Use of castor oil in pregnancies at term.

CONTEXT: Despite wide use of castor oil to initiate labor, the obstetric literature contains few references to this botanical laxative. Derived from the castor plant Ricinus communis, castor oil may possess properties that are useful in post-term pregnancies. OBJECTIVE: To evaluate the relationship between the use of castor oil and the onset of labor. DESIGN: Prospective evaluation. SETTING: A community hospital in Brooklyn, NY. PATIENTS: A total of 103 singleton pregnancies with intact membranes at 40 to 42 weeks referred for antepartum testing. Inclusion criteria included cervical examination, Bishop score of 4 or less, and no evidence of regular uterine contractions. INTERVENTION: Patients were alternately assigned to 1 of 2 study groups: a single oral dose of castor oil (60 mL) or no treatment. MAIN OUTCOME MEASURES: Castor oil was considered successful if labor began within 24 hours after dosing. Groups were compared for onset of labor in 24 hours, method of delivery, presence of meconium-stained amniotic fluid, Apgar score, and birth weight. RESULTS: Fifty-two women received castor oil and 48 were assigned no treatment. Following administration of castor oil, 30 of 52 women (57.7%) began active labor compared to 2 of 48 (4.2%) receiving no treatment. When castor oil was successful, 83.3% (25/30) of the women delivered vaginally. CONCLUSIONS: Women who receive castor oil have an increased likelihood of initiation of labor within 24 hours compared to women who receive no treatment. Castor oil use in pregnancy is underreported worldwide. This small series represents the first attempt to evaluate the medication.

[Hereditary progressive mucinous histiocytosis].

BACKGROUND: Hereditary progressive mucinous histiocytosis is an uncommon condition which to date has only been observed in women. The disease is a non-Langerhans histiocytosis with a stereotypic clinical presentation limited to the skin. The clinical, histological and genetic features of this disease differ considerably from other types of histiocytoses and overload diseases. We report the first case observed in France. CASE REPORT: A 49-year-old woman consulted for asymptomatic papules on the dorsal aspect of the hands known since childhood. The lesions had progressively spread to other regions of the body. The woman's mother and two sisters had the same condition but her two sons and her brother and nephews did not. Standard semi-thin slices and electron microscope histology demonstrated dense cellular infiltration of the reticular dermis with interstitial mucinous overload, voluminous histiocytes with characteristic toluidine blue positive granulations, and a cytoplasm rich in myelin and Zebra bodies respectively. The observed clinical, histological and genetic features were characteristic of progressive hereditary progressive mucinous histiocytosis. DISCUSSION: The pedigree in our case and in those reported in the literature suggest a dominant hereditary condition. The fact that only female cases have been reported to date would suggest X-linked transmission with early death of male fetuses. An autosomal dominant transmission limited to woman as a result of mitochondrial heredity or hormone-related phenomena cannot be ruled out. The progressive extension with age without a tendency for spontaneous regression and the electron microscopy aspect suggest a phospholipid deposit disorder similar to that observed in lysosomial diseases. However, primary macrophage proliferation in response to an unknown stimulus cannot be excluded.

Myomectomy and tuboplasty performed at the same time in cases of distal tubal obstruction with associated fibroids.

OBJECTIVE: To compare pregnancy rates and outcomes in patients who underwent tuboplasty alone versus those who had simultaneous myomectomy and tuboplasty for distal tubal occlusion. STUDY DESIGN: One hundred eighty-eight patients underwent tuboplasty alone. An additional 56 were treated by both myomectomy and tuboplasty, and 8 others underwent tuboplasty, leaving the myomas in situ. RESULTS: Of the 188 patients who underwent tuboplasty alone, 47.90% (90 patients) became pregnant; of these, 24 (26.7%) had a spontaneous miscarriage, 14 (15.6%) developed an ectopic pregnancy, and 52 (57.7%) achieved a viable birth. By comparison, of the 56 patients who had both tuboplasty and myomectomy, 53.6% (30 patients) conceived: of these, 11 (36.7%) miscarried, 7 (23.3%) developed ectopic pregnancies, and 12 (40.0%) achieved viable births. Of the eight patients who underwent tuboplasty while leaving the myoma in situ, five became pregnant, resulting in three viable births (37.5%) and two spontaneous abortions (25%). The number of pregnancies, ectopic gestations, miscarriages and viable births in the two former groups showed no statistical differences. (For the eight patients with the myoma in situ, the number was too small for statistical analysis.) CONCLUSION: Because there is no disadvantage to performing myomectomy at the time of tuboplasty for distal tubal occlusion, the combined surgical approach should be the procedure of choice rather than performing myomectomy in a separate operation later.

