Chronic polyneuropathy of undetermined cause.

The case histories of 519 patients with peripheral neuropathy on whom sural nerve biopsy had been performed were reviewed. In 67 patients (50 males, 17 females) (13%) who had symptoms of a symmetrical polyneuropathy for more than one year, the cause remained undiagnosed in spite of intensive investigation. Patients with inflammatory neuropathy were not included, but represented 17% of the whole series. The mean age of onset of symptoms was 50.6 years, and the median time from onset of symptoms to initial investigation was 2 years. Males were affected more commonly than females in a ratio of 3:1. The clinical features in 43 patients were those of a mixed motor and sensory neuropathy, in 17 patients a predominantly sensory neuropathy and in 7 patients a predominantly motor neuropathy. The mean CSF protein was 0.73 g/l and in only six patients was it greater than 1 g/l. Nerve conduction studies most commonly demonstrated mild slowing of motor conduction and impairment of sensory conduction. The usual pathological changes on sural nerve biopsy were those of chronic axonal degeneration. Forty seven patients (70%) were re-examined at intervals of time which ranged from 4 months to 12 years after their initial presentation and nerve biopsy (median, 3 years). As a group, they were only mildly disabled, the condition had a very slowly progressive course and there had been little change in their disability. A possible aetiological factor was found in 17 of the 47 patients (36%) and included malignancy, alcoholism, and benign paraproteinaemia. It is concluded that with intensive investigation the cause of chronic polyneuropathy of duration greater than one year remains undetermined in only about 13% of patients and that continued follow-up is worthwhile since a diagnosis may be established on re-examination.

Radiation hazards in scoliosis management.

Safe radiography in scoliosis management is based on a sound knowledge of 1) the radiographic imaging process, 2) the degree of risk to the patient from radiation exposure, and 3) the radiographic requirements to both evaluate and follow patients with spine deformity. This paper is a current review of the subject and work done at the authors' centers. It includes recommendations for reducing the radiation risk while maintaining necessary diagnostic information.

Initial experience with universal pacemakers.

Thirty-five patients were implanted with the Sequicor DDD pacemaker between September 1, 1981 and March 1, 1982. The mean age was 61.7 years (range, 17-88). According to their ECGs, the patients fell into two distinct groups--those with A-V block and those with sick sinus syndrome. Our initial experience has indicated an excellent electronic component function (except for 1 discrepant reed switch); high battery drain of the Sequicor; complications similar to those seen with DVI or VVI pacing; and an increase in the hemodynamic benefits resulting from DDD as compared to DVI or VVI pacing in 70% of patients.

Terminal care: evaluation of effects on surviving family of care before and after bereavement.

To evaluate the effects on the family of a comprehensive programme of terminal cancer care, 20 close relatives of patients who had died in a Palliative Care Unit (PCU) were compared with a matched group of 20 relatives of patients who had died of cancer in other wards of the same teaching hospital. Interviewed by telephone 1 year and 2 weeks after bereavement, relatives of PCU patients report significantly fewer psychological symptoms and less lasting grief and anger than relatives of patients who had died elsewhere. Factors thought to have contributed to good outcomes were successful relief of pain, awareness by relatives of the coming death of the patient and support given to relatives after bereavement. Two case examples illustrate these findings.

The prevalence of thyroid dysfunction in mentally handicapped in-patients.

A survey was made of the thyroid function of 402 mentally handicapped patients in a long-stay hospital and its satellite units. The prevalence of hypothyroidism in a 2-year period was 25/402 (6.2%), and 17/364 (4.7%) when people with Down's syndrome were excluded. Hyperthyroidism was found in 3/402 (0.7%). These results include 16 patients with hypothyroidism, and two with hyperthyroidism, who were newly detected during the surgery. With diagnosis, treatment was started. This occurrence of thyroid disorders is higher than in both the general population in the community and also in other psychiatric in-patients. Biochemical investigation is essential as the disorders are difficult to detect clinically, and untreated the patients are liable to the complications of hypothyroidism and hyperthyroidism.

Evoked potential changes in clinically definite multiple sclerosis: a two year follow up study.

