This study compares patient compliance with once- and twice-daily formulations of 5-isosorbide mononitrate. A total of 31 patients (20 men and 11 women) with stable angina pectoris were randomized to receive either 60 mg 5-isosorbide mononitrate in a controlled release formulation once daily, or 20 mg 5-isosorbide mononitrate twice daily. The results indicated that compliance assessed using the electronic Medication Event Monitoring System (MEMS) was better with the once-daily than with the twice-daily formulation; patients on the once-daily regimen performed better with respect to the total number of bottle openings, the number of openings per day, the timing of openings and the intervals between openings. The apparently superior compliance with the once-daily regimen appeared to be reflected in better efficacy; patients on the once-daily regimen experienced fewer angina attacks (a mean of 1.7 per 7 days, compared with 3.3 per 7 days for patients on the twice-daily regimen) and used fewer nitroglycerin tablets than those on the twice-daily regimen.

Research into the environmental factors in the aetiological diagnosis of asthma is a fundamental measure which must direct complementary test and therapy. Extrinsic asthma is mostly found in young people, with atopic antecedents. Immunological study is based on the clinical history and skin tests (IgE dosage, in vitro tests and provocation tests are difficult to apply in epidemiology due to their high cost). In children 3/4 of asthma cases are extrinsic. Differential diagnosis is especially a problem with rhinitis. There is only a small percentage of cases of non atopic extrinsic asthma which can bring other immunological mechanisms into play. So-called intrinsic asthma (late onset asthma, with continuous dyspnea, corticodependent) is mostly observed in adults. The age has an influence on the severity of the attacks, the curve of asthma with permanent dyspnea being a function increasing with age, whereas that of less severe asthma is a function decreasing with age. The role of extrinsic factors does not seem important in this category of asthmatics (10 to 25% according to the statistics). Immunofluorescence studies of the bronchial mucosa show up the possibility of local immunological reactions, whether specific or not (frequent presence of the C3 complement). This means that the immunologist can, in many cases of asthma, help the epidemiologist to define and to understand the physiological mechanisms of the disease.

Melatonin is secreted by the pineal gland with a circadian and a circannual rhythm. For some authors the circadian rhythm is modified during depressive states. The present study evaluates the melatonin circannual rhythm in five manic-depressive patients. Each patient is clinically and biologically evaluated every month during one year. The results of the present study show a lack of clinical relapse and a lack of seasonal variation of plasma melatonin level.
Many research studies have been conducted with melatonin, a pineal hormone, in psychiatry. Studies with melatonin in schizophrenic patients are rare. Blood plasma samples were drawn at 24 hours and 12 hours in 23 schizophrenic subjects and 26 controls, during the same season. Plasma melatonin levels were obtained using a specific radioimmunoassay technique. The 24 hours levels significant decreased in schizophrenic patients compared with controls (p less than 0.01). Methodology, specificity of results and concomitant study of cortisol levels are discussed.
