Studies with crystalline insulin from obese and lean mice of the BL/6J strain.

Crystalline insulin was extracted and purified from the pancreases of obese (BL/6J/-ob/ob) and lean mice (BL/6J and BL/6J-ob/+). The two insulin preparations were compared with respect to their radioimmunologic properties as well as their ability to stimulate glucose metabolism in rat epididymal adipocytes and epididymal adipose tissue from obese and lean mice. No significant differences could be seen between the two insulin preparations and thus an insulin of altered biological properties is not likely to be an adequate explanation for the symptoms observed in the obese mouse.

Secretory B-cell activity in insulin dependent maturity-onset diabetic.

In 10 insulin dependent maturity onset diabetics we found elevated basal C-Peptide levels (4.78 +/- 0.5 ng/ml. Normal range 1.1--3.6 ng/ml), which could be suppressed by insulin injection to the same extent as in sulfonylurea treated diabetics could be demonstrated. C-Peptide immunoreactivity in these patients therefore seems to be newly secreted rather than accumulated material. Since adrenalectomized patients could be suppressed in the same way, it is likely, that catecholamines are not the major factor in the mechanism of suppression. Therefore only decrease of bloodsugar levels seems to be accountable for the decrease of C-Petide levels. High C-Petide levels in insulin dependent maturity onset diabetics which cannot be stimulated but suppressed may be explained by a loss of glucoreceptor molecules.

[Are Hegar's rods omissible? (author's transl)].

The widening effect of Sulproston 500 microgram IM on the cervix uteri was investigated in a double-blind study conducted in 50 non-pregnant volunteers. In women in the postmenopausal stage who had been given Sulproston, resistance-free widening of the cervix with Hegar's rods was H 8.33 +/- 0.87. Easy passage of the cervix uteri and of the internal os uteri was achieved with Hegar 8.22 +/- 0.66 in the first half of the cycle and with Hegar 8.35 +/- 3.06 in the second half of the cycle. In all three groups, the size of the rods was significantly smaller following intramuscular application of physiological saline solution. The widening effect of PGE2 or PGE2 derivatives on the cervix, which has been known for quite some time in patients with pregnant uterus, applies in equal measure to the non-pregnant cyclic uterus and to the postmenopausal uterus.

[Plasma-catecholamines under electrostimulation and neurolept anaesthesia for retina and vitreous body operations (author's transl)].

We determined in two groups of patients with normal circulation and metabolism during operations on the retina and vitreous body the concentration of plasma catecholamines. The intensity of stress was compared between electrostimulation anaesthesia and neuroleptanaesthesia. Venous blood was sampled 1) before premedication, 2) 30' after premedication, 3) at the start of surgery, 4) 45' after the start of surgery, 5) 30' after extubation. Plasma catecholamines (adrenaline and noradrenaline) were determined using a modified trihydroxyindol-spectrofluoremetric method (combined "Batch-technique"). There was no relevant difference between the two techniques of anaesthesia when using the concentrations of plasmacatecholamines as an indicator of stress resulting from operation and anaesthesia.

The artificial beta cell (Biostator) in the adjustment of instable diabetics--results after 20 months.

In 55 poorly controlled insulin-dependent diabetics, we tried to discover criteria for an improvement of metabolism by means of the "artificial beta-cell" (Biostator). To this end, during the first 24 h of hospitalization, blood glucose was monitored continuously under conventional insulin therapy (monitoring period). Insulin requirement was determined during the next 24 h by the artificial beta-cell (feedback period). Corrections of diabetes regimen were made with reference to the insulin consumption during the feedback period and to the extent of the postprandial blood sugar increases and decreases during the monitoring period. The resulting new diabetes regimen led to a significant improvement of the daily blood sugar profiles.

[Gonadotropin secretion following pernasal stimulation with synthetic gonadotropin releasing hormone (GnRH) and a highly effective GnRH-analog in healthy men and prepubertal boys].

