Glomus tumour of the fingers.

Clinical and therapeutic aspects of glomus tumour of the fingers have been considered by analyzing the records of 15 patients with glomus tumours. The tumour was found in the pulp in 5 patients and in the subungual region in 10. The diagnosis may be difficult to make and patients are often believed to be malingering. Complete surgical excision under magnification is curative.

Multicentre combined chemotherapy protocol for large cell advanced non Hodgkin's lymphoma.

Between October 1985 and October 1989, 75 previously untreated patients with stage III and IV non Hodgkin's lymphoma, large cell type, were treated with an alternating weekly chemotherapy regimen including the following drugs: week 1: Doxorubicin, vincristine, cyclophosphamide, bleomycin, and intrathecal (i.th.) methotrexate and cytarabine; week 2: Methotrexate with leucovorin rescue; week 3: Doxorubicin, ifosfamide with mesna, etoposide, and i.th. methotrexate and cytarabine; week 4: Methotrexate with leucovorin rescue. Complete responders after three cycles according to this schedule (12 weeks) were given 18 gys cranial irradiation and randomized between one additional cycle or three monthly CHOP (consolidation treatment). Among 66 evaluable patients, 53 achieved a complete remission (CR 80 per cent) and seven a partial remission (11 per cent). There were six failures, and nine early deaths during the initial phase, mostly due to septic problems. Forty-one of the 53 CR patients (77.3 per cent) have remained free of disease with a median follow-up of 15 months (1-49). Eight of the 12 relapses occurred during the first year, the four others at 13, 14, 16 and 38 months respectively. The 2-year survival was 63 per cent for the whole group, and 77 per cent for the CR group. No difference has been observed up until now between the two groups with different consolidation treatment. Therefore, this protocol seems to be able to produce a high rate of complete and durable remission. The analysis of prognostic factors suggests that some high-risk patients should be considered for intensification therapy with the support of autologous bone marrow transplantation.

The effect of 131I for diagnostic purposes on serum thyroglobulin (hTg) levels in subjects with thyroidal disorders.

The concentration of serum thyroglobulin was measured in sera of subjects with various thyroid disorders, before and after the administration of tracer doses of 131I. The mean serum human thyroglobulin (hTg) concentration before administration of the isotope was 35.2 ng/ml +/- 7.8 (SE) in 13 subjects and 36.3 ng/ml +/- 7.9 (SE) 24 h after the administration of 131I. The data indicate that no significant release of thyroglobulin occurs 24 h after the administration of tracer doses of 131I. In 2 of the 3 subjects, however, in whom samples were obtained at 4 and 8 h after diagnostic administration of 131I, a modest rise in serum Tg levels was observed. Determinations of serum thyroglobulin levels within 24 h after the administration of a tracer dose of 131I are nevertheless valid providing one allows sufficient time to elapse for tracer decay.

Rapid isolation of DNA from Actinomyces.

DNA could not be quickly extracted from members of the genus Actinomyces by the usual methods of lysis. Treatment of 7 different actinomyces cells with lysozyme and achromopeptidase, both 5 mg/g wet cells, for 2 h, followed by SDS (0.2%), proteinase K (5 mg/g wet cells) and EDTA (lmM) for 1 h, lysed the cells. The yield obtained in one day was 337 micrograms per 200 mg of bacterial cells. The treatment was also found to work effectively on strains belonging to Veillonella, Staphylococcus, Fusobacterium and Bifidobacterium genera.

The design and synthesis of purine inhibitors of CDK2. III.

Cyclin-dependent kinases (CDKs) belong to a class of enzymes that control the ability of a cell to enter into and proceed through the cell division cycle. Using purine as a scaffold, we have synthesized a number of nanomolar inhibitors of CDK-2/cyclin E. In this report, the synthesis of a series of piperidine-substituted purine analogs will be presented, as well as some of their in vitro and in vivo biological effects.

[Evaluation of an immediate cleaning and decontamination device].

A new technical approach for immediate cleansing and decontamination was studied. Control of cleansing was observed using macroscopic and microscopic (S.E.M.) examinations on different instruments currently used in dentistry. Antiseptic solution was tested regarding four bacterial strains representative of the normal oral flora. The synergistic action of the combined solution plus pressure was effective for removal of debris before they dry. Antiseptic solution was bacteriostatic and at concentration level recommended by the manufacturer, viable colonies were not detected.

In vivo and in vitro induction of 2'-5' oligoadenylate synthetase by interferon-alpha in nodular non-Hodgkin's lymphoma and correlations with the clinical response.

We investigated the correlations between the in vivo-in vitro induction of 2'-5' oligoadenylate synthetase (2-5A synthetase) by IFN-alpha in cells isolated from patients with low-grade nodular non-Hodgkin's lymphoma (NHL) and subsequent clinical responses of these patients to IFN-alpha therapy. Eleven patients were treated daily with 9 x 10(6) U of IFN-alpha 2a in a phase II trial. After an eight week treatment, four patients achieved complete remission, one a partial response, one a minor response, and five failed to respond. Basal levels of 2-5A synthetase in lymph node tumor B cells and peripheral blood mononuclear cells (PBMC) isolated before therapy differed from patient to patient and were significantly lower than in PBMC from healthy donors (P less than 0.03). In vivo single injections of 9 x 10(6) U IFN-alpha 2a induced the 2-5A synthetase in PBMC from all patients to various degrees without quantitative relation to the clinical responses. Injection of a tenfold lower dose resulted in effects of similar extent in most cases. In vitro, IFN-alpha 2a induced the 2-5A synthetase in lymph node tumor B cells isolated before therapy, and the degree of induction was significantly higher in patients who proved to respond to therapy than in patients who displayed no or minor responses (P less than 0.013). This indicates that, in nodular NHL, the 2-5A synthetase assay may have some predictive value for responsiveness to IFN-alpha therapy.

[Quantitative study of the medullar fibrosis in 84 cases of idiopathic myeloid splenomegaly (author's transl)].

84 bone marrow biopsies were analysed by quantification methods with Classimat. The initial diagnosis was: idiopathic myelofibrosis with splenomegaly. A previous study divided the patients among three categories, according to the classification in three progressive stages of the bone marrow disease. The quantitative lecture was done after reticulin coloration, "in blind", and on several biopsies from the same patient. The results show good correlation with classical histologic methods. The quantitative measure of fibrosis is already important as soon as the first stages of the disease. Correlations with prognosis are more difficult to define.

[Initial colonization of composite resins by Streptococcus mutans: the influence of polishing].

The purpose of the present investigation was to compare the in vitro initial colonization of Streptococcus mutans on three composite materials (HERCULITE, FILLISPAD and SILIFILL) regarding the polishing procedure (SOFT-LEX, 3M). Surface roughness and bacterial deposit were analyzed by SEM and the deposit was estimated using a modified method of SKJORLAND. Results indicated that surface roughness was reduced by the polishing procedure, but this roughness was more important at the periphery of restorations. Bacterial deposit was strongly reduced on the polished materials but bacteria not adhered in the same number on the composites with a comparable surface roughness: the accumulation of bacteria was less important on FILLISPAD materials. It is concluded that the initial colonization of resin composites was not only dependent of surface roughness.
