[Estimation of vasopressin treatment in the upper GI bleeding using Doppler flowmetry].

The effect of intravenous vasopressin infusion in 5 patients with upper GI bleeding was studied by measuring portal blood flow velocity with pulsed Doppler ultrasound technique. In 4 of 5 patients portal blood flow velocity decreased at 1 hr after vasopressin infusion. In all patients the velocity decreased at 24 or 48 hrs. We think doppler is safe and useful method in the evaluation of vasopressin infusion therapy.

A new triphenylethylene derivative, TAT-59; hormone receptors; insulin-like growth factor 1; and growth suppression of hormone-dependent MCF-7 tumors in athymic mice.

TAT-59 ((E)-4-[1-[4-[2-(dimethylamino)ethoxy]-phenyl]-2-(4- isopropyl)phenyl-1-butenyl]-phenyl-monophosphate) treatment was performed on hormone-dependent MCF-7 tumors in athymic mice. TAT-59 given at 1, 5, and 20 mg/kg inhibited the estrogen-stimulated growth of MCF-7 tumors in athymic mice in a dose-dependent fashion. The most clear decrease in tumor growth was shown in the TAT-59 alone group, although it was not dramatic. Average serum concentrations of DP-TAT-59((Z)-[1-[4-[2-(dimethylamino)- ethoxy]phenyl]-2-(4-isopropyl)phenyl-1-butenyl]-4-hydroxybenzene) and DM-DP-TAT-59(desmethyl-DP-TAT-59), metabolites of TAT-59, increased in a dose-dependent manner. Much higher levels of DP-TAT-59 and DM-DP-TAT-59 were shown in tumors (target tissues of estrogen) as compared with muscles (nontarget tissues of estrogen) or serum. A serum concentration of DP-TAT-59 or DM-DP-TAT-59 corresponding to the physiologic levels of serum estradiol in premenopausal women was sufficient to inhibit the estrogen-stimulated growth of MCF-7 tumors in mice. TAT-59 induced a dose-dependent increase in estrogen receptor levels in the MCF-7 tumors. In contrast, it prevented the estradiol (E2)-induced increase in progesterone receptor levels in a dose-dependent manner. Insulin-like growth factor 1 levels measured in the MCF-7 tumors significantly decreased in the TAT-59 alone group and in the no treatment group as compared with the E2 alone group. These results show the pronounced antiestrogenic action of TAT-59 on hormone-dependent MCF-7 tumors in athymic mice.

Day-night variation of urine volume in Parkinson's disease.

Studies on the circadian rhythm of urine excretion in healthy men have demonstrated that the maximal urine flow occurs in the early afternoon and the minimal around midnight. In this study, an abnormality in the variation of urine volume was found in parkinsonian patients. Urine samples were collected during daytime (9:00-21:00) and nighttime (21:00-9:00). Fifteen healthy control subjects were examined and found to excrete 60% during the daytime and 40% during the nighttime of the total urine volume. Sixteen parkinsonian patients excreted 43% during the daytime and 57% during the nighttime. In contrast to the control subjects, the parkinsonian patients excreted a smaller volume of their urine during the daytime than during the nighttime. This finding might be related to the degeneration of dopaminergic and/or nondopaminergic neurons in the brain which control urinary excretion.

Expression of proliferating cell nuclear antigen in invasive ductal carcinoma of the breast.

Formalin-fixed, paraffin-embedded sections from 92 breast cancers (invasive ductal carcinomas) were immunostained with a monoclonal antibody against proliferating cell nuclear antigen (PCNA). The labeling index of PCNA ranged widely from 2 to 76 (mean 24.8)%. The labeling index was classified into three groups: low (< 25%), intermediate (25-50%), high proliferation (> 50%). Younger patients seemed to have a higher labeling index than older ones. The labeling index for tumors < or = 2 cm was lower than that of tumors larger than 2 cm. The labeling index in patients with high estrogen receptor (ER) levels (> 100 fmol/mg cytosol protein) was significantly lower than that in patients with low ER levels (< 10 fmol/mg cytosol protein). In relation to histological type, the labeling index of scirrhous carcinoma was significantly lower than that of papillotubular carcinoma or solid-tubular carcinoma. The high proliferating group had a significantly worse overall survival rate than the low proliferating group. Labeling index was shown to be a possible prognostic indicator.