Visual, spinal and somatosensory evoked potentials were performed on 56 patients with clinically definite multiple sclerosis at the beginning and end of a 2 1/2 year follow-up period. At the initial examination one or both visual evoked potentials were abnormal in all but nine patients (84%), five of whom had abnormalities of either spinal or somatosensory evoked responses; that is, one or more abnormal results were obtained from 52 of 56 (91%) patients. At the final examination there were abnormalities of one or more evoked potentials in 55 of the 56 (98%) patients. There was an increase in latency of the components of the evoked responses over the period; reduction in latency in individual patients was exceptional. The change in these electrophysiological measurements correlated with the increase in clinical disability of the group of patients over the period of study.

Severe tubulo-interstitial disease in a renal allograft due to cytomegalovirus infection.

A 34 year old female developed impaired function of her renal allograft 21 months post-transplant. This was associated with lethargy, pyrexia, tenosynovitis, pancytopenia and a colonic ulcer. Severe tubulo-interstitial changes with intranuclear inclusion bodies and intracytoplasmic herpes type viral particles were seen on renal biopsy. There was no evidence of rejection. Cytomegalovirus was cultured from the urine and there was a rise in CMV antibody titer. These findings suggested the renal impairment was due to a direct cytopathic effect of the CMV. Despite treatment with transfer factor and adenine arabinoside, there was progressive loss of graft function.

Innervation of the undifferentiated limb bud in rabbit embryo.

The concept that there are no nerves in the limb bud of mammalian embryos prior to differentiation has been re-examined. Rabbit embryos were collected at 260 and 290 hours gestation, which is prior to cartilage formation in the forelimb at 320 hours. Forelimb buds and adjacent neural tube were excised, fixed and embedded for light and electron microscopy. The limb buds were sectioned in two planes by serial 1 micrometer sections and inspected by light microscopy. Bundles of nerve fibres were seen within the proximal third of the limb bud, with distal ramification into adjacent zones of condensing mesenchyme. Electron microscopy confirmed the presence of axons and associated immature Schwann cells. These results demonstrate the existence of an anatomical framework through which a neurotrophic influence might be brought to bear upon mesenchyme prior to early differentiation.

The effect of intravenous diazepam on rise of intraocular pressure following succinylcholine.

Because succinylcholine raised intraocular pressure, its use to facilitate tracheal intubation for ocular surgery, especially in emergency open-eye cases, has been a controversial topic among anaesthetists for more than two decades. In recent years, intravenous diazepam pretreatment before succinylcholine has been reported to reduce the untoward side effects of myalgia, and elevation of serum potassium and creatine phosphokinase. This study was designed to assess the effect of pretreatment with intravenous diazepam 0.1 mg kg-1 on control (base-line) intraocular pressure and to determine if such pretreatment diminished the rise in intraocular pressure following the standard anaesthesia induction sequence of thiopentone 3 - 5 mg kg-1., followed by tracheal intubation. Such diazepam pretreatment was shown to reduce the intraocular pressure from control levels and to diminish the rise of intraocular pressure following succinylcholine and tracheal intubation. Because succinylcholine produces rapid onset of neuromuscular block for tracheal intubation and since only minor intraocular pressure elevation occurs following thiopentone and succinylcholine in patients pretreated with diazepam, its use in ocular surgery, including emergency open-eye cases, can be rationally advocated. The addition of 0.6 mg kg-1 d-tubocurarine to the diazepam pretreatment did not produce a further reduction of the increase of intraocular pressure following succinylcholine.

House dust mite counts in different types of mattresses, sheepskins and carpets, and a comparison of brushing and vacuuming collection methods.

Mite counts were compared in dust from three types of mattresses. Foam rubber mattresses appear preferable for mite sensitive atopic subjects; the surface of kapok mattresses averaged twice, and innerspring mattresses three times, the number of mites per square metre found on foam rubber mattresses (P less than 0.01). Sheepskins used for infant bedding were also found to be heavily infested, harbouring as many mites per square metre as bedroom carpet. Mite collections are best made with vacuum sampling methods in preference to a brush and results are best expressed in mites/m2.
