Prenatal diagnosis in the presence of an extra small chromosome in amniotic cell culture.

The amniotic fluid of a 20 years old Caucasoid woman was brought in for analysis because of a previous child with anencephaly. Q-banded karyotypes disclosed 47,XY, +mar. Clinical significance of this karyotype could not be determined. The couple elected therapeutic termination but were denied because the pregnancy was over 23 weeks gestation. Her blood culture revealed also 47,XX, +mar. Silver staining revealed the marker chromosome bisatellited. C-banded karyotypes revealed the marker chromosome with one C-band. This inherited marker chromosome, composed of satellites, stalks, and constitutive heterochromatin without other observable chromosomal material present, suggested that it may be genetically inactive. A normal baby boy was delivered and his 47,XY, +mar karyotype confirmed. In addition to the family study, silver staining and the C-banding procedures are indicated to determine the morphology of satellites, stalks, and constitutive heterochromatin, and the presence or absence of other chromosomal materials in the extrasmall chromosome in amniotic fluid cultures.

Transabdominal exposure of the thoracic aorta.

The thoracic aorta can be exposed from an abdominal approach by means of an incision in the diaphragm. This method has been used extensively by Crawford and colleagues but has not been described in detail. Patients with normal renal function before the operation suffered no permanent renal damage from periods of ischemia for up to 45 minutes under normothermic conditions. One patient with impaired renal function before the operation sustained permanent renal damage.

Cholelithiasis: length-of-stay analysis in two series of surgical patients.

Two series of patients undergoing surgical treatment for cholelithiasis at Hornsby Hospital are described. Data are drawn from the routine morbidity reporting system of the hospital and processed by manual techniques. The morbidity data presented in this analysis demonstrate the capacity of currently operating reporting systems to provide diagnosis-specific information, and to describe the behaviour of relevant subgroups within a "diagnostic population". The need for specificity when attempting to make comparisons of length-of-stay "performance" either between or within hospitals is highlighted.

Comparison between saphenous vein and Dacron Velour grafts for femoropopliteal bypass.

One hundred and thirty-eight femoropopliteal grafts were performed for arteriosclerotic occlusive disease of the femoral artery in a five-year period. One group of patients who had 81 saphenous vein grafts was compared with a similar group who had 57 Dacron Velour grafts. At three years 90% of the vein grafts were patent, compared to 45% of the Dacron Velour grafts. Twenty-seven per cent of all patients presented with threatenened limb loss. Amputation was averted in all but 5%. Intraoperative flow measurement did not prove to be a reliable indicator of long-term patency.

Polytetrafluoroethylene (PTFE) grafts for haemodialysis: patency and complications compared with those of saphenous vein grafts.

A comparison has been made between polytetrafluoroethylene (PTFE) and saphenous vein as graft material for the construction of arteriovenous fistulas for use in haemodialysis. Fifty patients with PTFE grafts have been examined and compared with 70 patients with saphenous vein grafts. At eighteen months the accumulative patency rate was 69.8% for PTFE grafts and 68.9% for saphenous grafts. Although the PTFE grafts were similar in terms of patency, their complication rate was higher. The infection rate and distal ischaemia rate for PTFE grafts were double those of the vein grafts. Two patients developed median and ulnar nerve paralysis respectively shortly after implantation of PTFE grafts. Because of this it is recommended that their use be restricted to the lower limb. Despite a higher incidence of complications, PTFE grafts are a satisfactory substitute if a suitable saphenous vein is not available.

Protein synthesis and aging: studies with cell-free mammalian systems.

A cell-free system devoid of polysomes, which translates natural mRNA, has been prepared from rat liver. It contains ribosomal subunits, ribosomes, aminoacyl-tRNA synthetases, tRNAs, and protein factors necessary for translation. Protein synthesis required an energy-generating system, mRNA, and 3 mM Mg2+ concentration, and it was inhibited by 7-methylguanylic acid. The total extent and the rate of protein synthesis were approximately 30% greater when the translating system was prepared from livers of 3-month-old rats, as compared to 30-month-old rats. A ribosome-free fraction containing the protein factors required for translation was also prepared from 3-month-old and 30-month-old rat livers and brains, by extraction with 0.5 M KCl. The high-salt extracts were analyzed for elongation factors EF-1 and EF-2 in a poly(U) translating system. Although the activity of EF-2 was similar in preparations from young and old rats, the EF-1 activity in the 3-month-old rat livers and brains was 30 to 40% greater than in 30-month-old animals. The protein synthesizing activity of high salt-washed ribosomes stripped of endogenous peptidyl-tRNA and mRNA, from livers and brains of young and old animals, was the same.

Malignant cardiac myxoma with cerebral metastases.

Two cases of cardiac myxoma with secondary involvement of the brain and death are reported. In one case, tumor emboli to the middle cerebral artery caused massive cerebral infarction, while in the other case true cerebral metastasis developed in the cerebrum and cerebellum, with rapid demise ensuing in both patients. It is emphasized that an emergency exists in cases ihat prompt echocardiography and computerized tomography of the brain are indicated. The second case represents the only instance of cardiac myxoma behaving as a true malignant neoplasm with actual invasion of the neural tissues recorded in the literature.

Chylothorax following division of patient ductus arteriosus in an adult.

A case of postoperative chylothorax following division of a patent ductus arteriosus in an adult is presented. The patient recovered following reoperation for ligation of the thoracic duct after an 11-day trial of conservative management. Arguments for conservative and operative management are discussed and the anatomy of the thoracic duct is shown.

Child observation and assessment centres: psychiatrist' and social workers' difficulties.

Two child observation and assessment centres run by a Social Services Department have quickly filled up with older children who stay indefinitely. Many of these children have physical as well as emotional problems and have been admitted as family emergencies in an unplanned way. The paper discusses how, if at all, the psychiatrist called in for advice by social workers can help. This is thought to be a widespread problem.
