Postnatal development of the hepatobiliary transport of phenolsulfonphthalein in rats.

1. The postnatal development of the biliary excretion of phenolsulfonphthalein (PSP) was studied in male Wistar rats. 2. Following i.v. injection of PSP at 200 mumol/kg body wt, a maximal biliary excretion of 175 +/- 10 nmol/min/100 g body wt and 32 +/- 5 nmol/min/100 g body wt was reached for unconjugated and conjugated PSP, respectively, in the adult group. 3. The maximal biliary excretion of conjugated PSP was significantly lower in the 20-, 30- and 40-day-old groups as compared to the adults. The excretion of unconjugated dye was also significantly lower in 20- and 30-day-old rats. 4. The postnatal development of PSP excretion was unrelated to changes in the activity of UDP-glucuronosyltransferase. The importance of other factors is also discussed.

Pelvic lymphadenectomy and transperineal interstitial implantation of IR 192 combined with external beam radiotherapy for bulky stage C prostatic carcinoma.

From January 1983 until June 1987, 51 patients with locally advanced prostatic carcinoma (47 Stage C, 4 bulky B2) were treated at Mayo Clinic (33 patients) and at William Beaumont Hospital (18 patients) with (a) 5 Gy delivered pre-operatively in one fraction, (b) pelvic lymphadenectomy and (c) interstitial implantation of the prostate with Ir 192 seeds via a perineal template (the Martinez Universal Perineal Interstitial Template) to deliver 35 Gy, and (d) 30.6 Gy external beam therapy in 17 fractions to prostate only fields. Initial clinical response has been excellent. Local control, with a median follow-up of 45 months, has been 100% by clinical exam and 84.5% pathologically in the subset biopsied. Disease-free actuarial survival at 5 years is 89%. Major toxicity has been limited to the rectum, but a modification of the brachytherapy technique has reduced this sharply. We conclude that bulky Stage C prostatic carcinoma can be successfully treated by this aggressive combination of modalities with acceptable toxicity.

Biliary excretion and choleretic effect of cefmetazole in rats.

The effect of cefmetazole, a broad-spectrum cephalosporin, on bile flow and composition in rats was studied. Intravenous injection of cefmetazole at doses ranging from 40 to 400 mumol/kg of body weight led to an increase in its biliary concentration and excretion rate, with a maximum at 30 min after injection. Excretion of cefmetazole into bile was associated with a marked choleresis. The magnitude of the increase in bile flow was dose dependent, with a maximal increase at a dose of 200 mumol/kg. Cefmetazole administration did not affect the secretion of bile acids or their osmotic activities, whereas the bile acid-independent bile flow increased by 49% at a dose of 200 mumol/kg. Cefmetazole administration at a dose of 200 mumol/kg significantly increased the biliary outputs of sodium, potassium, chloride, and bicarbonate (+36, +56, +28, and +31%, respectively) compared with outputs of controls. A linear relationship was observed between bile flow and cefmetazole excretion, 44 microliters of bile being produced per mumol of cefmetazole excreted into bile. Our results demonstrate that cefmetazole induces choleresis by stimulating bile acid-independent bile flow. This effect appears to be partly due to the osmotic properties of cefmetazole transported into bile.

Infection of hemodialysis catheters: incidence and mechanisms.

Fifty-three consecutive subclavian or jugular hemodialysis catheters inserted into 41 patients were prospectively studied over a period of 8 months in order to determine the incidence of infection and its mechanisms. The intravascular, intradermal and the Y catheter segments as well as both connections were cultured using a quantitative technique for the intraluminal surface. In addition, the intravascular and intradermal portions of the catheter were cultured using a semiquantitative technique for the external surface. Skin smears of the catheter entry site were also cultured, and blood cultures were similarly obtained if fewer developed. Twenty-nine of the 53 catheters (55%) were significantly colonized by one (19 cases) or more (10 cases) microorganisms. The source of the colonizing microorganisms was the skin in 17 cases (58%), intraluminal in 5 (17%), both routes in 5 (17%) and others in 2 (6.8%). Staphylococcus epidermidis (22 cases) and Staphylococcus aureus (4 cases) were the bacteria most frequently isolated. Nine of the 53 catheterizations (17%) were complicated by catheter-related septicemia due to S. aureus in 4 cases, S. epidermidis in 3 cases, Streptococcus faecalis in 1 and Proteus vulgaris in 1. Catheter-related bacteremia contributed to a patient's death in 1 case. Suppurative local infections of the catheter entry site developed in 3 cases, 2 of them with septicemia. We conclude that the rate of infection due to subclavian or jugular hemodialysis catheters is very high and that the skin is the most frequent origin of the microorganisms.

