Gastric emptying of combined liquid-solid meals in healed duodenal ulcer.

The gastric emptying rates of combined liquid and solid radioisotopically labeled meals in 47 healed duodenal ulcer subjects and 17 healthy control subjects are compared. No significant differences were found between the groups in emptying slopes and the emptying half-times or in the percent retention values at any of the counting intervals for either the liquid or solid meals. These results are compatible with the observation that the rapid gastric emptying in many patients with duodenal ulcer is associated with the disease and that healing results in a return to normal gastric emptying rates. However, since gastric emptying rates during active ulceration were not determined in our patients, a more definitive interpretation awaits a study comparing emptying rates obtained during and after healing of active ulceration in the same patient.

5-Azacytidine acts directly on both erythroid precursors and progenitors to increase production of fetal hemoglobin.

The effect of 5-azacytidine on erythroid precursors and progenitors was studied in nine patients with sickle cell anemia or severe thalassemia. Each patient received the drug intravenously for 5 or 7 d. 5-Azacytidine caused a four- to sixfold increase in gamma-messenger RNA concentration in bone marrow cells of eight of the nine patients and decreased the methylation frequency of a specific cytosine residue in the gamma-globin gene promoter in all nine patients. Within 2 d of the start of drug treatment there was a rise in the percentage of reticulocytes containing fetal hemoglobin (HbF; F-reticulocytes) without a significant change in the total number of reticulocytes, which suggested that there was a direct action of 5-azacytidine on erythroid precursors. Late erythroid progenitors (CFU-E), present in bone marrow after 2 d of drug administration, formed colonies containing an increased amount of HbF as compared with control colonies. Moreover, the number of CFU-E derived colonies was not decreased at these early times, which suggested that there was a direct action of 5-azacytidine on erythroid progenitors in the absence of cytotoxicity. Exposure of normal bone marrow cells in tissue culture to 5-azacytidine for 24 h reproduced both of these effects as judged during subsequent colony formation. The combined direct effects of 5-azacytidine on both the erythroid precursor and progenitor compartments resulted in an increase in HbF synthesis that was sustained for 2-3 wk. Toxicity to bone marrow as reflected by cytoreduction was evident after treatment in some patients but was not accompanied by an increase in HbF production. A correlation was found between the effects of 5-azacytidine on bone marrow, as assessed by in vitro measurements, and the hematological response of the individual patients to drug treatment.

Lymphokine abnormalities in aplastic anemia: implications for the mechanism of action of antithymocyte globulin.

Anti-thymocyte globulin (ATG) provides effective therapy for many patients with aplastic anemia, and its mechanism of action has been presumed to be secondary to lymphocytotoxicity. However, our studies of lymphocyte function in aplastic anemia show marked abnormalities of lymphokine production, which ATG may modulate. In 12 of 17 patients with aplastic anemia, interleukin 2 (IL2) production was markedly elevated in vitro (P less than .01 by paired statistical analysis). Expression of the IL2 receptor, or Tac antigen, on peripheral lymphocytes assessed by flow microfluorometry was also increased above the normal range in 11 of 15 cases. Studies of ATG suggested that it might act to stimulate lymphocyte function. In vitro, ATG is a mitogen, as measured by incorporation of 3H-thymidine into blood mononuclear cells; the response of cells to ATG from patients with aplastic anemia was exaggerated in comparison with normals. Cell proliferation was accompanied by production of IL2 to levels that were, in some cases, similar to those obtained with lectin stimulation. Finally, supernatants from lymphocytes cultured in the presence of ATG were able to replace adherent cells in providing growth factors for the support of nonadherent cells in methylcellulose hematopoietic colony assays. These results provide a mechanism for an "immunostimulatory" action of ATG in effecting hematopoietic response in some patients with aplastic anemia.

Moderate weight loss and sulfonylurea treatment of non-insulin-dependent diabetes mellitus. Combined effects.

Hypocaloric liquid formula diets were given for one month to 20 moderately obese patients with non-insulin-dependent diabetes mellitus divided into two equal groups; group 1 was treated with weight loss alone; group 2 received glipizide in addition to the hypocaloric diet. Mean weight loss was similar in the two groups (6.5 +/- 0.6 v 6.4 +/- 0.5 kg) and was associated with a statistically significant fall in mean fasting plasma glucose values from 293 +/- 15 to 232 +/- 24 mg/dL (group 1) and from 281 +/- 15 to 152 +/- 7 mg/dL (group 2). This was associated with 13% (group 1) and 36% (group 2) decrements in total postprandial glucose response. Neither fasting nor postprandial insulin levels changed significantly with weight loss, but estimates of insulin-stimulated glucose disposal demonstrated a 15% (group 1) and 42% (group 2) improvement. Finally, fasting plasma cholesterol and triglyceride concentrations fell significantly in both groups.

