Identification of a ouabain-like compound in toad skin and plasma as a bufodienolide derivative.

An ouabain-like compound (OLC) was purified from toad skin. The purification procedure consisted of three sequential separations on HPLC using amino and reverse phase chromatography. Using UV, NMR and Mass spectroscopy the structure of the purified material is suggested to be mono-hydroxy-14,15-epoxy-20,22-dienolide glycoside (resibufogenin). Evidence is presented that this compound is also present in the toad plasma. It is suggested that the endogenous bufodienolide derivative participates in the physiological regulation of the Na+,K+-ATPase activity.

Further characterisation of the inotropic effect of a bufodienolide glycoside--an endogenous ouabain like compound.

A ouabain like compound obtained from toad skin and plasma and identified to be a steroidal bufodienolide glycoside was found to displace ouabain from its binding site and to inhibit Na+-K+ ATPase, and have positive inotropic effects on cardiac muscle. The ionic and rate dependence of this positive inotropy was studied in the frog atrium. The effect was dependent on extracellular potassium, sodium, and calcium concentrations and on the rate of stimulation, which is similar to the properties of cardiac glycosides. Occasionally, the compound gave transient or even negative inotropic responses, as do the glycosides. The action potential configuration was also affected by the compound in the same complex pattern as is that of cardiac glycosides. It is concluded that the endogenous bufodienolide compound has the same physiological effects as cardiac glycosides. Since the same compound is present in toad plasma it may serve as an intrinsic humoral regulator of cardiac contractility. This study is the first detailed characterisation of the cardioactive properties of this compound.

Evidence for the presence of 'ouabain like' compound in human cerebrospinal fluid.

Material extracted and partially purified from human cerebrospinal fluid (CSF) is capable of: a, inhibiting [3H]ouabain binding to rat brain synaptosomes; b, inhibiting the activity of purified pig kidney Na+,K+-ATPase; and c, inhibiting ouabain sensitive induced 86Rb influx to tissue cultured fibroblasts. These results demonstrate the existence of an 'ouabain like' compound (OLC) in human CSF, and are consistent with the hypothesis of the function of this compound as a neuromodulator.

Long term prognosis for babies with neonatal liver disease.

A total of 123 patients with neonatal liver disease without extrahepatic bile duct obstruction or arteriohepatic dysplasia have been studied for six to 18 years. Idiopathic neonatal hepatitis, present in 73 babies, carried a high mortality due to liver failure (18%), septicaemia (6%), and associated defects (14%), especially in the first year of life (25%). Progression to chronic liver disease in non-familial idiopathic cases occurred in three of 40 reviewed patients. Only 12 of these children were completely healthy, the remainder having other permanent disabilities (57%). Four of nine familial cases of idiopathic neonatal hepatitis died in the first 12 months of life as did two of the four reviewed survivors. Progression to chronic liver disease or to death was a continuous process without any interval of recovery in all but one of these patients. Among patients with a presumed infective cause, cytomegalovirus infection caused a particularly benign form of neonatal hepatitis but was a frequent cause of brain damage or other disabilities. Babies who survived other infective liver diseases showed complete healing of the liver damage. Neonatal liver disease associated with alpha 1 antitrypsin deficiency progressed to death or chronic liver disease in three of nine patients and was not associated with a paucity of interlobular bile ducts.

[Clonidine poisoning].

Accidental clonidine poisoning of infants has been more frequently observed during the last years. The early onset of sedation together with miosis, hypotension, bradycardia and sometimes respiratory depression should draw our attention to clonidine poisoning. Treatment of cardiac and circulatory symptoms with atropine sulfate, epinephrine chloride, dopamine hydrochloride and intravenous fluids should start early. Ventilatory support must be available for children in case of apnoe. We report our experience with the treatment of five infants.

Pancreatic polypeptide secretion. A marker for disturbed pancreatic function in cystic fibrosis.

Pancreatic polypeptide, a 36-amino peptide, is released from the pancreas by a variety of stimuli, including intravenous Boots secretin. Studies in a generalized destructive and inflammatory process such as chronic pancreatitis have revealed a markedly diminished response to stimulation. To assess whether pancreatic polypeptide release in response to Boots secretin provides a useful measure of pancreatic destruction in cystic fibrosis, 41 patients with proven cystic fibrosis, aged 14 months-23 years, and seven control subjects, aged 18-24 years were studied. Serum pancreatic polypeptide, measured by radioimmunoassay, rose from a basal of 18.5 +/- 2.7 pmol/liter to a peak of 35.6 +/- 4.3 pmol/liter at 5 min in cystic fibrosis, and from a basal of 10.8 +/- 2.8 pmol/liter to a 5-min peak of 109 +/- 27.7 pmol/liter in control subjects. The basal levels of both groups were similar but the cystic fibrosis patients had a significantly lower peak response than controls (P less than 0.05). The peak over basal pancreatic polypeptide ratio was calculated and was less than five in 93% of cystic fibrosis patients, whereas all control subjects had a ratio greater than five. Pancreatic polypeptide measurements in response to secretin may be a convenient and useful means of following the course of pancreatic disease in a chronic illness such as cystic fibrosis.