Sulfonylureas and sulfonylcarbamates as new non-tetrazole angiotensin II receptor antagonists. Discovery of a highly potent orally active (imidazolylbiphenylyl)sulfonylurea (HR 720).

The synthesis and pharmacological activity of new potent nonpeptide non-tetrazole angiotensin II (AII) receptor antagonists are described. These compounds are 4-thioimidazole derivatives linked on N1 to a biphenylsulfonyl fragment by a methylene spacer. Different acidic sulfonamides such as sulfonylureas 12, sulfonylcarbamates 15, sulfonylamides 16, and sulfonylsulfonamides 17 have been investigated as replacements to the known potent tetrazole moiety at the 2'-biphenyl position. Their activity were evaluated by AII receptor binding assay as well as by in vivo (i.v. and po) assays such as inhibition of the AII-induced pressor response in pithed rats. Most of the synthesized sulfonyl derivatives showed nanomolar affinity for the AT1 receptor subtype. The N-propylsulfonylurea 12d and the ethyl sulfonylcarbamate 15b as representative members of this series exhibited high oral activity in the pithed rat model with ID50 values of 0.38 and 0.4 mg/kg, respectively. Structure-activity relationships on the imidazole ring linked to the methylbiphenyl N-propylsulfonylurea fragment demonstrated similar features to those found in the corresponding tetrazole series. For both class of compounds, the linear butyl chain in position 2 and a carboxylic acid in position 5 were important for high in vitro and in vivo activity. In most cases, replacement of the carboxylic acid was detrimental to in vivo activity while maintaining the in vitro binding affinity. Introduction of a methylthio group in position 4 was found to enhance oral activity compared to compounds with chloro or other alkylthio, (polyfluoroalkyl)thio, and arylthio groups. 2-Butyl-4-(methylthio)-1-[[2'- [[[(propylamino)carbonyl]amino[sulfonyl](1,1'-biphenyl)-4- yl]methyl]-1-H-imidazole-5-carboxylic acid (12d) as the most promising example of the series was synthesized as its dipotassium salt (50, HR 720). This compound 50 inhibited the specific binding of [125I]-AII to rat liver membranes with an IC50 value of 0.48 nM. In vivo, 50 dose-dependently inhibited the AII-induced pressor response in normotensive pithed rats (ID50 = 0.11 mg/kg i.v. and 0.7 mg/kg po). In addition, this compound produced a marked and long-lasting decrease in blood pressure in high renin animal models and proved to be superior to the corresponding tetrazole 45 as well as to DuP 753 or its active metabolite EXP 3174. Compound 50 has been selected for in-depth investigations and is currently undergoing phase II clinical trials.

Augmentation of protein synthesis and degradation by poor dietary amino acid balance in European sea bass (Dicentrarchus labrax).

Sea bass fry were fed a fixed ration of one of six isonitrogenous diets differing in essential amino acid balance or physical and chemical state of the protein source (Hydrolysate vs. intact protein) to induce different growth rates. The reference diet was based on fish meal, whereas the other diets contained fish protein hydrolysate, greaves meal (i.e., defatted collagen meal) or hydrolyzed feather meal added at 30 or 50% of crude protein at the expense of fish meal protein. Digestibility as well as fractional rate of whole-body protein synthesis was assessed. Whole-body protein synthesis was determined for each group of fish using a single injection of flooding dose of tritiated phenylalanine. Protein digestibility of the diets varied only by 5.5%. Specific growth rate and fractional protein specific growth rate, i.e., fractional protein accretion, were higher in fish fed the reference diet than in those fed the diets in which 50% of fish meal protein had been replaced by greaves or hydrolyzed feather meal protein. Compared with the reference group, whole-body protein synthesis was higher in fish fed these latter diets as well as in those fed the diet containing 30% greaves meal protein. The fractional protein accretion to fractional protein synthesis ratio, i.e., the efficiency of protein deposition, was lower in fish fed poorer dietary amino acid balance than in the reference group. The substitution of fish protein hydrolysate for intact fish protein led to a similar, though less pronounced phenomenon: nonsignificant increase in protein synthesis accompanied by significant increase in protein degradation.(ABSTRACT TRUNCATED AT 250 WORDS)

Neutralization of bovine herpesvirus type 4 by pairs of monoclonal antibodies raised against two glycoproteins and identification of antigenic determinants involved in neutralization.