Gastric emptying measurements: delayed and complex emptying patterns without appropriate correction.

Anteriorly acquired and geometric mean corrected gastric emptying curves of solids and liquid isotopic-labeled meals were compared in 37 subjects given 61 meals of three different sizes. Anterior data alone consistently and significantly underestimated solid-phase gastric emptying rates with all meal sizes when compared to geometric mean acquired data. However, with liquids there were only slight differences between anterior and anterior and posterior geometric mean corrected emptying-rates. The difference probably reflects greater attenuation of the 140 kev photon of 99mTc compared to the 247 keV photon of 111In. With anterior data alone, an apparent early delay in emptying of solids was present with all meal sizes and the resultant emptying curves were nonlinear in shape. Geometric mean correction resulted in the linearization of the solid-phase emptying curves and essentially eliminated the apparent delay in emptying or lag phase noted with the anterior data alone. Based on our results, geometric mean correction techniques are necessary for accurate assessment of radioisotopic-labeled solid meals.

Influence of meal weight and caloric content on gastric emptying of meals in man.

This study was designed to assess the relative influence of meal weight and caloric content on gastric emptying of liquid and solid meals in man. A dual radioisotopic method which permits noninvasive and simultaneous measurement of liquid- and solid-phase emptying by external gamma camera techniques was employed. Nine healthy volunteer subjects ingested 50-, 300-, and 900-g lettuce and water meals adjusted to either 68, 208, or 633 kcal with added salad oil. The following observations were made: (1) absolute emptying rates (grams of solid food emptied from the stomach per minute) increased directly and significantly with meal weight; (2) increasing meal total caloric content significantly slowed solid food gastric emptying but did not overcome the enhancing effect of meal weight; and (3) liquid emptying rates were uninfluenced by meal total kcal amount.

Fecal odorgrams. A method for partial reconstruction of ancient and modern diets.

Modern and ancient human feces (coprolites) and modern nonhuman animal fecal samples were subjected to headspace gas-chromatographic analysis for detection of odors related to dietary intake. The odors of grass, green leaves, licorice, meat, and corn were detected in coprolites dating to 6400 years BP (before present). Analysis of modern human samples revealed a large number of odors reflecting dietary constituents. The complexity and number of fecal dietary odors recovered was directly related to the number of food items ingested. Animal fecal odorgrams were characterized by a relatively large number of pleasant odors. Fecal odorgrams represent a new method by which modern and ancient diets may be partially reconstructed.

Concentrations and isotope ratios of carbon, nitrogen and sulfur in ocean-floor basalts.

Fresh submarine basalt glasses from Galapagos Ridge, FAMOUS area, Cayman Trough and Kilauea east rift contain 22 to 160 ppm carbon and 0.3 to 2.8 ppm nitrogen, respectively, as the sums of dissolved species and vesicle-filling gases (CO2 and N2). The large range of variation in carbon content is due to combined effect of depth-dependency of the solubility of carbon in basalt melt and varying extents of vapour loss during magma emplacement as well as in sample crushing. The isotopic ratios of indigenous carbon and nitrogen are in very narrow ranges, -6.2 +/- 0.2% relative to PDB and +0.2 +/- 0.6% relative to atmospheric nitrogen, respectively. In basalt samples from Juan de Fuca Ridge, however, isotopically light carbon (delta 13 C = around -24%) predominates over the indigenous carbon; no indigenous heavy carbon was found. Except for Galapagos Ridge samples, these ocean-floor basalts contain 670 to 1100 ppm sulfur, averaging 810 ppm in the form of both sulfide and sulfate, whereas basalts from Galapagos Ridge are higher in both sulfur (1490 and 1570 ppm) and iron (11.08% total iron as FeO). the delta 34S values average +0.3 +/- 0.5% with average fractionation factor between sulfate and sulfide of +7.4 +/- 1.6%. The sulfate/sulfide ratios tend to increase with increasing water content of basalt, probably because the oxygen fugacity increases with increasing water content in basalt melt.

