[Hodgkin's disease in children: long-term therapeutic results (author's transl)].

Between 1965 and 1976, 83 previously untreated children aged 15 years and under, with biopsy-confirmed Hodgkin's disease were assessed, treated, and followed-up at Hoôpital Saint-Louis, Paris. Clinical stages were IA-IIA for 59, IB-IIB for 19, and III-IV for 5 patients. Two main types of treatment were used: - monochemotherapy-radiotherapy in 26 patients who received mantle field irradiation followed by monthly vinblastine injections for 3 years; - 57 patients received a combination of MOPP and radiotherapy. The MOPP-radiotherapy treated patients have a significantly better survival and relapse-free survival than the monochemotherapy-radiotherapy treated patients (86.9 p. cent vs 76.1 p. cent and 83.5 p. cent vs 65.4 p. cent). Thirteen relapses were observed after a median complete remission period of 30 months: 6 patients are now free from disease and one is still under treatment. Ten patients died after a 55 months median survival: 7 died from Hodgkin's Disease and 3 from treatment toxicity. No secondary cancer or leukemia has been observed. The main long-term complications of therapy were sterility in male patients, after chemotherapy, and growth defects and disturbances of thyroid functions after radiotherapy.

Bone marrow transplantation in 65 patients with severe aplastic anemia.

This articles summarizes the experience of the Hospital Saint-Louis Bone Marrow Transplantation Team of bone marrow transplantation in severe aplastic anemia. Sixty-five consecutive patients have been transplanted with an HLA identical sibling marrow. Various conditioning regimen have been used. Conditioning regimen using high dose Cyclophosphamide alone or associated with Procarbazine and anti-thymocyte globulin gave a high number of bone marrow graft rejection. Therefore, a conditioning regimen using Cyclophosphamide and total body irradiation with lung shielding has been used for the last three years. This regimen suppressed bone marrow graft rejection. The main problems remain graft versus host disease and intercurrent infections. Despite these complications, 50 per cent of the patients become long term survivors and are apparently cured of their initial disease.

[A new clinical entity: "the algodystrophic intermittent claudication of the lower limbs syndrome" (author's transl)].

A new clinical entity is proposed by the authors, "the algodystrophic intermittent claudication of the lower limbs syndrome", characterised by diffuse pains in the foot, having neither a constrictive nature, nor a radicular distribution. The pain appears after walking a certain distance, forcing the patient to stop, and reappears when he starts to walk again. Bone isotopic examinations and repeated radiographic investigations should be conducted when confronted with such a clinical syndrome in order to confirm the diagnosis of algodystrophy. In view of the polymorphic nature of this affection, it is not surprising that new clinical aspects can still be described at the present time.

[Multiple chemotherapy (MOPP) followed by either focal or selective radiotherapy in clinical stages IA and II2A of Hodgkin's disease: results after four years of the use of prospective schedule (H 7701) in 79 patients (author's transl)].

Seventy-nine patients with clinical stages IA, or II2A of Hodgkin's disease were treated from January 1977 to April 1980 by a multiple therapy schedule, H 7701. Three courses of MOPP chemotherapy were first given to all patients. They were then randomly allocated to two groups: group 7701 S (38 patients) was treated by mantle irradiation, excluding the mediastinum when this was not initially involved, or inverted Y radiotherapy; group 7701 F (41 patients) was treated by focal irradiation only. After follow-up for 9-48 months (median: 26 months), overall survival was 98,6 p. cent and relapse-free duration 94,9 p cent. No statistical difference exists between the two randomized groups. Four patients relapsed; three are now free of disease after further treatment, while one patient has since died. With this chemotherapy-radiotherapy selective of focal sequence, staging laparotomy is not indicated. Results and side effects of this treatment schedule are compared with those of other treatment strategies.

[Acute monoblastic leukaemia. Clinical data and therapeutic results in 74 patients (author's transl)].

Seventy-four patients with acute pure monoblastic leukaemia treated between 1970 and 1978 were studied retrospectively. The disease was usually hyperleucocytic and tumoral in character. It occurred with equal frequency in subjects of both sexes and at all ages, with peaks at the two extremes of life. Rubidazone gave a high percentage (75%) of complete remissions, but the prognosis remained sombre, with a mean survival time of 200 days. The incidence of meningeal relapses was reduced by prophylactic measures at central nervous system level, but gingival and cutaneous relapses were frequent. The possibility of bettering the present modest therapeutic results by more intensive chemotherapy is discussed.

