One hundred and seventy-two diabetic patients undergoing local operations of the foot distal to the ankle joint were reviewed to explore the preoperative prognostic indicators of operative failure. Severity of infection, as measured by preoperative temperature and white blood count, correlated with that of failure. Subcutaneous gas, as seen roentgenographically, was associated with failure in five of six patients. No particular operation or type of wound and no general or specific medical or laboratory parameter correlated with failure. In the four and one-half years of the study, an improved operative success rate could only be correlated with improved preoperative management of infection, as reflected in a declining yearly preoperative white blood count and temperature. Delay of the operation until maximum medical control of the infection has occurred should improve the initial success rate in these patients.

A "dumbbell" neuroblastoma is a malignant neoplasm usually primary in the mediastinum or retroperitoneum with posterior extension through an intervertebral foramen to additional tumor within the spinal canal. Neurologic deficits are found in almost all cases. Nineteen patients with such tumors were reviewed with respect to diagnosis, management, and results. Treatment consisted of prompt laminectomy with total or subtotal excision of the extradural tumor. The primary tumor in the mediastinum or retroperitoneal area was removed at a second stage. All patients received postoperative radiation therapy. Eleven of the 17 patients observed more than two years are free of disease, a cure rate much higher than the overall experience with neuroblastoma. Substantial recovery of neurologic function occurred in most, but kyphoscoliotic deformities were a disturbingly frequent late complication.
The effects of intramuscular 200-mg nortestosterone decanoate (ND) on red cell 2,3 diphosphoglycerate (DPG) concentration and packed cell volumes (PCV) were compared in 9 patients undergoing chronic hemodialysis for renal failure and in 18 control patients undergoing hemodialysis but not receiving ND. Subjects had received the drug 4.3 plus or minus 0.3 (SEM.) months at the time of DPG determinations. DPG was similar in treated patients and in the control subjects: 6.27 plus or minus 0.22 mM/L RBC and 6.16 plus or minus 0.23 mM/L RBC, respectively. The initial PCV was also similar: 21.72% plus or minus 1.36 and 20.13% plus or minus 0.91. Although the mean PCV during treatment was higher for 5 of 9 ND-treated patients (p smaller than 0.01), the final PCV prior to termination of therapy was greater than the initial pretreatment PCV in only 2 of these 5 patients (p greater than 0.05). We conclude that 200 mg ND weekly has no effect on the red cell DPG and a transient unsustained effect in raising the PCV in male chronic hemodialysis patients over a 5-month period of treatment.

The effect of 1-beta-D-arabinofuranosylcytosine (ara-C) and 5-azacytidine (5-aza-C), alone and in combination, on DNA synthesis and cytotoxicity in hamster fibrosarcoma cells has been studied. After a 2-hr exposure of S-phase cells to ara-C at concentrations of 2 to 200 muM, the cells required about 4 to 6 hr to recover from inhibition of DNA synthesis. When 2 exposures to ara-C were used, maximal cytotoxicity occurred when the 2nd dose of ara-C was administered at the time when the cells recovered from the inhibition of DNA synthesis. When the S-phase cells were exposed to ara-C, the maximal killing effect of 5-aza-C occurred when this agent was administered 6 hr later, at the time when the cells had recovered from the inhibition of DNA synthesis. When S-phase cells were exposed to 5-aza-C, the maximal cell kill produced by ara-C also occurred 5 to 6 hr later. When the S-phase cells were exposed simultaneously to both ara-C and 5-aza-C, significant antagonism with respect to cytotoxicity was observed between these 2 agents. When cells in G1 were exposed to 5-aza-C, the cytotoxicity produced by ara-C on these cells when they entered S phase was additive with respect to the cytotoxicity produced by 5-aza-C exposure alone.
A hapten-sandwich procedure has been developed for specific labeling of cell surface antigens for fluorescence or electron microscopy. Haptens are azo-coupled to immunoglobulins specific for a cell surface antigen; the hapten-modified cell-bound antibodies can then be visualized by adding fluorescent antihapten antibody, or by adding antihapten antibody followed by hapten-modified markers for electron microscopy. Virus or high molecular weight protein markers are lightly cross-linked before conjugation with hapten to prevent their disruption. Such stable hapten-modified markers, and the accessibility of many different purified anti-azophenyl-hapten antibodies, make it feasible to distinguish more than one membrane antigen in a given labeling experiment. When mouse lymphoid cell populations are labeled with separate markers for Ig and for thymus-associated antigens, many cells exhibit the Ig marker exclusively or the thymic marker predominantly, and some cells are completely free of label.

Mitochondrial preparations isolated from neonatal swine hepatocytes show a marked increase in oxidative and concomitant phosphorylative capacity between birth and 2 days postpartum. There are no changes in the coupling parameters (respiratory control ratio and adenosine diphosphate/O ratio) with age. Changes in sedimentation properties in a sucrose gradient suggest qualitative changes in the mitochondria. Some of the lipid measurements (increased phospholipid) might be interpreted as supportive of this suggestion, although most could also be regarded as indicative of quantitative changes (increased number of mitochondria). Electron microscopy of isolated mitochondria and of the hepatocyte demonstrated an increased number of mitochondria but no change in shape, size, or structure as the pig developed. An increase in a number of cytoplasmic components (Golgi apparatus and endoplasmic reticulum) and a decrease in glycogen were also observed. The functional changes in mitochondria seem to occur within a short period of time (6-12 hr postpartum).