Dawn diarrhea.

A case of detergent-induced hemorrhagic colitis is presented. The endoscopic findings which were suggestive of a vasculitis, are depicted and discussed.

Characterization of a virus that causes transient aplastic crisis.

Transient aplastic crisis in children with congenital hemolytic anemias has been linked epidemiologically to infection with a serum parvovirus-like virus (SPLV). The virus is found in the blood in the early stages of the crisis, and serum containing SPLV inhibits erythroid colony formation in vitro. After sedimentation of virus-containing sera through a sucrose density gradient, colony inhibitory activity is present in the particulate fraction and separate from serum immunoglobulins. No inhibitory activity can be recovered from convalescent-phase sera after similar fractionation procedures. Inhibition of erythroid colony formation in vitro is not a feature of sera from other viral infections. The pattern of resistance of SPLV activity to chemicals and enzymes is compatible with it being a parvovirus. By using replating techniques, a target of SPLV has been identified as a late erythroid progenitor cell. Neither SPLV antigen nor anti-SPLV IgM was present in the sera of patients with other forms of bone marrow failure.

Comparison of Tc-99m labeled liver and liver paté as markers for solid-phase gastric emptying.

A radionuclide marker for studies of solid-phase gastric emptying should have a high labeling efficiency and remain relatively stable during gastric emptying. The availability of materials and the ease of preparation are also considerations in selecting radionuclide markers. We have compared the stability of intracellularly labeled chicken liver, surface-labeled chicken liver, and labeled puréed meat (liver paté) incubated with hydrochloric acid solution or gastric juice. Intracellularly labeled chicken liver and labeled liver paté were also compared in gastric emptying studies in humans. Our in vitro results demonstrated labeling efficiencies greater than 92% for both intracellularly labeled liver and labeled liver paté. The paté labeled with Tc-99m sulfur colloid was more stable than Tc-99m surface-labeled liver in vitro and its preparation was easier than with the intracellular labeling technique. Gastric emptying studies on normal subjects demonstrated equal performance of the intracellularly labeled liver and the labeled liver paté. Labeled liver paté is thus an alternative to intracellularly labeled chicken liver in measuring solid-phase gastric emptying.

Effect of age on gastric emptying of liquid--solid meals in man.

A dual radioisotopic method was employed to study the rate of gastric emptying of meals in ten males with an average age of 31 years and 10 elderly males with an average age of 76.4 years. All study subjects were fed a standardized 900-g meal labeled with a liquid (111indium-DTPA) and solid (99mtechnetium-tagged liver) phase isotopic marker. There were no significant differences in solid food emptying rates between the young and aged men. A delay in liquid emptying, however, was observed in the aged men. The clinical significance of this observation is unknown.

Superficially invasive vulvar cancer with nodal metastases.

Seven patients with superficially invasive vulvar cancer and lymph node metastases are reported. Histology of the primary tumor does not seem to correlate reliably with its metastatic potential in any individual case, though "spray," pseudoglandular, and small cell carcinomas may be particularly aggressive lesions. While wide local excision seems to be adequate treatment for the primary lesion, it is recommended that for lesions with greater than 1 mm of stromal invasion, bilateral inguinal-femoral lymphadenectomy should be performed for midline lesions, and at least ipsilateral inguinal-femoral lymphadenectomy should be performed for lateralized lesions.

Management of regional lymph nodes and their prognostic influence in vulvar cancer.

One hundred thirteen patients with invasive carcinoma of the vulva underwent radical vulvectomy and bilateral inguinal-femoral lymphadenectomy between 1957 and 1978. Eighteen had unilateral pelvic lymphadenectomy. Thirty-one patients (27.4%) had positive lymph nodes. The corrected actuarial five-year survival for patients with negative nodes was 96%, whereas it was 94% for patients with one positive node, 80% for those with two positive nodes, and 12% for those with three or more positive nodes. All patients with positive pelvic nodes or pelvic recurrence had three or more positive unilateral groin nodes, and all had palpably suspicious groin nodes preoperatively. Groin and systemic recurrences occurred in 2.9 and 3.8%, respectively, of patients with fewer than three positive unilateral inguinal-femoral nodes, as compared to 33 and 66%, respectively, of patients with three or more positive nodes. These data do not support routine pelvic lymphadenectomy in patients who have no clinically suspicious groin nodes and fewer than three positive nodes on histologic examination.
