Twenty-one patients with disseminated colon carcinoma and clinically significant liver metastases were treated with 5-FUDR via hepatic artery infusion (HAI). All patients had previously received systemic chemotherapy consisting of either 5-fluorouracil aone or in combination with other agents. At the time of the initiation of the HAI, clinical disease in all patients was progressing. A PR of hepatic metastases was noted in eight patients (35%) with a median and mean duration of response of 4.5 and 5.0 months respectively. The median and mean survival from the start of HAI for responders was 8.0 and 9.0 months and for nonresponders was 1.0 and 1.6 months respectively. It appears that a significant response rate can be achieved with HAI of 5-FUDR in spite of previous exposure to fluorinated pyrimidines.

The plausibility of preoperative chemotherapy and radiotherapy in the treatment of patients with carcinoma of the rectum is demonstrated. The finding of specimens with no tumor along with a higher proportion of Dukes' A lesions than expected was most encouraging. Whether this is a false impression because of the small number of patients, or whether it demonstrates upgrading of pathologic staging by preoperative adjuvant therapy, remains to be determined.

A digital image network has been installed at the Department of Radiology of the University of Santiago de Compostela, Spain, to create a "limited' Picture Archiving and Communication System (PACS). This experience is being dedicated to address problems associated with digital techniques in a research environment. The backbone of the system is a multiprotocol ethernet network. Attached to the network are a number of advanced devices such as DEC VAX and UNIX workstations. Currently, a high resolution film digitizer and a laser printer are under evaluation for radiologic image research. During a period of nine years, 1987 to 1995, experimental and clinical trials have been conducted on different film based digital radiography apparatus primarily dedicated to chest and breast imaging. Several research projects have been completed. In this paper we describe the results of these investigations and discuss the advantages and disadvantages of this digital technique. The results of the different completed studies will be presented separately. A description of the physical characteristics of the limited PACS system dedicated to a research environment will serve as background.
BACKGROUND AND OBJECTIVES: Interscalene brachial plexus block (ISB) is associated with phrenic block and diaphragmatic paralysis when high volumes (40-50 mL) of local anesthetic are injected. The goal of our study was to test if a low volume of local anesthetic administered while maintaining proximal digital pressure might more selectively block the brachial plexus and decrease the frequency of phrenic nerve block. METHODS: Twenty healthy patients undergoing ISB for orthopedic surgery of the upper extremity were randomly allocated to receive either 20 mL 1.5% mepivacaine while proximal digital pressure to the site of puncture was performed, or 40 mL 1.5% mepivacaine without digital pressure. Spirometry and clinical data were evaluated at baseline, 10, and 90 minutes after accomplishing the block and after the motor and sensory block resolved. Diaphragmatic excursion during deep inspiration was also evaluated 90 minutes after the block was performed, with the patient in the sitting position. RESULTS: Interscalene brachial plexus block produced diaphragmatic paralysis in all patients included in the study, as demonstrated by the pulmonary function testing and the chest radiograph. No significant differences were found in any of the parameters studied. At 10 minutes, baseline functional residual capacity had diminished by 34 +/- 10% in the 40 mL group and 37 +/- 13% in the 20 mL group. Maximum cephalad sensory dermatome level was also similar in both groups, being C 3 or above in all patients. Ipsilateral hemidiaphragmatic motion was similar in both groups (3.2 +/- 2.3 cm in the 40 mL group and 2.6 +/- 1.7 cm in the 20 mL group). However, in no case was dyspnea manifested. CONCLUSIONS: Decreasing the volume of local anesthetic and applying proximal digital pressure to the site of injection is not effective in reducing the cervical block spread and the frequency or intensity of diaphragmatic paralysis during interscalene ISB.
We have evaluated the technique of right bronchial intubation for selective right pulmonary ventilation using one lumen tracheal tubes as an alternative to double lumen tubes. We studied 20 patients ASA II-III with a relatively preserved pulmonary function who were programmed for left thoracotomy. We used Shiley nr. 9 or Mallinckrodt nr. 11 tubes. After endotracheal intubation the tube was blindly advanced to the main right bronchus. The position of the tube was assessed by auscultation and it was verified and modified, if necessary, by fibroscopic visualization. The tube was advanced in such a way that Murphy's hole of the endotracheal tube remained in front of the exit of the right superior lobar bronchus. In three patients (15%) blinded placement of the tube was appropriate and in 4 patients (20%) fibroscopic replacement of the tube was required. In the remaining 13 patients (65%) placement of the tracheal tube was considered incorrect: tube rotation in 7 cases, upper placement of the Murphy's hole with respect to the origin of the superior lobar bronchus in 4 cases, and excessive distal placement of Murphy's hole with respect to the superior lobar bronchus in 2 patients. Complications related with the incorrect position of the tube were: leaking of gas into the left bronchium in 5 patients (25%), displacement of the tracheal tube into the main left bronchus requiring withdrawal of the tube to the trachea in one case (5%), hypoxemia (saturation of O2 lower than 90%) in spite of ventilation with FiO2 = 1 in two patients, moderate hypercapnia in three cases, and atelectasis of the right superior lobe during the postoperative phase in three patients (15%).(ABSTRACT TRUNCATED AT 250 WORDS)

Twenty-five patients with disseminated gastrointestinal malignancies were treated in a phase I study with a combination of ftorafur and mitomycin C or florafur and methyl-CCNU. Most patients had been heavily treated previously. Gastrointestinal, central nervous system, and marrow toxicity were manageable. Tumor regression was noted in five of 25 patients. A large-scale phase II study in untreated patients with gastrointestinal malignancies appears indicated with a combination of these agents.