In infected cattle, bovine herpesvirus type 4 (BHV-4) induces an immune response with low neutralizing antibody levels or in the absence of such antibodies. For the study of this phenomenon, monoclonal antibodies (MAbs) raised against two BHV-4 glycoproteins identified previously (150K/120K/51K and 120K/16.5K) were used in neutralization tests. None of the MAbs except for MAb 16 could neutralize alone; pairs of MAbs against the 150K/120K/51K and 120K/16.5K glycoproteins were able to neutralize BHV-4 infectivity. MAbs involved in neutralization were used in competitive binding assays to identify epitopes relevant for BHV-4 neutralization. These MAbs showed a low avidity and a weak neutralizing activity, and they partially decreased BHV-4 attachment to cells. These results suggest that the BHV-4 glycoprotein domains involved in viral infectivity are poorly exposed to the immune system.

Serum estradiol as an aid in the diagnosis of ectopic pregnancy.

The value of serum beta-hCG measurement in the diagnosis of ectopic pregnancy is well established, and there have been recent studies on the use of serum progesterone levels. However, we have been unable to find any reports on the potential application of serum estradiol (E2) assays in the diagnosis of ectopic pregnancy. We therefore concurrently measured serum E2, progesterone, and beta-hCG in 100 women with ectopic pregnancies, as well as in 69 controls with normal intrauterine pregnancies and 36 women with threatened abortion. The mean (+/- standard deviation) E2 levels for ectopic-pregnancy patients, the normal controls, and the women with threatened abortion were 281.1 +/- 115.6, 788.2 +/- 45.5, and 788.8 +/- 40.6 pg/mL, respectively; the mean levels in the ectopic group were significantly different (P less than .0001) from those of the other two groups. All but one of the ectopic pregnancies had values below 650 pg/mL for E2 and 23 ng/mL for progesterone, and all but one of the normal intrauterine pregnancies had values above these levels. Our data suggest that the addition of the estradiol assay, with or without progesterone, to the early evaluation of patients suspected of having an ectopic pregnancy may be helpful in diagnosis.

Production and characterization of monoclonal antibodies to bovid herpesvirus-4.

Thirty-five hybridoma cell lines secreting monoclonal antibodies (Mabs) against bovid herpesvirus-4 (BHV-4) strain V. Test were produced. These hybrid cells resulted from the fusion of SP2/0 myeloma cells with splenocytes of BALB/c mice previously immunized with purified BHV-4. A modified indirect fluorescent antibody test (IFAT) was applied as a screening procedure and was compared with an indirect enzyme-linked immunosorbent assay. The selected Mabs were tested by the same IFAT against a panel of BHV-4 field isolates and against bovine herpesvirus-1, bovine herpesvirus-2 and alcelaphine herpesvirus-1 (AHV-1). Comparison of BHV-4 field isolates with Mabs confirmed their close antigenic relationships, but slight antigenic differences were observed between different isolates. One of the Mabs also reacted against AHV-1, indicating an antigenic relationship between BHV-4 and AHV-1. None of the Mabs reacting with BHV-4 possessed neutralizing activity against the strain used for immunization.

[Corrosion of a dental acrylic resin by alkaline peroxide used for cleaning].

Some clinical observations have brought the authors to suspect a corrosion of acrylic resins by an alcaline peroxide (Steradent) proposed as a denture cleanser. By way of a laboratory simulation they have shown that standardized samples of Lucitone 199 are corroded by the main active agents issuing from Steradent: oxygen and a high level of alcalinity. A daily use of Steradent can thus badly affect the physical properties of denture acrylic bases.

