A series of 522 patients with a diagnosis of squamous cell carcinoma or carcinoma in situ of the tongue were reviewed and followed. Leukoplakia was found in 88 of these patients before and after the diagnosis of tongue carcinoma. A high incidence of multiple carcinomas in the oral cavity and pharynx (116 times greater than expected) was encountered in tongue carcinoma patients. The observed incidence of multiple oral carcinomas in the group with tongue carcinoma with leukoplakia was five times greater than that of the subjects without leukoplakia. The incidence of secondary esophageal carcinomas was also high, 12 times greater than the statistical expection. However, there was no increased occurrence of secondary esophageal carcinomas in the patients with leukoplakia.
The role of radiation therapy as part of the multi-modal treatment of head and neck cancer appeared to be important with respect to functional and cosmetic morbidity. As for oral cancer, the value of brachytherapy was excellent in cases of localized disease. As far as the latter technique was applicable, patients with tongue cancer at T1, T2 and T3 could be treated by radiation as the initial choice unless palpable neck node metastasis was present. In contrast, cure of the lesion would not be expected from external irradiation alone even when combined with chemotherapy. Accordingly, in such advanced cases, salvage surgery should be applied with or without radiation and/or chemotherapy. As for maxillary cancer, the introduction of "trimodal therapy" has improved the local control rate only to a slight degree. After the application of conservative surgery followed by conventional trimodal combination therapy, local control and five-year survival rates have been improved markedly.

The results of radiation therapy in 152 patients with carcinoma of the major and minor salivary glands during the past 30 years were analyzed. Long-term survivors with recurrent disease and/or distant metastasis were common in the minor salivary glands as compared to the major salivary glands. The effects of radiation therapy depend on the degree of residual tumor after surgery. The treatment of choice for the tumor must be the surgery. However, local control was obtained by radiation therapy in many patients treated with brachytherapy or intracavitary electron beam in the minor salivary glands with a slight degree of residual tumor.

In the present study, the induction efficiencies of chromosome aberrations were analyzed in patients receiving various modalities of interstitial radiotherapy with small radiation sources in the oral cavity area and compared with those in patients treated with external telecobalt irradiation in the thoracic region. Further, as a local nonstochastic effect, the acute mucosal reaction was investigated. The mucosal reaction in the area treated by brachytherapy reached a maximum at two to three weeks after the implantation. The frequencies of dicentrics plus rings of peripheral lymphocytes, on the other hand, revealed rapid increases and approached plateau levels as early as two days after the implantation. The whole-body effects, evaluated on the basis of the chromosome aberration frequencies of peripheral lymphocytes in the patients who underwent brachytherapy, were compared with the effects observed in those patients treated with external radiotherapy or bleomycin injection and discussed with regard to their effectiveness in control of the tumor and side effects. The radiation doses used in the patients who received brachytherapy or a single external irradiation were found to exert the same effects on the chromosome aberration induction. However, even in the case of brachytherapy in which whole-body side effects were believed to be trivial, the peripheral lymphocyte count was temporarily reduced to one half or less of the preirradiation level. An equivalent whole-body dose of 50 cGy was obtained from the frequencies of dicentrics and rings.
Autopsy was performed on 67 patients who had squamous cell carcinoma of the maxillary sinus. These patients had undergone radiotherapy and died during a 30 year period, which ended for this study in 1982. The clinicopathological picture showed several differences according to changes of the mode of therapy and to the rise in local control rate. Postmortem examinations have shown a decreased death rate from extensive carcinoma, and increased distant metastases after the introduction of megavoltage irradiation and trimodal combination therapy (radiotherapy + surgery + intraarterial infusion of antimetabolites). In addition, the introduction of aggressive chemotherapy with antimetabolites and antibiotics has given rise to several gastrointestinal complications.

We have investigated the usefulness of combination therapy with radiation and HCFU for advanced and recurrent breast cancer according to a clinical controlled multicenter trial from 1982 to 1984. One hundred cases were registered and 82 of them were available. Treatment sites were the lymph nodes, skin, bone and lung, and the overall response rate was 58% in CR and 19% in PR, while the duration of remission was 18 weeks in CR. Side effects were found in 10% of the patients. Combination therapy with radiation and HCFU may be useful in multimodal treatment for advanced recurrent breast cancer.

Comparison of three pathologic classifications (Rappaport, LSG and Working Formulation) was made for 292 cases of non-Hodgkin's lymphoma who were treated at the Department of Radiology, Tokyo Medical and Dental University, from 1952 to 1984. The Working Formulation was found to be the most convenient classification for the treatment of non-Hodgkin's lymphoma in Japan. There were two remarkable characteristics in the cases of non-Hodgkin's lymphoma classified by the Working Formulation. They were better case distribution having a strong relation to clinical prognosis and fewer high-grade cases in Waldeyer's ring lymphoma.
Four hundred and sixteen patients with carcinoma of the maxillary sinus received primary therapy consisting of external beam irradiation alone or in combination with surgery and/or chemotherapy at the Department of Radiology, Tokyo Medical and Dental University Hospital, between 1953 and 1982. In our institution, methods of treating cancer of the maxillary sinus have been changed from time to time and showed different control rates and clinical courses. An actuarial 10-year survival rate of 21% has been obtained by megavoltage irradiation alone as well as 34% actuarial 10-year survival rate by megavoltage irradiation with surgery. After the introduction of conservative surgery followed by conventional trimodal combination therapy (antrotomy: intraoral partial maxillectomy + radiation + intra-arterial infusion), the local control rate has been improved. The amount of functional, cosmetic, and brain damages have been remarkably decreased by this mode of therapy. The actuarial five year survival rate was 67%. In addition, along with the improvement of the local control rate, the control of nodal and distant organ metastases have been emerging as one of the important contributions to the prognosis of this disease.
Thirty-eight patients with hard palate carcinoma and 82 with upper gum carcinoma received primary therapy consisting of radiation therapy alone or in combination with surgery. Fifty-three per cent of the primary lesions were successfully controlled by initial radiation therapy. Radiation therapy did not seem to give inferior results compared with surgery in the treatment of the primary lesion. The most important determining factors in the prognosis were the presence of cervical metastases, and bone involvement.
