[Summer holiday camps for asthmatic children--an attempted assessment (author's transl)].

118 schoolchildred (males:females = 2.5 : 1) with bronchial asthma--defined as recurrent allergic or non-allergic expiratory dyspnoea--were investigated as regards their clinical course, frequency of asthma attacks, exercise tolerance, anti-asthmatic drug requirements and spirometric changes during and after participation in specialized holiday programmes during the summer months in 1974, 1975, and 1976. Clear-cut improvement was observed with regard to asthma attack frequency (up to nearly 100%) and drug requirements (up to 76%). Steroid dependence was observed in most cases. No significant spirometric improvement was demonstrable. All data require critical evaluation and interpretation in view of seasonal variations in asthmatic manifestation and the influence of environmental factors. On reviewing the results it is concluded that treatment of the asthmatic child by supervised high altitude holidays (1400 m above seal level), combined with physical training and a respirator re-education programme, is still a justified therapeutic approach, but it cannot replace specific therapy laid down on the basis of careful diagnostic testing. Particpation of the asthmatic child in school gym classes is recommended. Abstention from physical exercis due to a misplaced protective attitude is not beneficial in most cases.

Regio- and stereoselectivity of various forms of purified cytochrome P-450 in the metabolism of benzo[a]pyrene and (-) trans-7,8-dihydroxy-7,8-dihydrobenzo[a]pyrene as shown by product formation and binding to DNA.

Highly purified cytochromes P-450(LM2) and P-450(LM4) and partially purified P-450(LM1), P-450(LM3b), and P-450(LM7) from rabbit liver microsomes exhibit different catalytic activities in the metabolism of benzo[a]pyrene (BzP) and (-)-trans-7,8-dihydroxy-7,8-dihydrobenzo[a]pyrene [(-)trans-7,8-diol] in a reconstituted enzyme system. The two highly purified cytochromes also exhibit differences in the activation of BzP and (-)trans-7,8-diol to intermediates that bind to DNA, as well as in the stereoselective conversion of (-)trans-7,8-diol to the highly mutagenic and carcinogenic diol-epoxides r-7,t-8-dihydroxy-t-9,10-oxy-7,8,9,10- tetrahydrobenzo[a]pyrene (diol-epoxide I) and r - 7,t - 8 - dihydroxy - c - 9,10 - oxy - 7,8,9,10 - tetrahydrobenzo[a]pyrene (diol-epoxide II). P-450(LM2) is more active than P-450(LM4) in the metabolism of BzP and in its conversion to products that bind to DNA. In contrast, P-450(LM4) is more active than P-450(LM2) in the metabolism of (-)trans-7,8-diol and in its conversion to products that bind to DNA. The ratio of activity (percent substrate metabolized) with BzP relative to that with (-)trans-7,8-diol is 21 for P-450(LM2) and 0.3 for P-450(LM4); P-450(LM1), P-450(LM3b), and P-450(LM7) gave intermediate ratios. Marked stereoselectivity in the oxygenation of the (-)trans-7,8-diol to the highly mutagenic and putatively carcinogenic diol-epoxides I and II was observed with P-450(LM4), whereas the other preparations showed less selectivity. The ratio of diolepoxide I to diol-epoxide II ranges from 0.3 for P-450(LM7) to 11 for P-450(LM4). The substrate specificity and regio- and stereo-selectivity of the different forms of cytochrome P-450 may regulate the balance between activation and detoxification pathways of BzP and therefore determine the susceptibility of individual tissues, strains, and species to the carcinogenic action of BzP.

Maleimide as an inhibitor in measurement of erythrocyte glucose-6-phosphate dehydrogenase activity.

Erythrocyte glucose-6-phosphate dehydrogenase activity is measured with a centrifugal analyzer by use of a commercial reagent kit and of the reaction glucose-6-phosphate + NADP+ leads to 6-phosphogluconolactone + NADPH. Rate of production of NADPH is measured and related to hemoglobin concentration. Maleimide is added to inhibit further production of NADPH in a secondary reaction by endogenous 6-phosphogluconate dehydrogenase. The method is compared with others that are designed to circumvent the secondary reaction by either (a) addition of excess phosphogluconate dehydrogenase to drive the secondary reaction to completion or (b) inhibition of endogenous phosphogluconate dehydrogenase by 2,3-diphosphoglycerate. The present method has the advantages that reaction rate more quickly becomes linear and reagent cost is less as compared with other methods. The within-run coefficient of variation was 3%. The various methods investigated showed good statistical correlation.

[Timing of a letal intracranial hemorrhage by means of the HbA/HbF ratio (author's transl)].

Possibilities and limits of the method for timing an intracranial hemorrhage by means of the HbA/HbF erythrocyte ratio are investigated in six newborns. The effect of labeling HbF erythrocyte containing newborn blood by transfusion and exchange transfusion of HbA erythrocyte containing donor blood as well as "self-labeling" after the first transfusion as a result of HbA--and HbF erythrocytes decaying at a different rate, is described. If a coagulum can be clearly allocated to the blood where it originates from, depends a) on the alteration of the HbA/HbF erythrocyte ratio by the above mentioned ways of labeling and b) on the size of the sample i.e. on the number of erythrocytes counted to determine the HbA/HbF erythrocyte ratio. The confidence limits of the HbA/HbF erythrocyte ratio for a given number of counted erythrocytes have been determined. It was taken into account whether the blood was labelled by a transfusion or by "self-labeling". The analysis of the period before, during and after the intracranial hemorrhage made a preliminary evaluation of etiological factors and some symptoms possible. The hypothesis that sodium bicarbonate dosages of 8 mEqu/kg/24 h are an important cause of intracranial hemorrhage has to be rejected on the grounds of this study's results.