Periodic leg movements in sleep with restless legs syndrome: effect of clonazepam treatment.

Fifteen patients with restless legs syndrome underwent whole-night polysomnographic recordings before and during clonazepam treatment. The treatment with 0.5 to 1.5 mg clonazepam improved subjective complaints of all the 15 patients. All the patients presented periodic leg movements on the polysomnograms before the treatment. The clonazepam treatment significantly decreased the total numbers of leg movements and the numbers of leg movements per hour without affecting the mean intermovement interval.

Usefulness of antiparkinsonian drugs during neuroleptic treatment and the effect of clonazepam on akathisia and parkinsonism occurred after antiparkinsonian drug withdrawal: a double-blind study.

Antiparkinsonian drugs used for 117 chronic schizophrenic patients receiving long-term neuroleptic treatment were withdrawn. Seventy-eight (66.7%) of the 117 patients were without akathisia and/or parkinsonism at least for 6 weeks after the antiparkinsonian drug withdrawal. A double-blind study of clonazepam was carried out for 22 patients and clonazepam was effective on 8 patients (100%) with akathisia and on 3 patients (75%) with parkinsonism. The authors conclude that these data support the need for discontinuous use of antiparkinsonian medication during the long-term neuroleptic therapy of chronic schizophrenic patients and the effectiveness of clonazepam in managing antiparkinsonian drug withdrawal-induced akathisia and parkinsonism.

Nocturnal myoclonus observed in a patient with neuroleptic-induced akathisia.

An all-night polysomnogram was twice recorded in a patient with neuroleptic-induced akathisia (NIA). The polysomnogram revealed nocturnal myoclonus. After disappearance of akathisia by the administration of clonazepam, we recorded the second polysomnogram. At the second examination, sleep efficacy increased and the total number of nocturnal myoclonus decreased remarkably. The mean inter-movement interval of nocturnal myoclonus prolonged. These findings suggest a close relationship between the mechanisms of neuroleptic-induced akathisia and nocturnal myoclonus.

[A case report of AML M0:CD7, 33 (+) AML M0 case initially presented with cervical lymphadenopathy].

A 59-year-old man was admitted because of generalized lymphadenopathy with fever and vomiting. His peripheral blood showed leukocytosis with a WBC of 93,500/microliters, and the bone marrow picture revealed a predominance of blast cells. The blasts were negative for peroxidase, alpha-naphthyl butyrate esterase and PAS, and had the phenotype of CD 7, 13 and 33 positive. A diagnosis of AML M0 was made, based on the criteria of the NCI-sponsored workshop in 1988. His initial status had been compromised by acute renal failure which necessitated hemodialysis. He responded partially to chemotherapy consisting of daunorubicin, cytarabine and prednisolone. However leukemia recurred and the patient suffered from various episodes of infection and died six months after admission. The Southern blotting showed the germ line configuration for TCR-beta chain and immunoglobulin heavy chain genes. No messenger RNA was detected for myeloperoxidase, c-myc and c-jun, while c-fms, c-fos and c-myb were expressed on Northern blotting. It is intriguing to detect c-fms and c-fos expression in these poorly differentiated leukemic cells.

A clinical study on intellectual impairment in parkinsonian patients during long-term treatment.

The severity of intellectual impairment of 89 patients with Parkinson's disease was evaluated with the Osaka Intelligence Scale for the Aged (OISA). Based on their intelligence levels, the patients were divided into three groups: normal, slight dementia and dementia. EEGs, psychiatric complications such as hallucination, degree of motor disability, history of medication, relationship between changes in intelligence and total amount of administered drugs were also examined. There was no significant correlation between the length of duration of the illness and the degree of intellectual impairment. Two characteristic subgroups were found among our subjects: a group of patients who rapidly became demented after the onset of Parkinsonism, and a group of patients whose intelligence was preserved for a longer period. The mean age of the onset of the disease in the former group was older than that of the latter. Patients of the former group exhibited psychiatric complications and EEG abnormalities more frequently. The severity of motor disability and medications administered at the time of the OISA examination did not differ between the two groups. Deterioration of the intelligence of the patients with Parkinson's disease did not correlate with the total amount of the administered antiparkinsonian drugs.

Low serum iron in patients with neuroleptic-induced akathisia and dystonia under antipsychotic drug treatment.

