RYBP, a new repressor protein that interacts with components of the mammalian Polycomb complex, and with the transcription factor YY1.

The products of the Polycomb group (PcG) of genes are necessary for the maintenance of transcriptional repression of a number of important developmental genes, including the homeotic genes. A two-hybrid screen was used to search for putative new members of the PcG of genes in mammals. We have identified a new Zn finger protein, RYBP, which interacts directly with both Ring1 proteins (Ring1A and Ring1B) and with M33, two mutually interacting sets of proteins of the mammalian Polycomb complex. Ring1 binds RYBP and M33 through the same C-terminal domain, whereas the RYBP-M33 interaction takes place through an M33 domain not involved in Ring1 binding. RYBP also interacts directly with YY1, a transcription factor partially related to the product of the Drosophila pleiohomeotic gene. In addition, we show here that RYBP acts as a transcriptional repressor in transiently transfected cells. Finally, RYBP shows a dynamic expression pattern during embryogenesis which initially overlaps partially that of Ring1A in the central nervous system, and later becomes ubiquitous. Taken together, these data suggest that RYBP may play a relevant role in PcG function in mammals.

Induction of puberty with human chorionic gonadotropin and follicle-stimulating hormone in adolescent males with hypogonadotropic hypogonadism.

OBJECTIVE: To evaluate the clinical and hormonal responses of adolescent males with hypogonadotropic hypogonadism (HH) in response to gonadotropin replacement with the use of long-term combined hCG and FSH therapy. DESIGN: Prospective clinical study. SETTING: Clinical pediatric department providing tertiary care. PATIENT(S): Seven prepubertal males with isolated HH with a mean (+/-SD) age of 15.44+/-1.97 years and seven prepubertal males with panhypopituitarism-associated HH with a mean (+/-SD) age of 18.1+/-3.24 years were studied. INTERVENTION(S): Human chorionic gonadotropin (1,000-1,500 IU IM) and FSH (75-100 IU SC) were administered every alternate day of the week until the total induction of puberty and spermatogenesis was achieved. MAIN OUTCOME MEASURE(S): Serum testosterone levels, testicular volume, penis length, and sperm count were evaluated after the administration of hCG and FSH. RESULT(S): All patients achieved normal sexual maturation and normal or nearly normal adult male levels of testosterone. The increase in testicular size was significant in both groups. Positive sperm production was assessed in four of five patients with isolated HH and in three of three patients with panhypopituitarism-associated HH. CONCLUSION(S): Long-term combined hCG and FSH therapy is effective in inducing puberty, increasing testicular volume, and stimulating spermatogenesis in adolescent males with isolated HH and panhypopituitarism-associated HH.

Hydroxyurea-induced leg ulcers: is macroerythrocytosis a pathogenic factor?

Hydroxyurea is a common cancer chemotherapy agent that inhibits ribonucleotide reductase, an enzyme essential to DNA synthesis. It is considered the drug of choice in the treatment of chronic myelogenous leukemia and essential thrombocythemia. The occurrence of leg ulcers have been described in 8.5% of patients receiving continuous treatment with hydroxyurea, but the cause of this complication is unknown. We report two additional patients and suggest that macroerythrocytosis, which occurs in almost all the patients taking hydroxyurea, may be a pathogenic factor. Macroerythrocytosis can be considered as an 'acquired' blood dyscrasia, and similar leg ulcers have long been known to occur with certain hereditary blood dyscrasias, such as sickle cell anemia, thalasemia, and spherocytosis.

The dermatologic pathological condition of musicians is a rare medical problem. We would like to draw attention to what is called "Fiddler's neck," a process that is peculiar to violin, viola, or cello players and that may be caused by two different mechanisms: contact allergic reaction or a mechanical action.

Cryosurgical treatment of professional chronic radiodermatitis.

BACKGROUND: Chronic x-ray dermatitis in professionals is a frequent problem for doctors in our country due to the fact that many of them widely used radiotherapy without any protection 15-20 years ago. Surgery has been the most accepted treatment, though it generally decreases hand function. OBJECTIVE: Up to now, cryosurgery was not usually considered as a possible treatment if the lesions were located on fingers. In this study, the advantages of cryosurgery for the treatment of professional chronic radiodermatitis with incipient pretumoral lesions are emphasized. METHODS: Cryosurgery was performed on six patients affected with chronic professional radiodermatitis that showed keratomas and ulcerations, using both spray (keratomas) and a probe 0.5 cm in diameter (ulcerations, in situ squamous cell carcinoma). Nerve block anesthesia with mepivacaine 1% was used in all cases. Before the treatment, all suspected lesions were biopsied; if invasive squamous cell carcinoma was revealed in the dermatopathological study, the patient was rejected. Variables such as blister and necrosis formation, pain, and achromatic, sensibility, and mobility disorders were studied. The follow-up period was 2 years. RESULTS: Immediate postoperative results showed great pain and blistering in all cases. Residual achromias were observed early postoperatively in all cases, but were repigmented 1 year after therapy in four cases (66%). Sensory alterations (hypo- and hyperthesias) were found in four cases (66%) 1 month after treatment, although this complication was not observed 6 months after treatment. Finger mobility was perfect in all cases 2 months after treatment, and there was no recurrence in any case after 2 years of follow-up. CONCLUSIONS: We believe cryosurgery must be considered as an excellent treatment for professional chronic radiodermatitis with keratomas, ulcerations, and incipient squamous cell carcinomas. Its use may prevent further dramatic surgical treatment, like amputations, allowing the preservation of finger function.

Mesenteric arteriovenous differences in glucose, lactate and insulin concentrations in rats and humans.

