[Significance of flow cytometry in the prognosis of tumors of the kidney in adults].

The DNA index was measured by flow cytometry on paraffin-embedded tissue blocks, from a series of 50 renal tumours studied retrospectively. These tumours consisted of 48 renal cell carcinomas and 1 oncocytoma. The mean follow-up was 3 years. TNM classification and Syrjanen's combined histological grading were used. There was a significant correlation between ploidy and nuclear grading (P = 0.02); between ploidy and TNM classification (72% of diploid tumours were T1 and T2, M0, N0, P = 0.01); between ploidy and development of metastases during the follow-up (71% of aneuploid tumours developed metastases, P = 0.04). In contrast, ploidy was not related with tumour differentiation (P = 0.052), and did not influence the overall long-term survival (P = 0.2). The small number of patients studied did not allow a significant survival rate to be established for each stage according to ploidy. The measurement of the DNA index by flow cytometry is prognostically useful for renal tumours, but must be compared with other prognostic parameters. Its therapeutic value will only be able to be assessed in the future; especially in conservative surgery indications, possibly in the choice of complementary therapeutic associations and even in the approach to treatment of metastases.

[Transient ischemic colitis. Diagnostic value of patchy epithelial atrophy].

Microscopic findings, observed in 58 colonic biopsies, have been described by the authors during transient ischemic colitis. One of these lesions, closely associated with the diagnosis and named patchy tubular atrophy, was considered as a marker for ischemia. Patchy tubular atrophy was observed at the beginning of the disease, and was regularly associated with normal and necrotic areas. Three morphological aspects characterized this condition: --non secretory glandular crypts covered with rudimentary basophilic epithelium, --crypts regularly distributed in a geometrical pattern with intact connective spaces, --absence of interstitial inflammatory infiltrate, and presence of fibrinous thrombosis with extravasation of erythrocytes. This morphologic pattern was specific enough to rule ou the other inflammatory or trophic bowel diseases. Therefore patchy tubular atrophy, observed during transient ischemic colitis, was considered by authors as a distinct entity.

The hepatotoxicity of 1-(2-chloroethyl)-3-cyclohexyl-1-nitrosourea (CCNU) in rats. Ultrastructural evidence of a delayed microtubular toxicity.

A few cases of liver involvement have been reported in patients receiving treatment with the antineoplastic nitrosourea CCNU. A single oral dose of 20 or 50 mg/kg CCNU in female Wistar rats induced an important increase in transaminases between day 2 and day 6, followed by a second, moderate increase between day 21 and day 28. Alkaline phosphatases and conjugated hyperbilirubinemia (threefold-increase) were noted for the two doses and were greater for the highest dose. Histological and ultrastructural studies disclosed hepatic lesions of two types: during the first phase of transaminase increase, inflammation of the portal tracts; during the second phase marked dilation of bile canaliculi and numerous filamentous bundles distributed at random throughout the liver cell cytoplasm like normal microtubules. Thus, CCNU induced pericholangitis and intrahepatic cholestasis with microtubular abnormalities. The long-term evolution of hepatic alterations revealed that in the 3rd month after a single oral dose of 20 mg/kg CCNU, lesions were persistent but stable; no reversibility was observed in the 3rd month after 50 mg/kg CCNU, and evolution towards cholangiolysis and biliary cirrhosis was noted. We suggest that CCNU causes a bimodal hepatotoxicity in rats: an early and prolonged ductal injury and a delayed anti-liver cell microtubule toxicity.

[Fibromatosis in children: regression with prolonged chemotherapy. Apropos of 2 cases].

Surgical excision is the usually recommended treatment of Fibromatosis in children. However the invading nature of this tumor is responsible for recurrences leading to mutilating surgery and eventually to death despite the histologically benign appearance. We observed two cases of decrease under lengthy chemotherapy. In the first case, a 4-year-old boy with a huge abdominal wall tumor had two biopsies leading to the diagnosis of fibromatosis. The tumor measured 15 x 14 x 10 cm, and compressed the right ureter when chemotherapy was started. Actinomycin 1,500 microgram/m2 at Day 1 and vincristine 1.5 mg/m2 at Day 1 and Day 8. This courses were repeated every four weeks. Tumor decreased slowly but constantly. After 17 months it measured only 5 cm in diameter. The biopsy confirmed the nonprogressive nature of the residual fibromatosis. In the second case, a 12-year-old boy had a tumor of the left anterior thoracic wall. The histologic study diagnosed a fibromatosis. Two courses of IVA (ifosfamide, vincristine, actinomycin), did not lead to any decrease. A wide excision was achieved but left invaded edges. Then the child was treated by actinomycin 750 microgram/m2 at Day 1 and vincristine 1.5 mg/m2 at Day 1 and Day 8, one course every four weeks during ten months. At this time biopsies revealed a scar without progressive fibromatosis. This two cases show the interest of a chemotherapy in extensive fibromatosis. A mild treatment but for a long time seems to succeed where more intensive courses have failed.

[Dura mater healing after repair with aponeurotic plasty. Comparison of results between classical sutures and fibrin adhesive glue. Fibrin adhesive in frontobasal injuries. An experimental study].