The concentration of iron was determined in the serum of 35 patients; 10 with akathisia, 8 with dystonia and 17 without akathisia or dystonia. The patients exhibiting akathisia or dystonia had significantly lower serum concentration of iron than the patients without akathisia or dystonia. Neuroleptic-induced akathisia or dystonia may be facilitated by the low serum iron.

A survey of the living conditions and psychological states of elderly people admitted to nursing homes in Japan.

We investigated the living conditions and the prevalence of depression in 920 elderly subjects admitted to 32 nursing homes. Their prevalence of depression was compared with that of 1153 elderly subjects living in the community. The prevalence of depression was determined by the Zung Self-Rating Depression Scale. Over half of the nursing home subjects were admitted directly from their familial residence and had been under supervision for more than 3 years. Most subjects participated in outdoor leisure activities, attended recreational events, communicated freely with others, and were visited by their children and relatives. Only 10% had living spouses. We evaluated 61% of the elderly people in the nursing homes and 36% of the elderly people in the community as depressed, a statistically significant difference.

[Acute renal failure due to endocapillary proliferative glomerulonephritis in a patient with IBL-like T-cell lymphoma].

A 72-year-old man was admitted of generalized lymphadenopathy and oliguria on December 12, 1987. Laboratory findings revealed progressive renal impairment, polyclonal hypergammaglobulinemia, and reduction of serum complements. A cervical lymph node was typically suitable for histology of IBL-like T-cell lymphoma. The surface markers of lymph node were mainly CD2 (+) and CD3 (+) and clonal proliferation of lymphoma cells was proved by TCR-beta gene rearrangement. Renal biopsy to examine the pathogenesis of acute renal failure revealed endocapillary proliferative glomerulonephritis without invasion of lymphoma cells. Both lymphadenopathy and renal failure were improved by successful administration of prednisolone and hemodialysis. Although relapsed tumor was partially responded to vincristine and prednisolone, he died of alimentary tract bleeding. We reported a case of IBL-like T-cell lymphoma with acute renal failure due to endocapillary proliferative glomerulonephritis.

[Altered expression of protooncogenes during clinical course in an AML case transformed from MDS].

The changes of expression of oncogenes in the mononuclear cells of MDS case was studied during his clinical course, in series. His bone marrow was considered to maintain its function partly in initial stage, since both peripheral blood and bone marrow responded to clinical episodes. However, his hematopoietic function was gradually impaired with the disease evolution to AML. We examined the expression of four oncogenes in the mononuclear cells of his three clinical stages, early RAEB-t, RAEB-t and AML, to study the cause of transformation from MDS to AML. Early RAEB-t cells expressed all oncogenes studied other than c-myb, while only c-myc was weakly observed in RAEB-t. AML cells expressed c-myc, c-jun and c-myb, except for c-fms. The expression of c-fms and c-jun of early RAEB-t was considered to reflect the monocytosis induced by infections, and the expressions of c-myb and c-myc of AML cells were regarded as one of malignant signs of tumor transformation. These findings suggest that the evolutional transformation of MDS to AML was affected by the altered expression of oncogenes.

[bcl-2 gene rearrangement and its expression in tumor cells].

Both the rearrangement and the expression of the bcl-2 gene in Japanese hematopoietic tumor cells were studied by Southern and Northern blot hybridizations. The expression of the bcl-2 gene was studied in seven cultured cell lines and twenty clinical samples, including eleven non-lymphoid tumors and nine lymphoid tumors. All lymphoid tumors except one sample from a patient with ALL expressed the bcl-2 gene. The bcl-2 gene was strongly expressed in B cell tumors. This gene was also expressed in T-ALL samples studied, although the expression was not as marked as in the B cell tumors. Non-lymphoid tumors did not express bcl-2 gene. bcl-2 gene rearrangement was studied in five cultured cell lines and six clinical samples including four follicular lymphomas and two T-ALLs. No abnormal bcl-2 gene configurations were found in any of the clinical samples. Among the cultured cell lines, the BALL-1 line showed two-fold amplification. The frequency of bcl-2 rearrangement in Japanese follicular lymphomas is reported to be lower than that seen in the American follicular lymphomas. Nevertheless, the increased expression of the bcl-2 gene seen in the Japanese B cell tumors studied supports the contention that the bcl-2 gene plays an important role in the pathogenesis of B cell tumors.
