[Stereotaxic radiosurgery:methods and indications in the treatment of cerebral arteriovenous malformations].

After Leksell and Steiner's pioneering experience with the use of the gamma units, another technically different system has been developed, using a linear accelerator. It's a very precise stereotactic radiosurgical approach which allows to deliver the necessary dose of radiation to the target volume while sparing the surrounding structures from potentially dangerous levels of radiation. This hyperselective irradiation is a new and complementary method of treatment of arteriovenous malformations located in central or functionally critical areas of the brain. The aim is to obtain a progressive and total obliteration of the lesion. The place of radiosurgery is obviously reserved for the patients who do not seem able to profit from the benefits of open surgery and/or embolization techniques.

Discectomies of the lower cervical spine using interbody biopolymer (B.O.P.) implants. Advantages in the treatment of complicated cervical arthrosis. A review of 150 cases.

The authors report their experience with 150 cases of discectomies performed with interbody grafts using a copolymer (Biocompatible Orthopedic Polymer or BOP). Made of N-Vinylpyrrolidone-Methylmethacrylate monomers, polyamide fibres and calcium gluconate. BOP is a biocompatible, biodegradable, osteoconductive matrix, easy to use and quite safe. These properties are quite well appreciated for cervical interbody grafting. After discectomy, stabilization of the spine is immediate, and fusion slowly occurs around and through the biopolymer within one year. The biomaterial avoids morbidity inherent in the harvesting of an autograft, as well as any limitations resulting from the use of allografts.

[Alzheimer's disease. Statistical analysis of CT scanner data].

Brain CT's of 30 patients with early onset probable Alzheimer's disease (according to NINCDS-ADRDA work-group criteria), were compared to those of 30 sex- and age-matched control subjects. This study has confirmed that patients with Alzheimer's disease have higher cerebral atrophy than control subjects, and that cerebral atrophy is correlated with the severity of the disease appreciated with the mini-mental state and the global deterioration scales. It revealed that patients were more different from control subjects for the subjective index and for the third ventricle index. CT has a good predictive value in differentiating groups, but has a poor predictive value in differentiating individuals.

[Surgical treatment of lumbar foraminal disk herniation. Value and indications for an interlaminar and extra-articular combined approach].

From 62 foraminal herniations seen over 2 years, the authors present a series of 22 lumbar foraminal disc herniations operated on by a combined interlaminar and extra-articular approach. After describing the technique the results are analysed. The objective of the combined approach is to provide satisfactory foraminal liberation without damage to the articular structures. They also define the indications. These are based on precise analysis of the topography of the hernia, provided by CAT scan examination. Three major topographical types may thus be defined.

[Percutaneous surgery of the dorsolumbar disks].

The authors report a new technique for percutaneous surgery of thoraco-lumbar discs. It allows nucleotomy or intersomatic grafting. This extra-articular posterolateral approach is performed under fluoroscopic monitoring by using a Kirschner's wire and some specific co-axial tubes. Scanographic polar coordinates are most suitable for valuable locating of thoracic discs and performing of technique. Indications and limits of the method are discussed with preliminary results obtained with the first 19 cases.

[Percutaneous surgery of the lumbar disk].

Percutaneous discectomy is performed with an equipment which has proved reliable in urological surgery, namely dilators and optic fibers. Carried out under local anaesthesia, it empties the intervertebral disc by an extra-spinal route, thus reducing the risk of fibrosis which is almost constant when the interlaminar route is used. Its indications must be discussed with those of chemonucleolysis.

[Indications for surgical treatment in dorsal disk pathology. 34 cases].

Thirty-four patients were operated for dorsal disk pathology by a postero-lateral approach. The first cases have been followed-up for over 4 years and have shown this method to be reliable. When associated with the "harpoon" marker technique, the surgical indications now include not only patients with progressive radiculo-medullary symptoms, but also cases of sudden decompensation and those limited to radicular symptoms.

Herniated cervical discs. Analysis of a series of 230 cases.

Over a period of 18 years the authors have collected together 230 cases of herniation cervical discs. Multifactor analysis of the results of the various therapies is given. Surgical decompression has been done, mostly using an anterior approach, and it effectively treats the radicular damage; it stabilizes and improves spinal cord deficits whether the herniation is spontaneous or traumatic, acute or chronic. Computed tomography facilitates its anatomical identification and the decision regarding the therapy to apply.

Posterolateral approach to tumours of the dorsolumbar spine.

The authors outline the possible approaches to vertebromedullary tumours of the thoraco-lumbar spine using unilateral or bilateral transverso-arthro-pediculectomy and corporectomy. In a personal series of 20 cases. The technique and indications are described and the results.

Thoracic disc herniations: evolution in the approach and indications.

Between 1960 and 1 June 1985, 21 patients were operated for thoracic disc herniations. The advent of new operative techniques (with transverso-arthropediculectomy) has brought about new indications in the management of such disorders. These new indications are important because such protrusions may be abruptly complicate by compression of blood vessels, which may produce abrupt and irreversible neurological deficit. The progress in the radiological examinations allows a more precise study of the topography and density of the hernia. Furthermore, the use of a surgical microscope and microsurgical techniques allow a surgical treatment without preoperative medullary angiography.

[Treatment of soft cervical hernia by percutaneous intradiscal injection of aprotinin].

Chemonucleolysis is widely used in the treatment of disease of the lumbar intervertebral disk. From a series of 34 patients the authors report their experience using this technique at the cervical level using aprotinin instead of chymopapain in cervical soft disk protrusion. They underline the technique, mechanism of aprotinin and indications.

[Postoperative abscesses and empyemas. Apropos of 13 cases].

13 cases of severe intracranial postoperative infections, abscesses and/or empyema, are reported. These lesions, whose clinical diagnosis is often difficult, have benefited from C.T. scan. The diffusion of the infectious process in operatory focus, the frequency of Staphylococcal origin and the importance of high doses antibiotherapy must be underlined. This one is, of course, an indispensable complement to reintervention, but it may be utilized alone with success in selected cases, or insure the definitive cure when reintervention is partial.

[Septo-optic dysplasia disclosed in adults].

A case of septo-optic dysplasia, presenting with focal epileptic fits in a 29 year old man, is described. Gas encephalography and CT scan demonstrated the main features of the malformation: a single ventricle overhanging a narrowed third ventricle, abnormal mass in the basal forebrain corresponding to an embryonic cyst, optic atrophy. Angiography revealed deep venous anomalies, and, in particular, absence or hypoplasia of the sinus recti.
