Needle dermabrasion.

In this article we describe a technique of needle dermabrasion (tattoo without pigment) used to improve achromic, hypertrophic, and unsightly scars. It is simple, safe (no complications), and it gives us consistently good results.

Nose surgery (rhinoplasty) without external immobilization and without internal packing: a review of 812 cases.

In a review of 812 cases of rhinoplasty, none of our patients had early bone or septal displacement; swelling, bruising, and pain were almost nonexistent. This confirms that an external splint would not have been of any benefit in these cases. Packing should help prevent epistaxis, synechiae, and early bone and septal displacement. Not using any packing, we have not encountered these complications. Besides, we have not seen a single submucosal hematoma or a septal necrosis. Therefore, we doubt the value of packing in our patients. The inconveniences and complications of external splints and internal packing are described. Early postoperative photographs show the reduced swelling and bruising, and late photographs show the final results. Difficult primary and secondary rhinoplasty cases are demonstrated.

Molecular dynamics studied by analysis of the X-ray diffuse scattering from lysozyme crystals.

It is now well established that the biological activity of proteins is related not only to their mean molecular structure, but also to their intramolecular mobility. Nearly all techniques sensitive to dynamics have given evidence for intramolecular mobility in proteins: NMR, ESR, Raman spectroscopy, fluorescence quenching, Mössbauer spectroscopy, neutron scattering, measurements of elastic constants and hydrogen-deuterium exchange. The dynamics of proteins has also been approached by theoretical calculations. We report here investigations of the atomic and molecular displacements in hen egg-white lysozyme crystals using a new technique. This technique, based on the X-ray diffuse scattering analysis (scattering out of the Bragg reflections), can yield information on the atomic displacements, provided that they are correlated. Rigid-body molecular displacements, correlated along short rows of aligned molecules in two perpendicular directions, have been detected and analysed (mean square amplitude of the order of 5 X 10(-4) nm2). This technique can be also applied to the detection and analysis of intramolecular displacements.

Rhabdomyosarcoma metastasizing as a malignant ectomesenchymoma.

Rhabdomyosarcoma is a common childhood malignancy that may occasionally occur as a component of a mixed mesenchymal tumor, e.g., a triton tumor or malignant ectomesenchymoma. A case is reported of a 13-year-old boy who had resection of a paratesticular embryonal rhabdomyosarcoma with subsequent radiation and chemotherapy. Two years later, a retroperitoneal metastasis was resected. Histology showed a mixture of rhabdomyoblasts, ganglion cells, and a third population of cells with combined features of these two distinct cell types. Electron microscopy confirmed the presence of rhabdomyoblasts with characteristic bundles of myofilaments and Z-band material, and ganglion cells with prominent nuclei and nucleoli, rough endoplasmic reticulum, dense core granules, filaments, and tubules. Notably, the third cell population showed features of both rhabdomyoblasts and ganglion cells. Immunohistochemistry confirmed the mixed population of rhabdomyoblasts (positive for vimentin, desmin, negative for S-100, NSE), ganglion cells (positive for S-100 and NSE, negative for vimentin and desmin), and the third population expressing all test antigens. The features of this metastatic lesion are those of a malignant ectomesenchymoma with combined rhabdomyosarcoma and ganglioneuroma components. It is postulated that this lesion results from biphenotypic expression of tumor cells that previously expressed only rhabdomyoblastic differentiation. The role of prior chemotherapy and radiotherapy in this particular case is unclear.

Levels of chlorinated hydrocarbon residues in Canadian human breast milk and their relationship to some characteristics of the donors.

A total of 412 breast milk samples from women in all provinces of Canada were analysed for polychlorinated biphenyls, eight chlorinated benzenes, 2,3-dichloronaphthalene, Mirex, alpha, beta, gamma and delta hexachlorocyclohexane, alpha and gamma chlordane, oxychlordane, transnonachlor, p,p'-DDT and some analogues, heptachlor epoxide, dieldrin and octachlorostyrene. No delta-hexachlorocyclohexane, heptachlor or aldrin were found in any of the samples, while median levels of the 1,2,4- and 1,3,5-trichlorobenzene, 1,2,3,4- and 1,2,3,5-tetrachlorobenzenes, gamma chlordane, o,p'-DDT and octachlorostyrene were all less than the minimum detectable level (MDL). All other compounds were present at median levels ranging from < 0.1 to 7.2 ng/g whole milk, but did not occur in all samples. Di to tetrachlorobenzenes, except 1,2,4-trichlorobenzene, 2,3-dichloronaphthalene and alpha-chlordane were observed in < 50% of the samples. From 1967 to 1986 a steady decline of hexachlorobenzene, gamma-hexachlorocyclohexane, DDTs, heptachlor epoxide and dieldrin was observed in Canadian breast milk. In addition, a decrease in some chlorinated benzenes, gamma-hexachlorocyclohexane and PCBs was also observed between 1982 and 1986. Levels of oxychlordane and trans-nonachlor remained constant. Canadian breast milk contamination appears to be one of the lowest among the industrialized nations. Regional and provincial differences in residue levels appeared minimal, although more often samples from Quebec and British Columbia had higher levels of some contaminants, such as oxychlordane, than samples from other provinces. No relationship was found between maternal age and residue levels, but some contaminants, such as PCBs, were significantly higher in the milk of mothers who breastfed their first child as compared to multiparous mothers. Breast milk residue levels were not related to fish consumption.