Pancreatic pseudocyst.

Pancreatic pseudocyst is a relatively rare complication of pancreatitis with a reported incidence of 1 to 5 per cent in patients with pancreatitis. The 5-year experience with pancreatic pseudocyst at Saint Francis Hospital and Medical Center and Mount Sinai Hospital has been reviewed in an effort to determine optimum diagnostic and therapeutic techniques. Twenty-eight patients were treated for this problem during the period of June 1976 through June 1981 with one death. All patients had operative therapy, with internal drainage being the procedure of choice. The most common presenting symptom was abdominal pain. The most useful diagnostic study proved to be ultrasonography. Complications occurred in nine patients (32%). These included bleeding, obstructive jaundice, infection, rupture, and recurrence.

Exposure to X-rays during small bowel biopsies in children.

Peroral small bowel biopsy is a decisive step in the diagnosis of coeliac disease and is performed frequently. The patient's exposure to ionizing radiation during fluoroscopic control therefore deserves consideration. Compared to data in the literature, the biopsy method employed at the University Children's Hospital in Vienna achieves a markedly shorter duration of fluoroscopy. These values were registered during 1039 biopsies. In 439 examinations the exposure area products were also documented In 10 consecutive biopsies additional measurements were carried out by lithium fluoride thermoluminescence dosimeters attached to the patient. Expert biopsy-technique with precise and thoughtful use of modern fluoroscopes on the one hand, and critical self-control by routine documentation of the exposure area product and duration of fluoroscopy on the other can markedly reduce the patient's exposure to X-rays.

Effects of ethylating agents on DNA synthesis in vitro: implications for the mechanism of carcinogenesis.

A highly carcinogenic ethylating agent, ethylnitrosourea (ENU), and a weakly carcinogenic one, diethylsulfate (DES) react with DNA to roughly the same extent but DES produces about 6 times as many unstable ethylated bases, which are gradually lost spontaneously under physiological conditions. The different rates of loss for the different DNA bases have been studied using polydeoxyribonucleotides. Spontaneous strand breakage following base loss is slow, lagging more than a week behind base loss at 37 degrees C; ultimately, DES results in much more spontaneous strand breakage than ENU. In DNA synthesis in vitro, using avian myeloblastosis virus (AMV) polymerase, no nucleotides are incorporated opposite missing bases in the template; when the template contains ethylated bases that are impaired in their ability to form specific hydrogen bonds, purine-pyrimidine mispairing can occur and mismatched nucleotides are incorporated into the daughter strand. ENU ethylates somewhat more sites leading to mispairing potential than DES. ENU also produces approximately 7.5 times more ethyl phosphotriesters than DES.

Field desorption mass spectrometry. II. Potassium cationization field desorption mass spectrometry of some penta- and hexapeptides derived from substance P.

In this article we present preliminary results of the application of potassium cationized field desorption mass spectrometry as an additional technique for the elucidation of structure and evaluation of purity of oligopeptides such as C-terminal penta- and hexapeptide analogs of substance P. In the resultant mass spectra both a protonated and a cationized molecular ion, MH+ and MK+ respectively, were observed. The m/z values of the two peaks were in agreement with the calculated molecular weights. The ratio between the relative abundancies of these ions (MH+/MK+) was found to be characteristic of the particular peptide and thus useful in the assessment of their purity. Among the 13 peptides studied, only two gave pyrolytic fragmentation leading to a more complex spectra.

Charcoal sorbent-induced hypolipidemia in uremia and diabetes.

Rats with hyperlipidemia associated with streptozotocin-induced diabetes or azotemia after subtotal nephrectomy were administered a diet containing 5% activated charcoal. Significant lowering of nonfasting serum cholesterol and triglyceride levels resulted. Charcoal-feeding also altered the abnormal high density lipoprotein electrophoresis pattern of diabetic rats toward normal.

[Cholestatic jaundice and hypophosphataemia in parenterally-fed premature infants--coincidence or causal connection? (author's transl)].

A report is presented of the chemical pathological findings in 14 premature and one full-term infant receiving almost exclusively parenteral nutrition during the first two weeks of life. Six infants developed cholestatic jaundice. The underlying diseases were the idiopathic respiratory distress syndrome in 10, gastroschisis in 3 and renal insufficiency in 1, while one was an otherwise healthy small for dates infant. After parenteral nutrition had been discontinued obstructive jaundice cleared by the third month of life except in one infant. Apart from the higher glucose intake during the second week, jaundiced infants principally differed from non-jaundiced infants by the development of significant hypophosphataemia. An attempt was made to correlate aetiologically the presence of cholestatic jaundice with the finding of hypophosphataemia on the possible basis of a disturbance of energy metabolism.
