Ectopic pregnancy--potentials for diagnosis using ultrasound and urine and serum pregnancy tests.

The ultrasound findings from 260 patients with a clinical suspicion of ectopic pregnancy have been analysed and correlated with the results of urine pregnancy tests and tests of serum LH and/or HCG levels. Most importantly in a practical clinical context, it was found that a negative serum test virtually excludes an ectopic pregnancy, and an empty uterus with an adnexal mass and/or the presence of free fluid together with a positive urine test gives a very high probability of an ectopic pregnancy. The absolute diagnosis of an ectopic pregnancy by the demonstration of a living fetus outside the uterus is an uncommon finding (8%). Conversely, an empty uterus alone on ultrasound examination in the absence of other ultrasound findings in those patients with a positive serum test is not a reliable guide to the presence of an ectopic pregnancy unless there is an irrefutable conception date at least 5 weeks previously. It is recommended that pathology laboratories and ultrasound departments establish absolute levels of HCG above which an intrauterine pregnancy should always be visible within the uterus. Given appropriate attention to the clinical condition of the patient, the combined use of diagnostic ultrasound, simple urine pregnancy tests and serum assays of beta HCG levels goes a long way to discriminating between those patients with and those without an ectopic pregnancy.

Sea-blue histiocytes in canine ceroid-lipofuscinosis (CCL).

Cells corresponding to sea-blue histiocytes are described in bone marrow aspirates from dogs suffering hereditary ceroid-lipofuscinosis. Sea-blue histiocytes also occur in humans with so-called neuronal ceroid-lipofuscinosis and both exhibit strong autofluorescence. By quantitative fluorometric techniques, each species have similar spectral characteristics and, thus, the English setter with CCL can serve as a useful animal model.

Myeloid differentiation-inducing factors produced by pokeweed mitogen-treated normal G1/0 lymphocytes but not chronic lymphocytic leukemia cells.

HL-60 human promyelocytic leukemia cells undergo myeloid differentiation and G1/0 specific growth arrest in response to an activity produced by pokeweed mitogen stimulated peripheral blood mononuclear cells. Elaboration of this myeloid differentiation inducing activity does not require lymphocytic proliferation or pokeweed mitogen induced B-lymphocyte differentiation. The activity is a product of initially stimulated G1/0 lymphocytes. It is not produced by proliferating lymphocytes. The activity is not elaborated by G1/0 peripheral blood lymphocytes of patients with chronic lymphocytic leukemia, cells which are proliferatively refractory to pokeweed mitogen stimulation.

Estimation of the radiation dose in man due to 6-[18F]fluoro-L-dopa.

The radiation dose to the organs of the human body after an intravenous administration of 6-[18F]fluoro-L-dopa was estimated using the recommendations of the International Committee on Radiological Protection (ICRP). The bladder wall received the highest dose, 6.95E-10 Sv/Bq (2,600 mrem/mCi), and as a consequence the dose to the genitalia was 1.6E-11 Sv/Bq (60 mrem/mCi). The major organs received a dose of 5.66E-12 to 1.87E-11 Sv/Bq (20 to 60 mrem/mCi). The effective dose equivalent was estimated at 5.39E-11 Sv/Bq (200 mrem/mCi).

gamma-Hexachlorocyclohexane stimulation of macrophage phospholipid hydrolysis and leukotriene production.

gamma-Hexachlorocyclohexane stimulates arachidonic acid release from macrophage phospholipids. It is also a powerful stimulator of leukotriene C4 production, yet (by comparison with zymosan) produces only a small effect on prostaglandin production. This suggests that synthesis of leukotrienes and prostaglandins can be independently regulated. The most likely mechanism of action of gamma-hexachlorocyclohexane is through its effect on phosphatidylinositol metabolism.

Treatment of the perineal wound after proctectomy by intermittent irrigation.

Between 1977 and 1983, 43 patients underwent proctectomy (40 patients for carcinoma, three for ulcerative colitis). The subsequent perineal wound received primary closure and the pelvic space was treated with intermittent irrigation and suction drainage using a double-lumen catheter. Primary healing of the perineal wound occurred in 56.4% of cases and in 89.7% of cases the perineal wound had healed completely by the sixth postoperative week. The relationships between perineal wound healing and the age and sex of the patient, the stage of the tumor, and the tumor distance from the anal verge were assessed. The period of hospitalization was significantly reduced when the perineal wound underwent primary healing. The results obtained in terms of wound healing are comparable with the best published results using continuous irrigation. Our method is advantageous in that it does not necessitate immobilization of the patient.

Recurrent retching with gastric mucosal prolapse. A proposed prolapse gastropathy syndrome.

A clinical syndrome is described in 22 patients who present with epigastric pain alone or with hematemesis which is associated with a precedent history of recurrent early morning retching or postprandial retching. The characteristic endoscopic finding is a knuckle of inflamed and sometimes bleeding gastric mucosa which repeatedly prolapses into the esophageal lumen during retching. Other upper gastrointestinal lesions are not found. Retching is thought to cause the forceful prolapse and subsequent traumatization of gastric mucosa. The episodes of vigorous retching and resultant gastric trauma are now considered to be the cause of the abdominal pain and hematemesis, and it is proposed that this combination of clinical features illustrates a distinct syndrome which we have called prolapse gastropathy. This condition is a significant cause of hematemesis and abdominal pain in a group of nonalcoholic patients who demonstrate an unusual sensitivity to gag and retch.