The present study was aimed to investigate the mesenteric arteriovenous differences in blood glucose and lactate and plasma insulin in humans (n=8) and rats (n=10). Arterial (abdominal aorta) and mesenteric vein blood glucose and lactate (enzymatic methods) and plasma insulin concentrations (radioimmunoassay) were measured in patients during abdominal surgery and in normal rats. Blood glucose levels were significantly (p < 0.05) higher in the abdominal aorta than in the mesenteric vein in both rats (9.2+/-1.0 vs 7.5+/-0.8 mmol/l) and humans (10.4+/-2.9 vs 8.5+/-2.7 mmol/l). Blood lactate levels were higher (p < 0.05) in the mesenteric vein in both rats (3.7+/-1.3 vs 2.8+/-0.9 mmol/l) and humans (0.7+/-0.23 vs 0.1+/-0.05 mmol/l). Plasma insulin concentrations were identical in the aorta or mesenteric vein in both rats (314.4+/-162.0 vs 311.4+/-94.2 pmol/l) and humans (62.4+/-43.2 vs 61.8+/-48.0 pmol/l). In conclusion, both rat and human intestine retained a high proportion of arterially administered glucose and released lactate under the studied conditions.

Cellular blue nevus with massive regional lymph node metastases.

BACKGROUND: Small, well-differentiated groups of nevus cells have been found occasionally in the marginal sinuses and parenchyma of regional lymph nodes that drain sites of cellular blue nevi. OBJECTIVE: The histologic, immunohistochemical, and karyometric description of a pigmented cutaneous lesion, with the features of cellular blue nevus and located on the leg of a 14-year-old woman, that was accompanied by synchronic presentation of massive inguinal lymph-node metastases. METHODS: The excised specimens were processed routinely, embedded in paraffin, and sectioned into 4-microm-thick slices. The sections were stained using hematoxylin-eosin and the ABC immunohistochemical method for demonstrating S-100 and HMB-45. Karyometric analysis was performed in a static cytometer using Feulgen-stained sections. RESULTS: The cutaneous lesion had the cytologic and architectural features of cellular blue nevus. The lymph nodes showed massive invasion by pigmented cells and contained extensive necrotic foci. After 3.5 years of clinical follow-up, the patient is free from disease. CONCLUSIONS: The absence of malignant features in the cutaneous lesion and the bland nuclear features of the pigmented cells in the regional lymph node metastases suggest that this case could be interpreted as an unusual form of benign cellular blue nevus with metastases. Nonetheless, other possibilities, such as malignant melanoma mimicking a cellular blue nevus or primary malignant melanoma of the lymph nodes with concomitant cutaneous cellular blue nevus, cannot be definitively excluded. A conservative surgical approach with close follow-up was recommended.

Experimental streptozotocin-reduced diabetes and intestinal glucose metabolism in the rat, in vivo and in vitro.

The intestine has a high glycolytic activity, but its metabolic role could be altered in diabetes mellitus. The aim of the present work was to investigate in vivo the glucose retained and the lactate produced by the intestine of normal and diabetic rats and in vitro the effect of different arterial glucose concentrations on glucose utilization and lactate, alanine, and pyruvate production in normal and diabetic rats when the glucose is supplied to the intestine exclusively via the vascular route. In vivo, the normal and diabetic rats retained similar percentages of the arterially supplied glucose (14.7 +/- 2.4, respectively). In vitro, when the preparations were perfused under hyperglycemic conditions, the glucose consumed, as a fraction of the quantity infused, was significantly lower (P < 0.05) in the diabetic (247.0 +/- 22.8 mumol/mmol infused glucose) than in normal (315 +/- 16.3 mumol/mmol infused glucose rats) rats. The lactate produced was significantly higher in diabetic than in normal rats whether the preparations were perfused under isoglycemic (P < 0.01; 1916.4 +/- 124.0 vs vs 1284 +/- 67.7 mumol/mmol consumed glucose) or hyperglycemic (P < 0.05; 1356.4 +/- 199.7 vs 898.0 +/- 87.3 mumol/mmol consumed glucose) conditions. There was significantly (P < 0.05) greater alanine release from the diabetic (123.7 +/- 21.8 mumol/mmol consumed glucose) than from the normal (40.7 +/- 10.3 mumol/mmol consumed glucose) rat preparations perfused under isoglycemic conditions.

Total upper lip reconstruction with bilateral fan flaps.

Reconstruction of the total upper lip may be easily performed using fan flaps. However, two details must be taken into consideration: 1) the wide pedicle in the commissural angle, including the facial artery, must be designed, and 2) the mucosal flap must be 8 mm longer, in the inner portions, than the skin flap used to reconstruct the lip semimucosa.

[Tinea faciei: epidemiology].

Because of its clinical peculiarities responsible for many diagnostic errors resulting in potentially toxic treatments, tinea faciei should be regarded as a process with a personality of its own. The authors have reviewed 46 cases of tinea faciei, evaluating and highlighting their clinical and epidemiological peculiarities. As regards age and sex, two peaks of maximum incidence can be found: one in childhood, the other in adults older than forty. Many lesions progress over more than 12 months before they are diagnosed. Their peculiar clinical features suggests various initial diagnoses, the most frequent of which are related to photosensitivity (lupus erythematosus, polymorphous light eruption). Among the most frequently isolated dermatophytes, T. mentagrophytes var. granulosa and T. rubrum are outstanding.

International dermatosurgery: reconstructive treatment of the vulva--a new procedure: experience on 23 cases over 4 years.

The postvulvectomy reconstruction caused the authors diverse problems in the first three operations. The use of a bipedicle advancement flap obtained from the anterointernal region of the thigh, modified depending on the extension of the vulvectomy and the elasticity of the skin allow us to suggest the use of this technique due to its safeness, the facility with which it is carried out, and reduction of the hospital stay, as this makes the method socially economical.
