[Antibiotic preparation of the colon or preoperative parenteral prophylaxis in colon surgery. A randomized study].

A double-blind randomised trial was carried out among 72 patients undergoing elective colorectal surgery. 37 patients received Metronidazole and Kanamycin systemically, 35 patients the same medicaments orally. We could not find any difference between the two ways of application. These results are in opposition to those of a comparable study of Birmingham, where the therapy was significantly less successful in the oral group. This fact is discussed and the parenteral, short-term prophylaxis is recommended.

Analysis of clinical laboratory data by biplot methods using a three-dimensional display: discrimination of renal stone-patients with idiopathic hypercalciuria and primary hyperparathyroidism.

The biplot technique is a very useful graphical method to display the relationships between row and column characteristics in two-way tables. This method is applicable as long as the rank-2 approximation explains a large part (e.g. 95%) of the whole variability. However, since in large tables only a rank-3 approximation will yield such a high degree of explanation, a three-dimensional biplot technique has been introduced, using a 3d-screen as a matter of presentation of clinical laboratory data. The value of such a procedure is illustrated using as a clinical example a patient population with recurrent renal stone formation due either to primary hyperparathyroidism or to idiopathic hypercalciuria.

Alterations in the enzyme profile in intensive care patients undergoing total parenteral nutrition.

Total parenteral nutrition (TPN) has been demonstrated to be an effective therapeutic means in improving the clinical course of the critically ill patients. Various metabolic complications are described; the cause of some of these remain unclear. The changes in some plasma enzyme indices (GOT, GPT, GIDH, LDH, HBDH, CPK, ChE, AP, gamma-GT) in two groups of critically ill patients undergoing TPN (group with more marked enzyme alterations and group with less marked alteration) were examined. Two types of alterations were found: (1) early increase of some enzymes (GOT, GPT, GIDH); (2) constant increase of plasma enzyme level during TPN (AP, gamma GT). These two evolutionary patterns were more evident in the complicated group and the enzyme changes were statistically significant for GOT and GPT (P = 0.05) and not significant for initial values of G1DH, ap and gamma-GT. Both groups presented constant elevated plasma values of LDH, HBDH, CPK and depressed constant ChE value during treatment; the difference was not significant in both groups for the same enzymes. The data were interpreted from a functional point of view; that is they were related to both the metabolic post-aggressive state and TPN. A relationship between the rate of protein catabolism and the inductive increase of some enzymes (GOT, GPT, G1DH) was found. Whereas a final induction in the energy metabolism is suggested for other enzymes (LDH, HBDH), the alteration of CPK, AP, gamma-GT and ChE was interpreted as dependent on: (1) direct muscular trauma (CPK); (2) functional increase in relation to the duration of TPN (AP and gamma-GT); (3) possible depressed malnutritive synthesis (ChE). The improvement of the enzymatic patterns with the early use of TPN and with the improvement of clinical and nutritional conditions was emphasized.

Murocmycotic slough of nasal floor and palate in the anephric patient.

A case of nasopalatine mucormycosis in a patient with chronic renal failure is reported. Early recognition is contingent on histopathological examination of tissue and culturing. Vigorous treatment with amphotericin B and surgical intervention averted possible orbital-cranial involvement and effected a cure. When a chronically debilitated patient has signs of intraoral necrosis or facial swelling and necrosis, or both, the clinician should be alert to the possibility of a mucormycotic infection.

[Hemolysis in ger test -- an alternative to hamagglutination inhibition test for search of antibodies against rubella virus (author's transl)].

640 sera of unknown immune status were tested in the rubella hemagglutination inhibition test (HI) as well as in hemolysis in gel test (HIG). The results have been analysed by statistical methods. The reproducibility of zones of hemolysis are 1.7 mm for the long term precision and 1.0 mm for the short term precision. Comparing the HIG and the HI tests we had a correlation of 97% = sensitivity (544 out of 561 sera) in positive reacting sera and of 87% = specifity (69 out of 79 sera) in the negative reactions. We consider a person to have antibodies against rubella if the zone of hemolysis had a diameter of 8 mm or above. In cases of diameters under 8 mm the results of IH and HIG-tests do not correlate. The reasons are not completely known and should be investigated.