The aim of the present experimental study was to compare the quality of the healing obtained with fibrin glue or with classical sutures to repair post-traumatic fronto-basal fistulae. The anterior part of the cranial base and a piece of dura mater are removed. The first group of rabbits (n = 26) is repaired with fibrin glue; the second group with separate sutures. The postoperative controls are realised by injection of 1 ml patent blue in the cisterna magna to detect C.S.F. rhinorrhea, x-ray study of the cranial bases and histological samples of transversal slices of the cranial base. The different biological findings seem to be important arguments to confirm the useability and trustworthiness of human origin fibrin glues to close meningeal wounds.

[Morphometric study of variations related to human aging in pulp unmyelinated and myelinated axons].

The evolution of myelinated and unmyelinated axon population in dental pulps during human aging was followed by electron microscopy and morphometry. A total of 2684 nerve fibers were measured in 16 subjects aged 10 to 72 years. The total distribution is bimodal with a recovering zone of 9.8% between unmyelinated and myelinated fibers. The mean diameter of A delta fibers was 1.96 mu and that of C fibers was 0.52 micron. The old pulps showed a loss of A delta fibers of small sizes (0.35-2 micron), the presence of numerous unmyelinated C fibers of very small diameters (0.1-0.25 micron) and a decrease of unmyelinated fibers of 0.5 to 1 micron comparative to young pulps. The logarithms of the diameters unmyelinated axons varied linearly with that of the subjects ages (CR = 0.63; p = 8, 8.10(-3]. The older the patients, the more the fiber C diameters were reduced. The decrease in numbers of A delta fibers could result from a local reduction of pulpal volume related to age or a response to a more general aging process of cerebral origin. The decrease in diameter of unmyelinated fibers in aging pulps would be a reduction of metabolic energy.

[Recourse to amniocentesis after the age of 38. Factors related to access to medical care].

Antenatal screening by amniocentesis was not much used in Haute-Normandie (France) in 1981. This study compared 59 women aged 38 and over who did benefit from this test with a control sample of 71 women who delivered in the same region at the same time and who were not screened. The use of the screening was linked to the socio-professional category of the woman, to her professional activity, to her nationality, to where she studied, to her parity, and the quality of her obstetrical care. The information that the women who were screened had about trisomy 21 and its screening was far higher in those who were screened. Regression analysis indicates that there are 3 factors that play an important role: whether the specialist or the general practitioner offered the screening test and the socio-professional category of the woman. Amniocentesis could be used much more efficiently if the population was better informed and if the medical corps made a special effort as well as providing more screening facilities.

[Intestinal stenosis, sequelae of necrotizing enterocolitis. Analysis of 9 cases].

Intestinal necrosis was a complication of necrotizing enterocolitis of the newborn in 9 of 42 cases. In 6 neonates it followed medical treatment of the enterocolitis, presenting in this case as an occlusive syndrome. After surgical treatment (3 cases) the diagnosis was made from radiologic imaging prior to reestablishment of digestive continuity. Surgery was necessary in all cases, and procedures used are described (technic, date of operation). The prognosis of enterocolitis is worsened by this stenosis (1 death in the 9 cases).

[Hepatopathy with a progressive course after one-time administration of (chloro-2-ethyl)-1-cyclohexyl-3-nitroso-1-uréa (CCNU) in rats].

A few cases of liver involvement have been reported in patients receiving treatment with CCNU. Nitrosourea CCNU is an antitumoral agent largely used in the treatment of some types of leukemia, Hodgkin's disease, bronchial or cerebral tumors. It was shown that CCNU induced pericholangitis and intra-hepatic cholestasis in rat: moderate after 20 mg/kg CCNU, these hepatic lesions were maximal on day 8. In the present report we were interested in the evolution of hepatic alterations a long time after cessation of drug administration. So we studied hepatic ultrastructure three months after a unique 20 mg/kg or 50 mg/kg CCNU administration. Lesions were stable after 20 mg/kg CCNU and no reversibility was observed 3 months after 50 mg/kg CCNU: evolution to cholangiolysis, to adenomatous transformation of parenchyma or biliary cirrhosis were noted.

Decreased brush border hydrolase activities without gross morphologic changes in human intestinal mucosa after prolonged total parenteral nutrition of adults.

Animal experimentation with total parenteral nutrition (TPN) has revealed the occurrence of atrophy of the intestinal mucosa and decreased enzyme activities of the brush border, notably the disaccharidases. These findings have heretofore not been confirmed in human investigation. We performed endoscopic biopsies in the third part of the duodenum in 7 adults before TPN, after 21 days of TPN, and after a progressive oral refeeding. We noted a clear-cut decrease of major enzyme activities during TPN (sucrase, maltase, lactase, glucoamylase, acid aminopeptidase, dipeptidyl peptidase) without any morphologic modifications as observed with standard histology. Electron microscopy showed a slight but significant decrease in the height of microvilli. The decreased enzyme activities were rapidly restored after oral refeeding. Thus, the functional consequences of the modifications observed during medium-term TPN in adults are probably limited.

[Morphometric analysis of human pulp myelinated fibers during aging].

