Pilot results of DMF treatment time index.

The DMF Index and its components have been utilized by various researchers to measure aspects of the caries process ranging from prevalence of carious lesions to the amount of dental care needed. It is generally recognized that DMF Teeth (T) measures life-time caries experience and is not a good predictor of the amount of treatment time needed in population groups. The purpose of this study is to evaluate the ability of certain modifications of the DMFT components to predict the amount of treatment time needed. In a sample of 109 adult cases, normal and diseased conditions were coded along with the usual DMFT measures and the number of missing teeth needing replacement (NR). Corresponding treatment procedures related to caries (periodontal treatment was not coded) were recorded and then converted to treatment times using the Relative Productivity Unit (RPU). Stepwise multiple regression and discriminant analysis procedures were used. The number of D teeth was linearly related to treatment time, but the number of NR and M teeth were not. Regression procedures indicated that the square root values of D and NR were the best predictors of treatment time (RPU) and produced a multipler of 0.69. When patients were classified into categories of treatment time (low, medium, high), the DMF Treatment Time Index was able to correctly classify 70% of the cases into their appropriate treatment time category.

Eliminating office architectural barriers to dental care of the elderly and handicapped.

Methods by which the most common architectural barriers to dental care for the elderly and handicapped can be overcome are described. Some physiological and medical conditions that necessitate special provisions in location and design of the office management of patients have been described. The consequences of accessibility have been discussed and suggestions for management of elderly and handicapped patients are presented.

Specialty preference and attitudes toward the aged.

The relationship between the attitudes and knowledge regarding the aged and specialty preference of undergraduate medical students at three medical schools was studied. No significant differences in attitudes were found when students were classified by class (freshman, sophomore, junior, or senior) or by sex. A significant though weak relationship between class and level of cognitive knowledge was found. Students indicating a preference for family practice as a specialty demonstrated levels of cognitive knowledge which did not differ significantly from all other students. Similarly, attitudes of family practice students did not differ significantly from attitudes expressed by other students. However, when all students expressing a preference for a primary care specialty were compared to students expressing preferences for non-primary care specialties, primary care students had significantly more positive attitudes toward the aged than their peers. Levels of cognitive knowledge did not differ significantly. The data appeared to suggest that students selecting primary care specialties are somewhat more humanistic and empathetically oriented, at least in regard to the aged, than their peers while exhibiting essentially identical levels of cognitive knowledge. The implications of these findings for family practice education are discussed.

[Lymphogranulomatosis in infancy. histology, therapy and prognosis (author's transl)].

Between 10 and 14% of Lymphogranulomatosis are infants. In recent years distinct advances have been made through accurate histological classification, subdivision into clinical stages and aggressive radiotherapy and polychemotherapy. Therapeutic success depends particularly on the primary extent of the disease. But some therapeutic decisions require conclusive confirmation by still lacking longterm observations. In particular the number of possible late relapses cannot be estimated at present.

RNA content in human lymphocyte subpopulations.

Human peripheral blood lymphocytes are stained with the metachromatic dye acridine orange and the fluorescence of individual cells is measured by flow cytometry. The relative content of stainable RNA per cell is estimated by comparison with RNase-treated cells. Non-T and T lymphocytes have different mean quantities of RNA per cell, and these classes exhibit different distributions of RNA content. Non-T cells have a unimodal distribution with a sharp peak and exponential distribution towards higher RNA values. T cells have a bimodal distribution with two separate peaks. When T cells having receptors for IgG (Tgamma cells) and IgM (Tmu cells) are separated, each of these cell populations displays a unimodal distribution. Of these three lymphocyte subpopulations, Tgamma cells have the lowest content of RNA per cell. Non-T cells have slightly higher RNA content than Tgamma, and Tmu cells have twice as much RNA as Tgamma cells. The RNA content, which surely relates to the different functions of these lymphocyte subpopulations, may also be a useful marker for rapidly distinguishing the lymphocyte subpopulations.

Geriatrics program for medical students. II. Impact of two educational experiences on student attitudes.

In an innovative program in geriatrics for undergraduate medical students, the preliminary report concerned attitudes toward the aged among health care personnel. This follow-up report concerns the stability of, and changes in medical students' attitudes toward the aged after participation in one of the following types of geriatric educational experience: 1) a Family Practice clerkship, or 2) a selective course in Medical Humanities. Although both experiences resulted in positive changes in general attitudes, the one that emphasized empathy toward the elderly along with observation of healthy aged persons produced more positive feelings about rehabilitation potential. Implications for the geriatric training of physicians are discussed in the light of these findings.

Reversible graft versus host reaction as cause of erythrophagic splenomegaly in a child?

The case history of a 9 months old infant with hepatosplenomegaly, pancytopnaenia and disturbances of clotting and cellular immune reactivity is reported. The spleen was removed and showed striking erythrophagocytosis by proliferating histiocytes, typical of "familial erythrophagocytic reticulosis" (Farquhar). A graft-versus-host reaction is discussed as a possible underlying cause. The favourable clinical course and full recovery point to an interrelation with primary hypersplenism.

Geriatrics program for medical students and family practice residents. I. Establishing attitudes toward the aged.

This report concerns the initiation of required experience in geriatrics for undergraduate medical students and a program of research designed to evaluate that experience. Through the use of questionnaires, an attempt was made to differentiate between attitudes toward the elderly and their rehabilitation potential as stated by medical students, family practice residents, and professional and nonprofessional nursing-home staff members. Important differences were found between professional and nonprofessional groups, but no significant differences among professionals. Implications for the training of professional and nonprofessional staff members are discussed, and further research plans specified.

[Extramedullary tumor formation by acute myeloid leukoses in children (author's transl)].

In rare cases, leukoses can produce tumors before general symptoms and changes in the blood picture appear. Two boys were reported in whom tumors of the eye socket were removed, the tumors were histologically classified as myelosarcoma. During the course of several months, the clinical picture of an acute leukemia developed. The difficulties involved in the exact histologic recognition of myelosarcoma, its localization, the differential diagnosis of tumors of the eye socket in children as well as the therapy and prognosis for children with myelosarcoma were discussed.

The effect of the organization and status of family practice undergraduate programs on residency selection.

Family practice, as a medical specialty, is designed to help fill the void in primary care availability. In order to expose medical students to family practice and provide a basis for choosing a residency in the field, many medical schools have developed undergraduate programs in family practice. This paper reports the results of a survey conducted in March 1975 on the status of undergraduate programs with particular focus on the relationships between administrative status, size of program, faculty size, and type of undergraduate curricula to the number of graduates choosing family practice as a specialty. The data indicate that there is a relationship between the commitment of the school to family practice, the size of the program, and the presence of required courses in the curriculum to the success of the program, as measured by the proportion of students in each school who choose family practice residencies.

Projecting shortages and surpluses of dentists from available data.

The usefulness of manpower projections are reviewed and methodologies for projecting are reviewed. Estimates of current (1975) and projected (1990) surpluses and deficits of dentists are presented for Illinois. The model used to derive these estimates has the following characteristics. 1. Only generally available data are used. 2. Mortality rates of dentists, production and retention of new dentists, dental productivity by age, and immigration of dentists are all accounted for. 3. It is assumed that the distribution of new dentists in the future will be similar to the distribution of currently practicing dentists. 4. The assumptions regarding the demand variables of dental productivity and patient visits per person per year can be varied according to the specific situation.