High-frequency transformation of Rhizobium meliloti.

Transformation of R factor RP4 and its derivative pRK290 from Escherichia coli to Rhizobium meliloti is reported. The efficiency of transformation was in the range of 10(-5) per viable cell. In addition, chromosomal DNA prepared from one R. meliloti strain resistant to streptomycin was transferred to the isoleucine-valine-requiring mutant susceptible to streptomycin.

Maternal serum levels of estradiol, progesterone and human chorionic gonadotropin in ectopic pregnancy and their correlation with endometrial histologic findings.

Plasma levels of estradiol 17-beta (E2), progesterone (P) and beta-human chorionic gonadotropin (b-HCG) were measured in 59 patients with ectopic pregnancy and in control patients made up of ten women with normal intrauterine pregnancies and five patients with threatened abortion. The gestational ages of the three groups were not statistically different, the means being 6.6, 6.5 and 6.7 weeks, respectively. The endometria in the patients with an ectopic pregnancy were examined histologically and the correlation with the hormonal levels was studied. Mean levels of b-HCG, E2 and P in patients with ectopic pregnancies (4,893 +/- 5,435 S.E.M. milli-international units per milliliter, 311 +/- 191 S.E.M. picograms per milliliter and 8.3 +/- 5.5 S.E.M. nanograms per milliliters, respectively) were significantly lower than those measured in normal pregnant control patients (b-HCG = 22,173 +/- 2,696 S.E.M. microunits per milliliter, p less than 0.00001; E2 = 769 +/- 81 S.E.M. picograms per milliliter, p less than 0.0001 and p = 37.8 +/- 6.1 S.E.M. nanograms per milliliter, p less than 0.0001), and in patients with threatened abortion (b-HCG = 20,310 +/- 1,688 S.E.M. milli-international units per milliliter, p less than 0.0001; E2 = 803 +/- 91 S.E.M. picograms per milliliter, p less than 0.001 and P = 29.7 +/- 2.9 S.E.M. nanograms per milliliter, p less than 0.001). Mean levels of P in ectopic pregnancies with secretory type endometria (10.4 +/- 6.0 S.E.M. nanograms per milliliter), were significantly higher than those with proliferative endometria (5.0 +/- 3.2 S.E.M. nanograms per milliliter, p less than 0.001). Data is provided not previously known, on the levels of E2 and P in ectopic pregnancy and correlation with endometrial histologic factors.

Characterization and primary structures of DNA-binding HU-type proteins from Rhizobiaceae.

The DNA-binding HU-type proteins from several species of Rhizobiaceae including Rhizobium meliloti, two strains of Rhizobium leguminosarum with highly different phenotypic characters and Agrobacterium tumefaciens, were characterized and their amino acid sequences were determined. HU-type proteins isolated from R. leguminosarum L18 and A. tumefaciens are identical and show slight differences with the R. meliloti HU-type protein. On the other hand the R. leguminosarum L53 HU-type protein is quite different from the proteins cited above; several amino acid substitutions encountered in this protein result in significant changes in the folding of the polypeptide chain. The biochemical characteristics of these proteins are in good agreement with the respective position of these bacteria in the phylogeny determined by numerical taxonomy.

Components of the total serum thyroid hormone concentrations during pregnancy: high free thyroxine and blunted thyrotropin (TSH) response to TSH-releasing hormone in the first trimester.