[Coprologic survey in Bilharzia infected children of the Adzope region (Ivory Coast)].

During a stool examination survey on 174 bilharzia attained children living in a rural area of Ivory Coast, 93% were found to have intestinal parasites mainly Entamoeba coli, Necator americanus and Ascaris lumbricoides. The prevalence of the cysts of Entamoeba histolytica (4,7%) is lower than the one found in France (7%). Concerning the other intestinal parasites, their respective occurrence have been compared with those noted in other regions of Ivory Coast and of Africa.

[Anatomo-pathologic aspects of phycomycoses. 6 clinical cases observed in Ivory Coast].

Six cases of superficial phycomycosis have been recently demonstrated in Ivory Coast, the total number of cases being therefore 8 in this country. After a brief review of the etiology, clinical studies, treatment, and geographical distribution of the 150 world cases known to-day, the author studies the histological lesions of this hypodermic granulatous inflammation, whose characteristic aspects (polynuclear micro-abcesses, palissade histiocytes, filaments sheathed in an eosinophil precipitate) are easy to recognise. These anatomopathological aspects vary according to the topography, clinical evolution and treatment. The author presents a summary of the only ultrastructure known today. He points out the significance of this anatomo-pathologic study, which shows the evolution of the immunitary reaction and permits a relatively precise pronostic.

[A multifactorial etiologic approach to diabetic dyschromatopsia. Study of 100 diabetics].

Diabetic dyschromatopsia is frequent and is a true complication of diabetes mellitus. Causative factors other than retinopathy have been suggested, but they remain unclear. We have explored the color vision of 100 diabetics aged 16 to 65 (88 insulin-dependent, 12 non-insulin dependent) with Lanthony's D15 desatured panel. Degenerative complications were looked for, especially by fundoscopy and electrophysiological exploration of peripheral nerves using specific scoring. 73% of the diabetics had dyschromatopsia. Dyschromatopsia was significatively more frequent when retinopathy was present (26 out of 30 diabetics with retinopathy versus 47/70 without). We explain the absence of a strict parallelism between dyschromatopsia and retinopathy by the intervention of other factors. Whereas the equilibration of the diabetes was not different between the groups with or without dyschromatopsia, patient age, microalbuminuria, blood pressure and alcohol intake were higher in patients with dyschromatopsia. The greater prevalence of peripheral neuropathy in patients with dyschromatopsia, confirmed by electrophysiology, and independently from the existence of retinopathy, is an indicator of the existence of neuronal disease, whose level remains to be determined. Our results are the indispensible preliminary step to a study of the respective importance of these pathogenic factors.

[Improvement of diabetic polyneuritis with naftidrofuryl. Preliminary results].

This study was a 6-month open trial of the effects of naftidrofuryl fumarate (600 mg/day orally) on the clinical and electrophysiological signs of diabetic polyneuropathy in 15 patients. A scored clinical evaluation and an indexed electrophysiological examination were done at inclusion, and after 3 and 6 months. Mean clinical score and electrophysiological index was improved after 3 months of therapy, the improvement persisting at 6 months, with no change in control of diabetes. These results could be related to the vasodilating properties of the drug, causing an increase in nerve temperature, or to a specific neurotropic action, as recently demonstrated in rats. Controlled studies are necessary to confirm these results, and explore the mechanism of this improvement.

[Antimalaria fluorescent antibodies evolution in young children living in a stable malaria area (author's transl)].

The detection of malarial fluorescent antibodies have been performed in an Abidjan dispensary on 30 newborn babies with their mothers, 30 children 3 months old and 120 children from 6 to 24 months. This survey took place during the small rainy season and it demonstrates that: --synthesis of specific antibodies is significantly starting after the 3rd month; --the rise, after six months, of the malarial antibodies is parallel to the plasmodic index checked during the same period in the same area; --the rate of antibodies in 2 years old babies is almost the same that in adults; --living conditions in rural areas accelerate the synthesis of antibodies which, adversely, is slowed down by regular chemoprophylaxis; --Plasmodium berghei is a valuable antigen for mass survey; --Abidjan town is an hypoendemic zone of stable malaria in a mesoendemic area.