Diagnostic value of quantitative cultures of bronchoalveolar lavage and telescoping plugged catheters in mechanically ventilated patients with bacterial pneumonia.

We compared the diagnostic value of quantitative cultures of bronchoalveolar lavage (BAL) and telescoping plugged catheter (TPC) samples in 34 nonimmunocompromised, mechanically ventilated (MV) patients with suspected bacterial pneumonia. A control group of seven "noninfected" MV patients was also studied. In 92% of patients with bacterial pneumonia (32 of 34), simple endotracheal aspiration samples recovered one or more microorganisms. Both BAL and TPC samples cultured colony-forming units (cfu) greater than or equal to 10(3)/ml of one or more microorganisms in 56% (19 of 34) of patients. TPC and BAL culture results agreed on 88.5% (54 of 61) of the recovered microorganisms. Sterile TPC and BAL cultures agreed on 80% (4 of 5) of the cases. Microorganisms cultured from blood samples were also cultured from BAL and TPC specimens. Culture results from the two techniques completely disagreed in only one case (3%). In the control group, one TPC and two BAL cultures yielded microorganisms in cfu greater than or equal to 10(3)/ml. Specificities of BAL and TPC were 71 and 86%, respectively, whereas specificity of endotracheal aspiration was only 14%. Both the bacterial index obtained by TPC and BAL, as well as the quantitative cultures, correlated moderately well (r = 0.78 and 0.72, respectively, p less than 0.001 for both correlations). BAL and TPC results caused changes of antibiotic treatment in 11 of 23 survivors. Neither BAL nor TPC caused complications. Our results demonstrate that BAL and TPC diagnose bacterial pneumonia in MV patients with similar accuracy. Culture results from both techniques showed excellent qualitative and reasonable quantitative agreement.

Sex-related differences in the hepatobiliary transport of phenolsulfonphthalein in the rat.

Sex-related differences in the hepatobiliary transport of phenolsulfonphthalein (PSP) were investigated in male and female Wistar rats. Maximal biliary excretion of unconjugated PSP was significantly higher in females while the excretion of the conjugated dye and liver UDP-glucuronosyltransferase activity toward PSP were higher in male animals. Orchidectomy decreased enzyme activity and excretion of the conjugate, whereas ovariectomy produced the opposite effect. Both in gonadectomized males and females maximal biliary excretion of the unconjugated dye was significantly reduced. Testosterone treatment increased the excretion of both conjugated and unconjugated PSP and transferase activity in orchidectomized males. Combined treatment of gonadectomized females with estradiol plus progesterone led to excretions of both conjugated and unconjugated PSP and UDP-glucoronosyltransferase activities similar to those found in control rats. These data indicate the existence of sex-related differences in the conjugation and biliary excretion of PSP in the rat and its modulation by sex hormones.

Spontaneous atherosclerotic lesions and prostacyclin formation in rabbits: effects of combined dipyridamole and aspirin.

The influence of dipyridamole and lysine acetylsalicylate on the incidence of atherosclerotic lesions and on arterial prostacyclin formation was studied in rabbits. Male New Zealand rabbits received i.m. for 16 months dipyridamole (12.5 mg/kg/day) and lysine acetylsalicylate (0.5 mg/kg/day). The incidence of spontaneous atherosclerotic lesions in the aorta was reduced by 16.4% with respect to untreated animals. Administration of the drugs significantly increased prostacyclin formation with respect to the untreated rabbits both in animals developing (1124 +/- 197 pg/mg/3 min vs 316 +/- 49 pg/mg/3 min) or not developing lesions (499 +/- 40 pg/mg/3 min vs 246 +/- 19 pg/mg/3 min). The observed increase in prostacyclin formation could account for the lowered incidence of atherosclerotic lesions in rabbits receiving the combination of dipyridamole and lysine acetylsalicylate.

Impairment of sulfobromophthalein biliary excretion and inhibition of glutathione S-transferase activity induced by perhexiline maleate in rats.

