibroblast cultures maintained over 24 days in vitro and their interaction with the fibrogenic group of asbestos minerals is reviewed. The differential effects of nine dusts given as a single dose to 3-day-old fibroblast cultures which were then analyzed for DNA, protein and cell mat hydroxyproline after 3 weeks are discussed. A comparison is made of the ability of these minerals to promote reticulin deposition in vivo following instillation into rats with the ability of each mineral to alter cell mat hydroxyproline levels in fibroblast cultures. Because this comparison is poor for some of the minerals investigated, it is concluded that with the current methodology the fibroblast cultures do not provide an adequate in vitro test system for assessing the potential fibrogenicity of a mineral in vivo.

Exsanguination due to hemorrhage caused death in 18 percent of patients with penetrating iliac vessel trauma. Improved survival demands earlier recognition and expedient operative control. One hundred fourteen patients were treated at a single institution during an 11 year period. Gunshot wounds were the predominant cause of injury and combined arteriovenous injury was present in 40 percent of the patients. Twenty-one of the 28 deaths were directly related to vascular injury. All deaths occurred in patients who were initially hypotensive. Seventy-six percent of patients presented with entry wounds below the umbilicus, positive abdominal examination, and hypotension. Earlier recognition is based on this triad. Failure to respond to volume repletion portends a grave prognosis, but it is often reversed by rapid operative intervention. All hypotensive patients with entry wounds below the umbilicus and positive abdominal examination should be taken immediately to the operating room for resuscitation and operation.
Rats were examined for biochemical changes at the lung surface and in lung tissue 1, 4, and 13 weeks after a single instillation of the soluble of insoluble fraction of stainless steel welding particles, or potassium dichromate containing concentrations of hexavalent chromium (CrVI) equivalent to those found in the welding particles. Most of the toxicity of the welding particles 1 week after instillation could be related to the content of soluble CrVI, though the insoluble particles also produced changes at the alveolar surface. The regression of inflammatory changes 4 and 13 weeks after instillation was probably due to the removal of soluble components such as CrVI from the lung.
