[Calcitonin determination as a possibility for the evaluation of surgical result in medullary thyroid carcinoma (C-cell carcinoma)].
In 4 patients with medullary thyroid carcinoma serum calcitonin was measured before and after surgical treatment. In 3 cases serum CT was normalized after operation, whereas one case showed persisting hypercalcitonism as a hint for metastasation. Selective venous catheterization with CT-determination was useful for lacalizing metastases.

On the efficacy of piracetam in geriatric patients with acute cerebral ischemia: a clinically controlled double-blind study.
Using a randomized, double blind group comparison, the efficacy and tolerance of piracetam as an additional therapy of hydroxyethyl starch, venous infusion and low dose heparin treatment, which are the basis of therapy in our hospital, has been studied in patients displaying acute cerebral ischemia. A total of 56 patients were enrolled, of whom 27 were given piracetam and 29 served as a control during a 28-day period. Using single photon emission computer tomography (SPECT) analysis, we observed that in 23 piracetam-treated patients (85.2%) a reduction in the area of brain regions displaying an impaired flow rate occurred (P < 0.001; Fisher's exact two-sided test). Only six of placebo-treated patients (20.7%) showed an improved flow rate. Analysis of the recorded computer tomography map gave an improvement coefficient >2 (flow rate marginally or better improved) for 23 piracetam patients as compared with 5 placebo patients (P < 0.001, Uleman test).
Protein synthetic rate in the sheep placenta in vivo: the influence of insulin.
The metabolic gradient between the placenta and maternal tissues is large when expressed as fractional protein synthesis rate; the half-life of the mixed placental proteins is one-fifth of these in the maternal liver, but a little longer than those of the fetal liver and heart. This high turnover rate provides the priority of the uptake of the feto-placental unit for nutrients, to satisfy the genetic growth potential, enabling equivalent weights of the conceptus to take up amino acids in greater quantities than the maternal liver, the adult organ with the highest protein synthetic rate. The results suggest that fractional synthesis rate of the placental proteins was increased when fetal plasma insulin concentrations were raised. Should this enhancement of protein synthetic rate occur in the human subject, it might be responsible for the heavy placentae found in diabetic subjects whose blood sugar is uncontrolled, and account for their apparently large functional capacity.
Intensive care and the elderly.
During the last 20 years the number of elderly patients treated in our intensive care units (ICUs) has almost doubled. Today about 30% of all ICU-patients are more than 70 years of age. Therefore, this subject has become more and more important, both in public policy and for scientific research. In the following, the author will report data collected on the elderly patients admitted to ICUs, such as length of stay in the ICU, costs, mortality rate, quality of life after discharge and acceptance by the patient. Furthermore, special situations such as cardiopulmonary resuscitation and mechanical ventilation are discussed in detail. The question of whether patients should be admitted from nursing homes cannot be answered completely. Finally, forensic and ethical aspects of intensive care in the elderly are examined.