The effect of the antianginal agent perhexiline maleate (160 mg/kg i.g., daily for 4 days) on the biliary excretion of sulfobromophthalein (BSP) and BSP-glutathione and the hepatic activity of glutathione S-transferases was investigated in Wistar rats. Perhexiline maleate caused a significant reduction in the maximal biliary excretion of BSP (-28%). The decrease corresponded to a lowered excretion of the conjugated dye whereas the excretion of the parent compound did not change significantly. Administration of the drug caused no effect on the maximal biliary excretion of infused BSP-glutathione. Liver glutathione concentrations were similar in control and treated rats. Perhexiline maleate significantly reduced liver glutathione S-transferase activities toward BSP (-25%), 3,4-dichloronitrobenzene (DCNB) (-21%) and 1-chloro-3,4-dinitrobenzene (DNCB) (-27%). Kinetic studies of the enzyme in liver cytosol showed that perhexiline maleate induced an uncompetitive inhibition for the BSP substrate with a reduced Vmax and Km. The results indicate that the reduction in glutathione S-transferase activity plays an important role as a factor determining the impairment in the hepatobiliary transport of BSP caused by perhexiline maleate.

The effect of methimazole on bile flow and bile acid secretion in the rat.

The effect of methimazole on bile flow and composition was studied in male Wistar rats. Administration of methimazole as a single bolus (175-700 mumol/kg body wt) or infusion over three hours (1 mumol/100 g body wt/min) significantly increased bile flow and the biliary output of bile acids and inorganic electrolytes. Choleresis was a transient phenomenon and at the end of the experiments bile flow had returned to control values, although bile acid output was significantly lowered. When the amounts of bile acids arriving into the liver were increased by taurocholate infusion or decreased by cholestyramine pretreatment, a transitory enhancement was also found in bile flow and bile acid output, but the amounts of bile acid secreted and the choleretic effect induced were different according to the bile acid secretory rate found before methimazole administration. The bile acid content in the liver of taurocholate-infused rats was reduced during methimazole-induced choleresis. Our data indicate that the higher bile flow in methimazole-treated rats is related to an enhanced biliary secretion of bile acids probably due to a transitory stimulating effect on the transport into bile of the intrahepatocytary bile acid pool. The choleretic effect is not apparently shared by other thiocarbamide compounds.

Neonatal and maternal platelets: activation at time of birth.

Determination of the plasma concentrations of beta-thromboglobulin (BTG), thromboxane B2 (TxB2) and platelet factor 4 (PF4) were made at the time of birth in 18 newborns and their respective mothers. Both groups show significant elevation of all these molecular markers, suggesting marked platelet activation. The elevated TxB2 levels in the newborn group, 25 +/- 8 pg/ml, are compatible with a normally functioning and activated platelet prostaglandin pathway. Mode of delivery, vaginal or caesarean section, does not significantly influence the degree of activation in either group. Ultrastructural platelet examination did not reveal any morphologic differences between maternal and newborn platelets. There appears to be marked activation of the newborn and maternal platelet systems at the time of birth, and we postulate that this may explain in part the transient platelet dysfunction observed in newborns.

The quality and extension of nerve fibre regeneration in the centrocentral anastomosis of the peripheral nerve.

Centro central anastomosis (CCA) is the connection created by means of a nerve autograft placed between the fascicles of the proximal stump of a sectioned nerve. We have used this technique in the laboratory and in the human to avoid the formation of a terminal neuroma after amputation of a limb. The aim of the present study is to know what is the quality and how long is the progression of the regenerating nerve fibers coming from a fascicle and growing inside the nerve graft and into the other central fascicle. 56 male, adult, Sprague-Dawley rats were operated on. The 4 rats of Group I were used for control: the right sciatic nerve was exposed and dissected in its four branches. One piece of 10 mm was taken of each of the sural and the peroneal branches for hystological and histomorphometric studies. In the 16 rats of Group II, after the section of the sural and peroneal branches a terminoterminal suture between their proximal stumps was made. In the 16 rats of Group III a CCA with an interposed nerve graft of 7 mm was achieved. 5.5 and 6 rats of Groups II and III were sacrificed at 10, 40 and 365 days after the operation and the entire graft was taken for histological analysis. In the 20 rats of Group IV, 60 days after CCA, the graft was sectioned at its center to produce wallerian degeneration of nerve fibers. 10 days after this second operation the animals were sacrificed and the nerve specimens were taken for histological study.(ABSTRACT TRUNCATED AT 250 WORDS)

Olive (Olea europea) pollen allergens--I. Immunochemical characterization by immunoblotting, CRIE and immunodetection by a monoclonal antibody.

The pattern of reactivity of the Olea europea crude extract antigens was analysed after electroblotting to nitrocellulose from SDS-PAGE. The antigens contained in the 17, 19 and 42 K bands were most reactive with specific IgE from individual sera. Following immunization with a crude extract, one monoclonal antibody (OL-1) was raised against components which exhibited IgE binding capacity in electroblotting and crossed radioimmunoelectrophoresis (CRIE). Monoclonal antibody OL-1 reacted with the 17 and 19 K antigens and with three arcs of crossed immunoelectrophoresis (CIE), one of which is considered to contain a major allergen by CRIE.

