[Total right hemispherectomy: neurophysiological study after twenty-six years].

Twenty-six years after a total right hemispherectomy for tumor when thirteen years old, a right-handed patient presented with disturbances of equilibrium, urinary incontinence, and a marked reduction in motor impulses, in spite of a subnormal intelligence quotient (0.85). Computed tomography examinations suggested the presence of a normal pressure hydrocephalus. However intracranial pressure monitoring and isotopic and cerebrospinal fluid perfusion tests showed the absence of any cerebral obstruction, and free communication between the cerebral and spinal spaces, though intracranial volume capacity accommodation was greatly reduced. Clinical examination demonstrated good quality sensory appreciation by the left half of the body, the possibility of detecting an object in the visual hemifield supposedly blind, and perfect location of a sound source. Somesthetic evoked potentials were studied by stimulating the median nerve in the left and right wrist regions. Left hemisphere evoked potentials from a contralateral stimulus, were of normal latency, amplitude and morphology, the response to an ipsilateral stimulus also presenting an early potential component (N 26 starting at N 21). This exceptional finding in a hemispherectomized patient could be related to activity of the spinoreticulothalamic tracts in the lemniscal or extralemniscal systems. Auditory function tests demonstrated perfect stereoaudiometry and the quality of perception during dichotic listening tests. Auditory evoked potentials in the brain stem included only minor functional-type anomalies, with slightly increased latencies in the pons on the left, and the mesencephalon and pons on the right. The presence of a bilateral wave VI was related to activity of the median geniculate pathway. Visual function tests showed good quality left photomotor reflexes and positive object detection in the anopsic hemifield. Results of these tests were reproducible during the same examination and after an interval of several months. The relevant literature is reviewed.

[Sub, retro- and supracerebellar arachnoid cysts and pseudocyts in adults. Six observations].

Four arachnoïd cysts and two arachnoïd communicating pouchs in sub, retro and supra cerebellar situation are presented. These formations are rarely encountered in adults and distinction is often difficult from other cystic lesions whose structure is different (neuroglial and ependymal cysts). Intellectual slowing, mild ataxia or urinary disturbances, related to chronic hydrocephalus may be the only clinical symptoms. After computed tomography, isotopic cysternography, rather than metrizamide cysternography, must precise the possible communication between the cystic pouch and the subarachnoïd spaces, and the possible ventricular reflux. These informations are mandatory before selecting the treatment. In case of a non communicating cyst, total or subtotal excision should be preferred.

[Delayed traumatic intracranial hematomas. Value of computer tomography].

Repeated Computerized axial Tomography (CAT) scanning of 4 patients with head injuries showed tardive development of 2 intracerebral hematomas and 2 extradural hematomas. The initial CAT scans were normal; the hematomas were late in developing. The value of repeated post-traumatic CAT scanning, and the pathogenesis of late developing hematomas are discussed.

Anatomy and radiology of the sellar spine.

Five anatomical and radiological observations of a spine protruding into the pituitary fossa are reported. This osseous spine, about 4 mm long, arises in the midline from the inferior part of the anterior aspect of the dorsum sellae and extends upward and forward. The possible origins of this malformation are discussed.

[Arteriovenous malformations in deep cerebral area. Topographic classification. Therapeutic possibilities and results about 46 cases (authors' translation)].

The authors study the clinical dispositions of 46 arteriovenous malformations developed in deep cerebral area and detected in 41 cases by an haemorrhagic accident and after make a radiological classification. They class the M.A.V. of nucleus centralis with the localizations of the nucleus caudate's head, the insula and lentiformis nucleus and the capsulo thalamic area; the ventriculo-cisternalis M.A.V., the corpus callosum M.A.V. and complicated forms. With this classification they analyse the different operative technics that permit to obtain a complete extirpation in 26 cases about 39 operated patients. The results are excellent in 19 cases, good for 8 cases and mediocre in 4 cases. Eight patients are died during post operative time. The most important factors for the final result are the disturbance of consciousness before operative time and the malformations developed about several anatomical area. At last the authors insist on the fact that, if during next years, the embolism technics are not effective, the operation will be indicate in spite of the technical difficulties because the spontaneous evolution is always dangerous.

[Oculocephalomotor disorders and bilateral thalamo-subthalamic infarction (author's transl)].

A 33-year-old woman on oral contraceptives suddenly developed neurological disorders characterised by decreased alertness, absence of motor and verbal spontaneity, anterograde amnesia with difficulties in verbal and visual learning. She also presented paralysis of upwards and downwards ocular movements affecting mainly saccadic movements while convergence was normal. An initial almost total absence of spontaneous head movements and later an impossibility to carry out downwards movements of the head. This apparently represents a syndrome still not reported in human pathology. The clinical course was favourable. CT scan suggested the presence of a paramedian infarct of the region supplied by the thalamosubthalamic artery of the type II b of Percheron. An analogy is suggested between these disorders of oculocephalomotor behaviour and those observed by Denny-Brown in experimental bilateral retrorubric lesions. The possibility of a lesion in the internal thalamic nucleis and the adjacent subthalamic region is considered.

[Pseudo-tumoral cerebellar infarction. Diagnostic, therapeutic and prognostic aspects in 12 cases (author's transl)].

In those 12 observations, without associated lesion in the brain stem, diagnosis is done 3 times at autopsy, 8 times at surgical time, and once before it, on clinical and radiographic arguments. Diagnosis must be systematically evoked in front of vestibular symptoms, before severe signs, related to intracranial hypertension and brain stem compression, become evident. Iodoventriculography (7 times) and principally computerized tomography (2 times) bring conclusive arguments for diagnosis. Those infarcts are situated 9 times in the territory of the postero-inferior cerebellar artery and 3 times in the area supplied by the antero-superior cerebellar artery. Spontaneous evolution is always lethal, but prognosis after surgery is still severe (4 long term survivals in 9 cases) for the vascular system of those patients remains fragile.

[Prognostic importance of the study of venous drainage in cerebral arteriovenous aneurysms].

The authors analysed in particular the venous drainage of 370 arteriovenous aneurysms operated during 20 years in neurosurgery at Lille's Hospital. If in many cases the study in details of the different arterious pedicles take an important place in the discussion of the indications and operative technics; in other cases a bad estimate of the interest of careful study of veins can be at the beginning of great disappointments. Also the authors insist on the necessary study of the venous system in totality by seeing after agenesia of some passages, the respective drainage of the angioma and the brain, the existence of a saturation venous phenomenon. The analysis in details of the anatomical and functional distribution of the venous drainage in these malformations can be taken into consideration in the operative decision. The analysis of results in ten preference observations suggest that it is an important element in the arteriovenous aneurisms' evolution and in the final prognostic.