In view of the considerable variations in contour shapes noted in transverse sections of myelinated axons from dental pulps a morphometric method involving image analysis (TAS Leitz) was used to calculate directly the surfaces of the nerves in sections of human root pulp using the Bodian stain. The sample studies consisted of 26 patients aged 10.5 to 72 years. The variation in numbers of myelinated pulpal nerves was analysed by age. Of 3606 myelinated fibres measured, 95% had a diameter less than 5 microns (A delta fibres) and about 0.5% had a diameter greater that 7 microns (A beta fibres). The mean diameter of these myelinated fibres was 2.90 microns. The statistical analysis clearly showed a difference in distribution between these fibres in young and very old subjects. The total number of pulpar myelinated fibres decreased with age particularly at the expense of a A delta fibres. The A beta fibres remained constant. This decrease of A delta fibres with age may be related to a reduced sensitivity to the perception of dental pain transmitted by these fast-conducting A delta fibres.

[Amyloid pseudotumor disclosing non-secretory myeloma. Ultrastructural demonstration of the role of histiocytes in intratumoral amyloidogenesis].

A 69 year old woman presented initially with an amyloid tumour of the rib. Six months later, she developed a plasma cell tumour of the left acromion diagnosed as an IgG kappa myeloma. Electron microscopy of the initial costal lesion and the plasma cell tumour showed a close resemblance between the dystrophic plasmacytes, which had no morphological evidence of intracytoplasmic amyloid fibrils, and the histiocytes, many of which contained these fibrils and were probable sites of amyloid secretion. This suggests that the costal amyloid tumour was probably the result of transformation of a previous plasma cell tumour, and that intra-tumoural amyloid deposits have the same origin as the systemic amyloidosis observed in cases of multiple myeloma, i.e. the histiocyte.

[Hepatotoxicity of (chloro-2-ethyl)-1-cyclohexyl-3-nitroso-1-urea (CCNU) in the rat].

Few cases of liver involvement have been reported in patients receiving treatment with CCNU. CCNU is an anti-tumoral agent used in the treatment of leukemia, Hodgkin's disease and bronchial or cerebral tumors. A single daily dose of 20 or 50 mg/kg CCNU in female Wistar rats induces an important increase of transaminases, reaching 10 times initial level between day 2 and day 6, followed by a second and moderate increase between day 21 and day 28. Three-fold increased alkaline phosphatases and conjugated hyperbilirubinemia were noted for the two doses, and was greater for the higher dose. Histological and ultrastructural studies showed two types of lesions: during the first phase of transaminase elevation, edema and inflammatory infiltration of the portal spaces; during the second phase of transaminases elevation, numerous bundles of pericanalicular microfilaments and severe dilation of the biliary tract. Hepatic cells had few alterations although some necrotic foci were observed, particularly at the higher dose. So CCNU induced an intrahepatic cholestasis with pericholangitis in the rat.

[Hereditary visceral myopathy: an entity in idiopathic intestinal pseudo-obstruction].

A 17 year-old girl with chronic idiopathic intestinal pseudo-obstruction is reported. Abnormalities of smooth intestinal muscle were shown on light and electron microscopic studies of the excised small intestine and led to the diagnosis of visceral myopathy based on the following features: vacuolar degeneration of intestinal smooth muscle cells with replacement by fibrous preferential involvement of the external longitudinal muscle layer normal myenteric plexus. For the first time similar ultrastructural changes were found on histological study of the colon. Manometric studies revealed a diffuse disease involving the esophagus, small bowel, and bladder. Anorectal abnormalities, never described before, were reported. Family involvement was shown by abnormal esophageal and anorectal manometries in the patient's brother and by paternal history of fatal small intestine occlusion without mechanical obstruction. The prognosis of severe forms of visceral myopathy is generally poor because of the inefficiency of drugs. In this case, after a long period of parenteral nutrition with maintenance of a good nutritional status, a terminal ileostomy (with a special procedure to avoid evagination) associated with a second stage total colectomy allowed to stop parenteral nutrition.

True hermaphroditism: from female to male endocrine status.

True hermaphroditism was revealed by monthly intrascrotal bleeding in a 21-yr-old subject of male phenotype who had undergone surgical treatment for gonadal ectopy at the age of 7 yr. The presence of an ovary was demonstrated by the endocrine profile of an ovulatory menstrual cycle. Evidence for the presence of a testis was provided by a plasma testosterone increase after hCG administration (5000 IU/day for 3 days) and its spontaneous response to an endogenous preovulatory LH peak. Further endocrine studies revealed that both gonads were stimulated by endogenous gonadotropins. At surgery, a hemiuterus and an ovary with corpus luteum were found in the left hemiscrotum, and a testis and epididymis were found in the right hemiscrotum. After removal of the ovary, the subject passed from a predominantly female to a male endocrine status, which suggests that the endocrine secretion of the testis was inhibited by the negative feedback effect of ovarian steroids on gonadotropin secretion.

[Morphometric study of hepatocytes in steatosis].

By morphometric analysis of fatty liver, we propose to value volume density of fat vacuoles and liver hypertrophy. By this method we appreciate liver cell modification in malnutrition.