Olive (Olea europea) pollen allergens--II. Isolation and characterization of two major antigens.

A dialyzed extract of olive (Olea europea) pollen was fractionated by anion exchange chromatography on DEAE-Sepharose CL-6B using a discontinuous gradient of ammonium bicarbonate. The most important protein allergen was obtained from the 0.3 M fraction after gel filtration on Sephadex G-100 and separation by lentil-lectin Sepharose-4B. The major allergen of olive pollen was contained in the effluent and was designated Olea Antigen I. This material inhibited the RAST activity of 15 patients' sera that were tested. Analytical IEF demonstrated a major band at pH 5.3 and two minor ones at pH 5.6 and 5.0. When these were run into SDS-polyacrylamide gel electrophoresis in a second dimension, all were separated into two bands of mol. wt 17 and 19 K. A second protein, which is the next most important allergen, Olea Antigen II, was obtained from the 0.5 M fraction by chromatofocusing in a 4-7 pH range followed by filtration on Bio-gel P-30. Olea Antigen II had a mol. wt of 8 K as assessed by SDS-PAGE. IEF analysis displayed one main band at pH 3.6 and two minor bands at pH 3.8 and 4.0, respectively. OL-1, an anti-Olea europea monoclonal antibody (MAb) previously reported by us Lauzurica et al. (1988) reacted with the 17 and 19 K antigens from the crude extract and with Olea Antigen I but not with Olea Antigen II.

Changes in hepatic and serum gamma-glutamyltransferase activities following chronic ethanol intake in rats.

The effect of chronic ethanol consumption on serum and liver gamma-glutamyltransferase (GGT) activities was studied in male Wistar rats. Animals were fed for 1-9 weeks a liquid diet containing 36% of total energy as ethanol or isocaloric carbohydrates. Compared to control diet, chronic ethanol significantly and progressively increased serum activity from 3 weeks of treatment. Liver GGT activity was also enhanced although changes were not parallel to those found in serum. Chronic ethanol intake led to an enhancement of liver glutathione concentration with a 45% increase at 3 weeks of treatment and a decrease thereafter. It is suggested that the increased hepatic GGT activity is not the only determinant of the enhanced serum levels of the enzyme and that it is not related to the modifications of liver glutathione content induced by ethanol consumption.

The role of pars flaccida in human middle ear sound transmission.

The role of the pars flaccida in middle ear sound transmission was studied with the use of twelve otoscopically normal, fresh, human temporal bones. Peak-to-peak umbo displacement in response to a constant sound pressure level at the tympanic membrane was measured with a noncontacting video measuring system capable of repeatable measurements down to 0.2 micron. Measurements were made before and after pars flaccida modifications at 18 frequencies between 100 and 4000 Hz. Four pars flaccida modifications were studied: (1) acoustic insulation of the pars flaccida to the ear canal with a silicone rubber baffle, (2) stiffening the pars flaccida with cyanoacrylate cement, (3) decreasing the tension of the pars flaccida with a nonperforating incision, and (4) perforation of the pars flaccida. All of the modifications (except the perforation) had a minimal effect on umbo displacement; this seems to imply that the pars flaccida has a minor acoustic role in human beings.

Phenolsulphonphthalein conjugation and biliary excretion in the rat: influence of phenobarbital and 3-methylcholanthrene.

The effect of phenobarbital and 3-methylcholanthrene pretreatment on the biliary excretion of phenolsulphonphthalein (PSP) was investigated in male Wistar rats. The dye was injected at a single dose of 200 mumol/kg body wt. About 20% of the compound was excreted as a glucuronide in the controls, the liver UDP-glucuronyltransferase activity toward PSP being 0.064 +/- 0.005 nmol.min-1.mg protein-1. Treatment for two weeks with phenobarbital (354 mumol.kg body wt-1.day-1) caused a transient increase in conjugated and unconjugated PSP excretion, but glucuronyltransferase activity was not modified. 3-Methylcholanthrene pretreatment for 4 days (75 mumol.kg body wt-1.day-1) also enhanced biliary excretion of the dye, but the increase corresponded only to the glucuronide and glucuronyltransferase activity was significantly enhanced by 20%. Our data indicate that not only the rate of biotransformation but also other factors could be responsible for increased PSP biliary excretion following administration of microsomal enzyme inducers.