The distribution of thyroid hormones between free solution and their several protein-binding sites during pregnancy was studied under physiological conditions of temperature and pH. Single serum specimens were obtained from individual women at different stages of pregnancy. During the first 5 weeks of pregnancy, mean serum free T4 and free T3 concentrations were 50% higher than in nonpregnant women or women during the third trimester. Free T4 was increased significantly throughout the first trimester, but because of wide variance, free T3 was significantly above control values only during the first 5 weeks. Free T4 and free T3 concentrations decreased to control levels in the third trimester. These changes in free T4 concentrations are consistent with a weak thyrotropic action of hCG, which attained maximal concentrations early in the first trimester and then decreased markedly in the second and third trimesters. TRH testing of women scheduled for abortion in the first and second trimesters revealed marked inhibition of TSH response to TRH in those first trimester women who had elevated free T4 concentrations. The percent free T4 did not decrease during the first 5 weeks, but then declined progressively to term as T4-binding globulin (TBG) affinity, defined as the product of the capacity and affinity constant, progressively increased. T4 bound to TBG (T4-TBG) increased from early in the first trimester to term, and then decreased in postterm pregnancy and postpartum. T4 bound to prealbumin (T4-PA) and to albumin (T4-Alb) decreased significantly in the third trimester compared with either control or first trimester concentrations. The concentration of free T3 was positively correlated with T4-PA (r = 0.25) and T4-Alb (r = 0.31), but not with free T4 (r = 0.18) or T4-TBG (r = -0.30) concentrations. These results suggest that 1) only the high concentrations of hCG present in the first trimester of pregnancy have a thyrotropic effect in excess of normal levels of TSH, and this can be sufficient to suppress the TSH response to TRH; 2) hepatic TBG secretion continues to respond to the continuously rising estrogen levels throughout pregnancy; and 3) T4-PA and T4-Alb, but not free T4 or T4-TBG, are possible precursors for the extrathyroidal generation of T3.

Comparative study of the DNA-binding HU-type proteins from slow growing and fast growing strains of Rhizobiaceae.

The DNA-binding HU-type proteins have been isolated from two very different strains of Rhizobiaceae : Agrobacterium tumefaciens and Rhizobium japonicum. These proteins have been called HAt and HRj respectively. Their electrophoretic mobility on polyacrylamide gel, amino acid composition and crossed immunoreactivity have been compared to that of the homologous protein isolated from Rhizobium meliloti: the protein HRm . The proteins HAt and HRm show close similarities whereas the protein HRj differs markedly from the two others. The physico-chemical characteristics of the HU-type proteins from these Rhizobiaceae are in good agreement with the respective position of these bacteria in the taxonomy.

The effects of age, dietary fat and bile salts, and feeding rate on apparent and true metabolisable energy values in chickens.

1. Apparent (AME) and true (TME) metabolisable energy values of four diets, containing 50 or 150 g animal fat or maize oil/kg, were estimated in young and adult chickens. The diets were given with and without a supplement of 5 g bile salts/kg. Three feeding rates, ad libitum, and 1/6th or 1/3rd of ad libitum were compared. 2. In younger birds, endogenous energy losses were higher when measured in starved birds than when estimated by regression analysis. 3. In young birds, feeding rate had no effect on AME of diets containing 50 g lipids/kg but it significantly affected TME. These results were reversed in adults. AME and TME values of diets containing 150 g lipids/kg were affected by the feeding rate in both young and adult birds. 4. Bile salts increased metabolisable energy values of diets containing high concentrations of saturated fatty acids and their effect was more pronounced in young birds.

The effects of endogenous energy, type of diet, and addition of bile salts on true metabolizable energy values in young chicks.

A trial was carried out using 3-week-old chickens of a commercial breed to study the effects of either a fat-free diet or a diet containing 150 g/kg of animal fat on the endogenous energy losses measured with starved birds. The effects of the addition of different levels of bile salts to such diets and the accuracy of true metabolizable energy (TME) with respect to the other modes of expression of metabolizable energy were also examined. The excreted endogenous energy values were shown to vary not only according to the type of diet (P less than .01) but also in relation to the dietary intake level (P less than .01). Because it is directly related to endogenous energy, TME proved to be an inaccurate parameter in young chicks unless values were corrected for N-balance. If the values of both apparent metabolizable energy (AME) and TME are corrected for N-balance, they are normally comparable and independent of the dietary intake level if the diet contains virtually no added fat. These findings indicate that most of the endogenous excreta are composed of nitrogenous metabolites. However, neither AME nor TME values of fat-rich diets are independent of dietary intake. The addition of bile salts had no effect on the metabolizable energy values of the fat-free diet. However, in the case of the diet rich in saturated fats, they compensated either for insufficient bile secretion or for endogenous bile salts degraded by the intestinal microflora. Thus, the digestive utilization of dietary fat, especially that of the saturated fatty acids, palmitic and stearic acids, was increased. In addition, metabolizable energy was significantly improved (P less than .01) by the addition of bile salts when the dietary intake level increased to the ad libitum level.
