Fine structure of palpebral molluscum contagiosum and its secondary conjunctival lesions.

One case of molluscum contagiosum of the lid, which has been followed by follicular conjunctivitis and superficial punctate keratitis, has enabled us to compare the ultrastructure of the skin tumor and of the conjunctival lesion. Electron microscopic observation is the only way for discovering the causal Poxvirus. Up to now isolation of the viral strain has not been possible. Examination of the skin tumor showed different maturation forms of the virus and associated cell alterations. In the conjunctiva there was an inflammatory reaction, with exocytosis and infiltration of the chorion by lymphocytes and plasma cells. Intracytoplasmic organelles were formed in the superficial epithelial cells. Their size and structure resembled that of the immature virions found in the epidermic cells. The comparison between skin and conjunctival lesions suggests the following sequence of events in the pathogenesis: the virus, leaving the lid tumor, probably penetrates the conjunctival cells and, without finding the conditions necessary for its full development, produces abnormal viral inclusions unable to become a mature virus. The excision of skin nodules thus cures the infection, since it prevents further reinfection by the skin virus. The morphological criteria on which this hypothesis is based must be confirmed by chemical and morphometrical studies.

[Extramembranous glomerulitis associated with visceral tumours. Clinical and histological study based upon 10 cases and a review of the literature (author's transl)].

The frequency of the association between extramembranous glomerulopathy and a visceral tumour suggests the responsibility of various antigen-antibody systems related to malignancy, at the origin of the nephropathy. In a number of special cases, the role of these systems has been more precisely demonstrated. The cases reported here emphasise the need to seek a visceral tumour when the onset of extramembranous glomerulitis occurs after the age of 40 years, in particular since the onset of proteinuria may precede the first clinical signs of the tumour by several months. The presence of polynuclear cells in the glomerular capillary loops, associated with extramembranous deposits, is an additional element of orientation in the direction of neoplastic aetiology.

Treatment of encephalopathy during fulminant hepatic failure by haemodialysis with high permeability membrane.

Forty-one patients with fulminant hepatic failure and coma underwent 180 periods of haemodialysis with polyacrylonitrile membrane (AN 69 HD). Hepatic failure was due to viral hepatitis in 40 and drugs in one. Total recovery of consciousness occurred in 17 patients (43.6%), and partial in seven (17.9%)--that is, an overall figure of 61.5%. Regain of consciousness was not related to liver regeneration as assessed by levels of factor V and hepatocyte volume fraction. At the time of the first haemodialysis, neurological status was significantly impaired in the patients who could not be aroused. Mean duration of coma grade IV averaged 6.1 +/- 4.3 days and mean duration of illness until death or decerebration 8.6 +/- 8.3 days. Of the 17 patients who totally regained consciousness, nine recovered and eight died (three from intercurrent complications and five with no liver regeneration).

[Ocular herpes : clinical variations and virus antigenic type (author's transl)].

Fifty-seven herpes simplex virus strains, isolated from ocular herpes, were screened by complement fixation, seroneutralization and thermosensitivity tests. They were distributed as follows : 44 type 1, 3 type 2, 7 intermediate type ; 3 had a type 2 thermosensitivity and a type 1 neutralization. Discrepancies in these results, with the three tests, show that there is, in each typed group, strains of virus which react differently, and this could indicate a continuity from one type to another. As the strain progressively reacts more like the type 2, clinical symptoms become more severe. In a certain number of cases where several simultaneous or successive samples were performed, there was a hypothetic possibility of change in the behaviour of herpes simplex virus strains in the eye.

Glycosylation of Escherichia coli L-asparaginase.

Reductive coupling with sodium cyanoborhydride has been used with lactose and N-acetylneuraminyl lactose to prepare glycosylated Escherichia coli L-asparaginase. A substantial degree of modification can be achieved without significant loss of enzyme activity. The lactosylated enzyme shows increased thermal stability and resistance to proteolytic cleavage and is cleared more rapidly from the plasma of mice, compared to native asparaginase. The effect on clearance varies directly with the degree of lactosylation. Asparaginase modified with N-acetylneuraminyl lactose, in contrast, with approximately 13.6 mol of N-acetylneuraminyl lactose/mol of enzyme, is cleared more slowly, with a t 1/2 that is approximately twice that of the native enzyme.

[Ultrastructural study of a case of molluscum contagiosum (author's transl)].

This is the case of a female child with molluscum contagiosum widely disseminated around the mouth and the eye-lids. Antibiotics were used to stop local staphylococcal superinfection. Follicular conjunctivitis and keratitis punctata disappeared only after complete removal of all cutaneous lesions. Biological diagnosis was done by direct observation of the Poxvirus with the electron microscope, since isolation of the virus itself is impossible. Ultrastructural study of the excised tumor has shown the different stages of the growth of the virus, and the associated cellular lesions.

[Myotonic dystrophy and acute respiratory insufficiency].

Myotonic dystrophy or Steinert's disease may be discovered during acute respiratory failure, sometimes caused by a general anaesthetic. It complicates chronic respiratory failure which is present in almost all cases, both restrictive and obstructive, the clinical signs of which progress with the myopathy. Apart from myotonic degeneration of the respiratory muscles, a hypoventilation syndrome of central origin has been described, but the etiology of this respiratory failure is dominated by repeated aspiration pneumonia favoured by constant dysphagia and passage of food into the trachea and poor cough reflex. One should attempt to correct this.

Immunological study of the aqueous humor in ocular herpes simplex.

(1) Isolation of herpetic virus and immunological studies of the serum and aqueous humor are two complementary methods of investigation in the diagnosis of ocular herpes simplex infection, their relative value depending on the clinical situation. (2) Of the different immunological techniques that of antibody titration by the hemagglutination method is an easy, sensitive test, but only if the Goldmann-Witner ratio is calculated. By this technique, the diagnosis of herpetic infection can be made in unusual clinical presentations or old cases in which all specificity had disappeared.