Potent long acting analogs of GnRH are of great interest especially in view of pernasal (p.n.) treatmen of hypogonadism of hypothalamic origin and of cryptorchidism. To find the necessary p.n. dosage of such a substance, serum LH and FSH were measured in 6 normal adult human males after p.n. application of various doses of D Leu6-des-Gly10-GnRH ethylamide. 50 microgram of the GnRH analog were necessary to obtain increased serum gonadotropins over a period of at least 8 hours. By repeated p.n. application of 200 microgram of synthetic GnRH every 2 hours in 6 normal adult males a considerable increase of serum gonadotropins could be demonstrated as well. Pernasal application of 200 microgram GnRH repeated at an interval of 1 hour in 3 cryptorchid boys produced a distinct increase of the serum gonadotropins. The intraindividual comparison of 200 microgram GnRH and 20 microgram of the GnRH analog in one boy showed equivalent net increases of the gonadotropins. With the analog the gonadotropin increase lasted for about 6 hours.

[Saralasin-induced changes of blood pressure, renin and aldosterone in essential and renal hypertension (author's transl)].

In 34 patients saralasin was infused after variable degrees of sodium depletion in order to differentiate between essential and renin-induced hypertension. After sodium-depletion of short duration mean arterial pressure dropped more than 10 mm Hg in 9 of 25 patients with essential and in 7 of 9 patients with renin-induced hypertension. After long-lasting sodium depletion the fall of mean arterial pressure exceeded 10 mm Hg in 11 of 16 patients with essential and in 8 of 9 patients with renin-induced hypertension. Thus saralasin did not discriminate essential and renin-induced hypertension. Also, plasma renin concentration before and after saralasin did not allow to differentiate between the two forms of hypertension. The changes of renin during infusion of saralasin was negatively correlated to the change of blood pressure. Renal vein renin ratio in patients with renovascular hypertension was not modified by saralasin. Renin and aldosterone changed inversely during saralasin infusion.

[Gonadotropin secretion after stimulation with a highly effective analog of the gonadotropin-releasing-hormone (GnRH) in normal adult human males (author's transl)].

Potent long acting anologs of GnRH are of great interest especially in view of treatment of hypothalamic hypogonadotropic hypogonadism and of cryptorchidism. To find necessary parenteral doses of such a substance immunoreactive serum LH and FSH were measured in 4 normal adult human males after s.c. injection of various doses of D-Leu6-des-Gly10-GnRH-EA. 5 microgram of the GnRH anlog were necessary to obtain increased serum gonadotropins over a period of at least 8 h. Comparable maximal levels were seen after i.v. bolus injection of 25 microgram natural GnRH a dose as used in diagnostic routine tests in 3 probands. Comparing the areas under the curves of LH secretion an over 15 times greater effectiveness of the analog was calculated.

[Peri-operative care of diabetics (author's transl)].

The generally accepted principles of treatment of a diabetic as a case for operation or emergency are listed and substantiated with reference to the varied pathophysiological processes. In particular the preoperative attitude of the diabetic and the special anesthesiological problems are gone into.

Atrial septal defect: acute left heart failure after surgical closure.

Sixteen of 766 patients operated on for isolated arterial septal defect developed severe acute left heart failure that necessitated partial reopening of the defect; 6 died. The prognosis depended mainly on prompt surgical intervention. The most frequent pathological and anatomical finding was marked hypoplasia of the left ventricle. Pulmonary hypertension appeared to be of minor importance. Hemodynamic data published by others support the hypothesis that this complication is often the result of markedly impaired left ventricular compliance associated with left ventricular hypoplasia.

Treatment of cryptorchidism with pernasal gonadotropin-releasing hormone therapy.

Twenty-five boys between 1 and 10 years of age with unilateral or bilateral cryptorchidism were treated with 200 microgram of gonadotropin-releasing hormone (GnRH) pernasally six times daily until descensus was completed, or for 10 weeks at most. Complete descent of the tests occurred in 16 patients, usually after 2 to 5 weeks of treatment. No adverse side effects have been observed. Radioimmunologic measurements of serum luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone, and prolactin, carried out before treatment, at the end of treatment, and 6 months after treatment, showed a transitory increase of the LH responsiveness to GnRH in only four of the patients and in the group as a whole a slight but significant decrease of FSH responsiveness. There were no signs of precocious puberty. GnRH antibodies were not found.