Chemotherapy--radiotherapy association in Hodgkin's disease, clinical stages IA, II2A: results of a prospective clinical trial with 166 patients.

One hundred sixty-six patients with clinical stages IA, II2A Hodgkin's disease were treated between April 1972 and December 1976 with three courses of multiagent chemotherapy (methylchlorethamine, vincristine, procarbazine, prednisone) followed by mantle irradiation--excluding mediastinum for those with initial upper cervical presentation and absence of mediastinal involvement--or inverted Y radiotherapy. None had staging laparotomy. With a follow-up of 12--84 months, median 40 months, the overall survival is 93.5% and the overall relapse-free survival 89.9%. Eight patients died, three of them with evident disease. Ten patients relapsed; four are now free of disease after retreatment. With chemotherapy-radiotherapy sequence, staging laparotomy is not indicated. Results and side effects of this treatment strategy are compared with those of other treatment policies.

High risk acute lymphocytic leukemia: a study of 141 cases with initial white blood cell counts over 100,000/cu mm.

The cases of one hundred and forty-one patients (85 males, 56 females) treated for hyperleucocytic acute lymphocytic leukemia (H-ALL) were reviewed. In all cases the initial white blood cell count was over 100,000/cu mm. One hundred patients (71%) attained complete remission (CR). The median duration of CR was six months and the median survival was nine months for all patients and 11 months for those who attained CR. Age, initial hemoglobin, and the height of initial white blood cell count over 100,000 had no significant prognostic value. Relapses occurred earlier in patients with a mediastinal mass. The results depended on the treatment used. With modern treatment, including more intensive chemotherapy and central nervous system prophylaxis, CR rate increased from 65% to 81% and median duration of CR improved from four months to ten months. The most important prognostic difference was related to the sex: CR rate was higher (78.5% vs. 66%) and median duration of CR and hematological remission was longer for females (nine months vs. six months and ten months vs. 6.5 months, respectively). This difference only appeared with modern treatments, however: before 1972 the median duration of CR was four months for both sexes, and after 1972, it was eight months for males and 17 months for females. This difference could be explained by the site of the first relapse, which was testicular in only 2% of cases before 1972 and 27% (47% of the males who relapsed) after 1972.

[Myeloma together with acute myeloblastic leukaemia in an untreated patient (author's transl)].

In a 66-year old man without previous major disease the unexpected finding of pancytopenia on routine blood count during an acute respiratory infection led to the diagnosis of IgG-producing myeloma without radiological bone lesions, associated with partial blastocytosis and abnormalities in granulocyte maturation. No treatment was given. Within 15 days, the circulating blastocytes became more numerous and type M1 acute myeloblastic leukaemia was diagnosed. A combined rubidazone and cytosine arabinoside treatment failed, and the patient died rapidly.

Experimental and clinical activity of a new anthracycline derivative: detorubicin (14-diethoxyacetoxydaunorubicin).

This report concerns the experimental activity and the preliminary clinical results obtained with detorubicin, an anthracycline. Experimentally, in comparison with doxorubicin, detorubicin is less toxic, less immunodepressive, and less mutagenic, its experimental antitumor activity is equal or superior, and differences are found in pharmacokinetics. Its most interesting activities in clinical cancer therapy are found in non-Hodgkin lymphomas, in carcinomas, and in soft-tissue sarcomas.

The effects of pig follicular fluid upon the ultrastructure of rat granulosa cells in tissue culture.

The ultrastructure of rat granulosa cells from preovulatory follicles has been investigated after incubation in control fetal calf serum (FCS) medium with or without LH (1 micron/ml), and in total or steroid-free pig follicular fluid (FF) medium. While the cells had a similar ultrastructure after 1 day of incubation in these media, significant differences were detectable after 3 days. In FCS supplemented with LH, autolysis and phagocytotic activity remained elevated, and cells did not develop any of the typical ultrastructural features of steroid-secreting cells. In contrast, total FF induced complete differentiation of mitochondrial cristae from the lamelliform to the villiform type. Steroid-free FF induced the same array of effects as total FF, except that mitochondrial differentiation was not complete. These observations together with those of previous studies, in which progesterone secretion was measured in cells incubated in the same media, suggested that the development of the capacity to secrete progesterone in rat luteal cells could be directly related only to mitochondrial differentiation.