Small angle x-ray scattering studies on adenovirus type 2 hexon.

Adenovirus type 2 hexons have been studied in solution by small angle X-ray scattering, and the following molecular parameters determined: radius of gyration (Rg) = 4.9 nm, molecular weight (M) = 310.000, invariant volume (Vinv) = 630 mn3, maximal distance (Dmax) = 14.5--15.5 nm. A diffraction pattern was obtained up to an angular increment of h = 2.5 nm-1. Various models for the hexon have been explored by calculating the diffraction pattern from the Debye formula for 1200 spheres arranged to define the scattering volume of each model. Models were first built according to electron micrographic results. Later, preliminary results of a crystallographic study were used for model building. The experimental pattern and the pattern resulting from the model determined by crystallographic methods were compared and showed good agreement.

Effects of atropine and methacholine on deposition and clearance of inhaled particles in the donkey.

The influence of sympathetic and parasympathetic controls on regional particle deposition and mucociliary clearance rates was studied in the donkey in vivo. The deposition and clearance characteristics for gamma-tagged monodisperse ferric oxide microspheres were determined for inhalation tests after atropine and methacholine injections and were compared with the characteristics determined for control tests with the same animals and aerosols. Additional tests were performed in which methacholine was injected immediately after the test aerosol inhalation and 2 1/2 h before the particle inhalation. Particle deposition is shifted distally by atropine. Methacholine produces a proximal shift initially and a distal shift 2 1/2 h later. Atropine slows mucociliary transport. Methacholine produces an acceleration in bronchial clearance initially but causes a reduced rate of clearance 2 1/2 h later. Atropine slows mucociliary transport. Methacholine produces an acceleration in bronchial clearance initially but causes a reduced rate of clearance 2 1/2 h later. It also produces an initial surge in tracheal transport, which frequently leads to tracheal mucus refluxing and thereby an increase in average tracheal residence time. The drugs were used to modify the normal deposition and clearance characteristics of a particular animal so that they resembled the normal characteristics of a different animal, suggesting that much of the large intersubject variability may be attributable to different levels of autonomic tone.

Nuclear scattering radiography of the spine and sphenoid bone.

Nuclear scattering radiographs of a portion of a spine and a sphenoid bone have been obtained using a 1 GeV proton beam. The ability of the method to yield three-dimensional representations is illustrated by three series of adjacent pictures corresponding to the three orthogonal planes (elementary volume: 5.2 mm3 and 0.9 mm3). The sensitivity of the method is discussed. Nuclear scattering radiographs are compared with ones obtained by conventional X-ray tomography and computed tomography. Nuclear scattering radiography also may be used to analyze the partition of hydrogen within the tissures. Hydrogen radiographs obtained in this way are shown.

Production of a novel red pigment, rubrolone, by Streptomyces echinoruber sp. nov. I. Taxonomy, fermentation and partial purification.

A taxonomic study of Streptomyces X-14077 (NRRL 8144), which produces a water-soluble purple-red pigment complex, revealed it to be a new species which has been named Streptomyces echinoruber sp. nov. The pigment complex was produced in deep culture fermentation and isolated by solvent extraction and concentration. The major pigment component, rubrolone, has low toxicity and may have potential as a food coloring agent. It appears to be devoid of antibiotic activity.

[Biochemical and clinical studies on the influence of 2-mercaptopropionylglycine on liver cell function (author's transl)].

2-Mercaptopropionylglycine was used for the treatment of chronic hepatitis. The drugs showed very good clinical results. In biochemical studies it was found that there was no effect on phosphate transport. Instead, there was an increase of reactive SH-groups sensitive to oligomycin in the membrane of rat liver mitochondria. The ATPase activity in mitochondria was diminished, while ATP-contents of the mitochondrial suspensions were increased by the drug. In the presence of ATP-MG++ there was an increase of oligomycin-sensitive contraction of the mitochondria. In aged mitochondria, the P/O ratios were improved. Reversal of electron transport with formation of NADH by succinate in the presence of rotenone and ATP was, concentration dependently, improved by MPG. Thus, MGP obviously acts at some specific sulfhydryl groups of the mitochondrion. It is concluded that the action of the drug in chronic hepatitis may be interpreted via an improvement of mitochondrial function.
