Hyperfunctioning parathyroid carcinoma presenting as acute pancreatitis in pregnancy.

Hyperparathyroidism during pregnancy is an uncommon condition. Several case reports and literature reviews in the last two decades have reported only benign tumors but have demonstrated that this disease is associated with significant neonatal morbidity and mortality as well as maternal morbidity. Parathyroid carcinoma is a rare disease entity associated with a much more severe course and prognosis than parathyroid adenomas; it has not been reported in a pregnant woman. We report here a case of a hyperfunctioning parathyroid carcinoma that presented as acute pancreatitis in pregnancy. The details of the case management and outcome in both the patients and infant are discussed. A summary of the literature on the findings and management of parathyroid carcinoma is also discussed in relation to this case report.

Instrumentation of the spine for fracture dislocations in children.

13 children, ages 11-16 years, sustained severe compression fractures or fracture dislocations of the thoracolumbar spine. All cases were treated with Harrington instrumentation with fusion and six also had decompressive laminectomy. All injuries had significant instability and neurologic deficits (nine complete). Follow-up of 2-10 years is provided for analysis of maintenance of reduction and neurologic improvement. This technique appears to offer stability, reduction of orthopedic defects, and assumption of a more aggressive rehabilitation program without introducing significant operative morbidity or neurologic deficit.

Interaction of apoprotein from porcine high-density lipoprotein with dimyristoyl lecithin. 1. The structure of the complexes.

The morphology and structural organisation of the complexes formed from the apoprotein of porcine high-density lipoprotein and dimyristoyl phosphatidylcholine (lecithin) have been studied using the technique of small-angle X-ray scattering. Scattering measurements made in solvents of varying electron density were interpreted in terms of a scattering-equivalent model for the structure of the complex. This model is described by an oblate ellipsoidal morphology with dimensions at 20 degrees C: major axis 11.0 nm, minor axis 5.5 nm. Within this overall shape the lipid hydrocarbon chains are organised in an apolar core whilst the lipid polar head groups and protein are located in a outer shell 0.85 nm in thickness. The oblate morphology demonstrates that the structure of the complex is directed by the fundamental bilayer organisation of the lecithin. The dimension of the minor axis (5.5 nm) indicates that phospholipid hydrocarbon chains are orientated perpendicular to the interface.

Zamia palm (Macrozamia reidlei) poisoning of sheep.

Sheep mortalities caused by the ingestion of leaves of the zamia palm (Macrozamia reidlei) were recorded from 6 properties in Western Australia from 1966 to 1971. Deaths occurred in sheep less than 2 years of age and involved from 2 to 9% of the flock. The primary lesion was acute hepatic necrosis, neurological effects were not recorded. The condition was reproduced experimentally when crushed kernel, mascerated leaf or leaf extract was administered by stomach tube.

Disk susceptibility studies with cefazolin and cephalothin.

Cefazolin and cephalothin disk susceptibility and minimal inhibitory concentration determinations were conducted on 591 clinical isolates. Cefazolin demonstrated superior activity, as shown by lower minimal inhibitory concentrations, and a greater percentage of isolates inhibited in the disk susceptibility test. The cephalothin antibiotic class disk by the standard Bauer-Kirby method failed to detect susceptibility to cefazolin in a significant percentage of Escherchia coli, Enterobacter species, and Enterococcus isolates. A separate cefazolin disk with a susceptibility cut-off point of 18 mm is recommended. An alternative to a separate cefazolin disk would be a reinterpretation of the cephalothin susceptibility disk zone diameters so that it would more adequately predict cefazolin activity.