[Respiratory therapy in childhood asthma. Part 2. Eutonic training].

Children with obstruction mainly due to spasms of bronchial smooth muscle will benefit most from breathing exercises. Attainment of the individual physiological balance of muscular, nervous and psychogenic tensions--the eutonic tension--is of great importance for control of regular unhurried breathing. This will be supported by stimulation of nasal breathing which increases diaphragmatic excursion. The eutonic state is the fundamental basis for success of chest rotations and rhythmic movements to stimulate further relaxation. Awareness of the fact that breaths come by themselves -- "automatically" -- reduces anxiety of shortness of breath. Observations of a group of asthmatic children in a summer camp support this form of treatment of asthma.

Mitochondrial genetics. XI. Mutations at the mitochondrial locus omega affecting the recombination of mitochondrial genes in Saccharomyces cerevisiae.

1. A series of CS revertants has been selected from various strains (both omega+ and omega-) carrying a CR mitochondrial mutation at the RIB1 locus. The properties of mitochondrial recombination exhibited by these CS revertants in various crosses, have been examined systematically. The omega allele of the CS revertants has been defined in crosses with omega+ and omega- tester strains using two criteria: the polarity of recombination and a new criterium called relative output coefficient. We found that mutations of omega appear frequently associated with the mutations at the RIB1 locus selected from omega- strains but not with those selected from omega+ strains. A new allelic form of omega (omega n) which had not been found amongst wild type yeast strains is characterised. Similarly omega n mutation was found frequently associated with CR mutants at the RIB1 locus selected from omega- CS strains but not with those selected from omega+ CS strains. The omega n mutants, and the omega+ and omega- strains, explain the groups of polarity previously observed by Coen et al. (1970). 2. Main features of mitochondrial crosses with omega n strains (omega+ x omega n, omega- x omega n and omega n x omega n) are analysed. Recombination is possible between the different mitochondrial genetic markers. No high polarity of recombination is observed and the frequency of recombinants are similar to those found in homosexual crosses (omega+ x omega+ and omega- x omega-). A striking property, observed for the first time, exists in crosses between zota+ omega n CS strains and some zota- CREO mutants: the zota- CREO are unable to integrate by recombination their CR allele into the zota+ mit-DNA of omega n CS strains while being capable of integrating it into omega+ CS or omega- CS genomes. 3. It is proposed that the omega locus is the site of initiation of non reciprocal recombination events, the omega+/omega- pairing specifically initiates the non-reciprocal act while omega+/omega n or omega-/omega n pairings do not. 4. The molecular nature of the omega n mutation and its bearing on the structure of the omega locus are discussed. It is suggested that omega n mutations correspond to macrolesions (probably deletions) of a segment of the mit-DNA covering the omega and RIB1 loci. If omega n is a partial deletions of the omega- sequence the omega+ could be an additionnal deletion of the omega n sequence. 5. The occurrence of spontaneous CR and ER mitochondrial mutations has been analysed by the Luria and Delbrück fluctuation test in omega- and omega n isonuclear strains. Results of these tests indicate that an intracellular selection of resistant copies preexisting the action of the anttibiotic occurs.

[Respiratory therapy in childhood asthma. Part 1 (author's transl)].

The therapeutic approach in childhood asthma can be substantially supported by various forms of respiratory therapy. Objective improvements and increased well-being have been demonstrated by different authors. The physiology of correct and defective breathing are outlined briefly. The major aim of respiratory therapy is the return to normal breathing patterns including normalized clearing functions of the lung. If successful even moderately severe attacks of bronchial asthma may be averted. Personality changes in the form of increased self-confidence and loss of fear of dyspnoea are important. Part 1 outlines physicial treatment forms such as postural drainage and percussions of the chest.

[Treatment of extreme obesity in a case of Prader-Willi-Labhart-syndrome (author's transl)].

A boy 14 years of age, weighing 93,45 kg and 143 cm in height was refered to our clinic with hyperphagia and pain in the joints. The patient showed the Prader-Labhart-Willi-syndrome. Through maximal diet therapy, 2 admittances in our ward and through intensive psychological outpatient care, we have seen a weight loss of 26 kg in 18 months and clear improvement in the patient's health. We would like to draw attention to the fact that a weight reduction diet is a cooperative family effort. To satiate uncontrollable hunger an intermittent dose of Fenfluramine (0,5 mg/kg) is recommended.

[Combination of the syndrome of Sturge-Weber and the syndrome of Klippel-Trénaunay (author's transl)].

Up until now 39 cases of combined Klippel-Trénaunay syndrome and Sturge-Weber syndrome have been described. Here follows the report of a girl, now 4 years of age, displaying a full combination of these syndromes. Only a small part of the body surface is not covered with naevi teleangiectatici laterales. The patient has clear hypertrophy of the left cheek and of the left lower extremity, less noticeable on the left upper extremity. For therapeutic reasons the left side of the head and the left lower extremity were thoroughly angiographically examined--this revealed typical abnormalities. The vessel-alteration of the lower extremity are not extremely far developed and arteriovenous fistulas on a large scale are also absent. This allows us to dismiss the F.P. Weber syndrome on the one hand, while it explains the absence of complications of the Klippel-Trénaunay syndrome, as described in literature, on the other. The significance of the alterations of lymph nodes in this disease, which we are the first to describe, is at present not fully clear. The cerebral attacks have until now showed only a temporary response to medication.