Treatment of cryptorchidism with a potent analog of gonadotropin-releasing hormone.

Pernasal therapy of cryptorchidism with D-Leu6-des-Gly10-gonadotropin-releasing hormone ethylamide (D-Leu6-des-Gly10-GnRH-EA), a potent, long-acting GnRH analog, was attempted. Eleven prepubertal cryptorchid boys received between 25 microgram once daily and 25 to 50 microgram twice daily for 5 to 12 weeks. Complete testicular descent was achieved in 4 of the 11 boys. GnRH tests (1.5 microgram/kg intravenously), conducted in six boys before treatment, after 4 weeks of treatment, and in 2 boys 3 months after treatment, did not reveal changes in gonadotropin secretion indicative of precocious puberty or of decreased hypophyseal sensitivity to GnRH. Antibodies to the GnRH analog or to GnRH could not be detected.

Gonadotropin and testosterone secretion in normal human males after stimulation with gonadotropin-releasing hormone (GnRH) or potent GnRH analogs using different modes of application.

Gonadotropin-releasing hormone (GnRH) and some potent long-acting GnRH analogs, applied by different routes of administration, were tested in six healthy human males. The effects on gonadotropin secretion were compared with the one after intravenous (i.v.) bolus injection of 25 microgram of GnRH. The net increase of luteinizing hormone (deltaLH) in serum produced by 25 microgram of GnRH i.v. was matched by subcutaneous (s.c.) injection of 100 microgram of GnRH, dissolved in 20% gelatin or without gelatin; 5 microgram of D-Ser (TBU)6-des-Gly10-GnRH-ethylamide i.v.; 5 microgram of D-Leu6-des-Gly10-GnRH-ethylamide i.v.; and 50 microgram of D-Trp6-des Gly10-GnRH-ethylamide given pernasally (p.n.). D-Leu6-des-Gly10-GnRH-ethylamide, 50 microgram p.n., produced one-half such increase as did also multiple p.n. administrations of 200 microgram of GnRH every 2 hours. With 100 microgram of GnRH and all analogs, elevation of serum LH lasted for about 7 to 9 hours. The longest elevation was observed with GnRH dissolved in gelatin and with D-Ser (TBU)6-des-Gly10-GnRH-ethylamide, as reflected by the greatest areas under the curves of net increase. The longer the duration of the action of LH secretion, the higher was the observed increase in follicle-stimulating hormone (FSH). Correlation between effect on LH secretion and testosterone secretion was not found. By infusion of 1 microgram/kg/hour od D-Leu6-des-Gly10-GnRH-Ethylamide in two men over a period of 15 hours, a plateau of gonadotropin levels was reached within 7 to 9 hours. These plateau levels, especially of FSH, were higher than after bolus injection or p.n. application.

[Early and late results after surgical treatment for pulmonic stenosis with intact ventricular septum (author's transl)].

From 1958 to 1975, 309 pts. were operated on for pulmonic stenosis (PSt) with intact ventricular septum. The lesion was of the isolated valvular type in 81,5%, of the infundibular type in 9,1%, and of the combined type in 9,4%. A closed transventricular valvulotomy (Brock) was performed in 33 cases, primarily in infants. 130 pts. underwent transarterial valvulotomy in inflow occlusion, and 146 pts. were operated with the aid of extracorporeal circulation. Hospital mortality was 3,9%. All three operating methods showed almost identical early results, whereas the long-term results indicated a superiority of the open techniques. The results were less satisfying in pts. more than 20 yrs. of age than in younger ones. In 10 cases (3,4%) follow-up revealed recurrent PSt requiring re-operation, mainly because of persisting infundibular hypertrophy and markedly thickened (dysplastic) valve cusps.--Indications for the different surgical techniques are discussed on the base of the results.