A rapid method for detecting inhibitors of both cyclo-oxygenase lipoxygenase metabolites of arachidonic acid.

A method for detecting inhibitors of arachidonic acid metabolism is described. Guinea pig peritoneal polymorphonuclear leukocytes incubated with [14C] arachidonic acid were stimulated with calcium ionophore A23187. The radiolabelled products were identified by HPLC and GC/MS as cyclo-oxygenase and lipoxygenase metabolites. A TLC system was utilized to separate these metabolites which were then analysed by automatic quantitative scanning. This method allowed rapid determination of the radiolabel incorporated into each metabolite. This method of analysis was used to study the actions of indomethacin, NDGA, benoxaprofen, and BW775C on the formation of radiolabelled arachidonic acid metabolites.

Does obesity affect recovery? A study using intravenous methohexitone and Althesin for short procedures.

One hundred and twenty unpremedicated patients, scheduled for minor gynaecological procedures, were randomly allocated to receive Althesin or methohexitone to supplement N2O and O2 anaesthesia. Patients were assessed as thin, medium or fat according to the degree of obesity as measured by skinfold calipers. The range of mean times for the patients to open their eyes was 3.1-4.0 minutes, to be orientated was 4.4-6.7 minutes and to perform a manipulative test was 27.7-32.1 minutes. No statistically significant differences were seen in these indices of recovery whether they received Althesin or methohexitone or whether they were thin, medium or fat. A paper and pencil test was also performed. Induction doses of the intravenous agent were calculated on a body weight basis and increments given only in response to movement. The mean total dose of Althesin was decreased from 1.33 mg total steroid/kg for thin patients to 1.06 mg total steroid/kg for fat patients (p less than 0.01). The mean total dose of methohexitone was decreased from 3.4 mg/kg for thin patients to 2.9 mg/kg for fat patients which was not significant.

Effect of alfentanil and fentanyl on recovery from brief anaesthesia.

One hundred female patients were allocated randomly to receive either no analgesic supplementation to methohexitone, nitrous oxide in oxygen anaesthesia, or supplements of fentanyl 1.5 micrograms kg-1 or alfentanil 8, 12 or 16 micrograms kg-1. No differences were seen in the time to recover consciousness, regain orientation or manipulate shapes into a box. Analgesic supplementation resulted in a decrease in the requirement for methohexitone, and improved operating conditions. Some patients in each group who received fentanyl or alfentanil became apnoeic for more than 30 s duration. The duration of apnoea increased with larger doses of alfentanil.

Comparative evaluation of the therapeutic efficacy of a pH-sensitive enteric coated pancreatic enzyme preparation with conventional pancreatic enzyme therapy in the treatment of exocrine pancreatic insufficiency.

The therapeutic efficacy of a pH-sensitive enteric coated pancreatic enzyme preparation was compared with conventional pancreatic enzyme preparations in 6 adult patients with exocrine pancreatic insufficiency. Fecal fat excretion and postprandial duodenal recovery of orally ingested pancreatic enzymes were evaluated after ingestion of each preparation. Fecal fat excretion decreased significantly (p less than 0.005) on treatment with pH-sensitive and conventional pancreatic enzyme preparations. Postprandial concentration and delivery of trypsin and lipase in samples aspirated from duodenojejunal junction were higher after ingestion of conventional pancreatic enzyme preparation as compared to the pH-sensitive enteric coated preparation. The difference, however, did not reach statistical significance. Our observations suggest that the pH-sensitive enteric coated pancreatic enzyme preparation is only as effective as conventional pancreatic enzyme preparations in controlling fat malabsorption in patients with exocrine pancreatic insufficiency. Failure of pH-sensitive enteric coated preparation to deliver greater quantities of pancreatic enzymes at duodenojejunal junction is most likely related to the impaired release of enzymes from microspheres due to low intraluminal pH in the upper small intestine in pancreatic insufficiency.

Rheumatoid arthritis in a Chippewa band. II. Field study with clinical serologic and HLA-D correlations.

We present an in-depth study of rheumatoid arthritis (RA) in a Chippewa band. Of the 227 band members, 168 (74%) were evaluated. The unusually high prevalence of RA was confirmed in 7.1% of those studied or, minimally, 5.3% with a 100% completion rate without additional cases found. Seropositivity in those with clinically definite RA was 92% relative to rheumatoid factor and 75% for ANA. Despite the high prevalence (68%) of HLA-DR4 in this closed population, there was a significant correlation of DR4 with RA (100%). The implications of these observations in this population isolate are discussed.

Auscultatory indirect measurement of blood pressure in dogs.

An indirect method of measuring blood pressure (cuff plus stethoscope) was evaluated in 70 dogs weighing 15 to 30 kg (17.5 +/- 8.8 kg; mean +/- standard deviation). A cuff 12 cm wide was used. The measurements were most audible with the cuff on the upper foreleg of the dog and with the stethoscope placed in the medial epicondylar region just distal to the cuff. The cuff was inflated to greater than systolic pressure and allowed to deflate slowly. In 70 lightly sedated dogs, systolic blood pressures averaged 145 +/- 25 mmHg (mean +/- standard deviation) and diastolic blood pressures averaged 84 +/- 14 mmHg. Indirect measurements were compared to direct measurements (femoral arterial catheter). Systolic pressures obtained by this direct method averaged 138 +/- 29 mmHg (mean +/- standard deviation) and diastolic pressures averaged 84 +/- 17 mmHg. The correlation coefficient for systolic pressure was 0.96 and for diastolic pressure 0.97.
