A case of fatal intracranial hemorrhage is reported in an eleven year old girl with acute idiopathic thrombocytopenic purpura following a viral infection. The patient was randomized to the IgG-arm of the ITP therapy study. Immunoglobulin administration was not followed by a raise of the thrombocyte count. Neither the IgG therapy nor intensive therapeutic measurements were able to prevent the fatal course of cerebral hemorrhage in this case. Pathological and immunological findings indicate that our patient suffered from a fulminant ITP which must be considered as a part of a still active viral disease.
The effect of dexamethasone on the synthesis of acute phase proteins has been studied in primary cultures of rat hepatocytes. In the absence of dexamethasone no detectable amounts of alpha 2-macroglobulin were synthesized by hepatocytes cultured for 1 day. alpha 2-Macroglobulin synthesis was induced by dexamethasone concentrations of 10(-8) M or higher with a maximum at a concentration of 10(-7) M. alpha 1-Acid glycoprotein was synthesized in the absence of dexamethasone; however, its synthesis was also greatly stimulated by dexamethasone concentrations of 10(-8)-10(-6) M. Synthesis of alpha 1-proteinase inhibitor was stimulated only 1.4-fold at a dexamethasone concentration of 10(-7) M. The kinetics of induction of alpha 2-macroglobulin and alpha 1-acid glycoprotein were studied at a dexamethasone concentration of 10(-7) M. After an initial lag phase of 3 h the synthesis of both proteins showed a steady increase during 2 days. Synthesis of albumin remained unchanged under these experimental conditions. Unlike alpha 2-macroglobulin and alpha 1-acid glycoprotein tyrosine aminotransferase activity increased already during the first 3 h of induction by dexamethasone with a maximum at 12 h followed by a slight decrease.
Aspergillus flavus grown on yeast extract-sucrose medium produced higher amounts of aflatoxin B1 in the presence of 0.025% sorbic acid than without this chemical with a maximum at 17 days of incubation. Addition of 0.05 to 0.0125% sorbic acid stimulated T-2 toxin production of Fusarium acuminatum cultures grown on maize meal. The highest amounts of the mycotoxin were detected in 14-day-old cultures containing 0.025% sorbic acid. It is assumed that certain amounts of sorbic acid near the minimal inhibitory concentration reduce the activity of the tricarboxylic acid cycle; this may lead to an accumulation of acetyl coenzyme A, which is an essential intermediate in the biosynthesis of aflatoxin B1 and T-2 toxin.
The sonography of three patients with a transpapillar catheter within the bile duct shows typically findings as the picture of catheter as a tubular structure with high level reflectivity, reduction of bile duct calibers and air in the bile ducts. The diminuation of the bile duct calibers is an argument for normal transpapillary bile flow and occurs before the change of laboratory parameters. In case of jaundice the differentiation between metastatic tumours of the liver and occlusion of the catheter is possible on the basis of ultrasound. After the diminuation of calibers of the bile ducts a metastatic tumours of liver are better to be seen.

The sulfhydryl group reagent N-ethylmaleimide was found to inhibit in a dose dependent manner regulatory volume decrease of human peripheral lymphocytes swollen in buffered hyposmotic NaCl media. In hyposmotic KCl media NEM treated lymphocytes prevented an additional secondary swelling seen in control lymphocytes. The data suggest that N-ethylmaleimide acts on ion transport mechanisms involved in volume regulatory changes. This effect contrasts with the stimulation by N-ethylmaleimide of apparently volume sensitive K/Cl fluxes in certain mammalian red cells.
On the basis of the patients treated by us we investigated the reasonable use of sonography in nephrologic questionings. As a result we drew the conclusions that the main fields of application are the various forms and stages of the renal insufficiency, the differential diagnosis of space demands and existing contraindication to contrast remedies or X-rays. In contrast to the up to now performed practice on suspicion of tumour and retention of urine in the bladder the sonography should be performed as first picture-giving method before the excretion urogram.
Thrombocytopenic purpura occurred in two women aged 67 and 77 years, who earlier on had been pregnant, six and twelve days after blood transfusions. The platelet deficient phase lasted 16 and more than 50 days. Differentiation from other causes of thrombocytopenic haemorrhage was based on immunohaematologic findings. Both patients were negative for the platelet specific antigen PlA1 (Zwa). The platelet specific antibody (anti-PlA1) responsible for the purpura could be demonstrated and characterised immunologically.
Flow cytometry, usually applied to cells which have time independent features, can also be used for kinetic experiments where the change of cell populations with time is investigated. Dedicated time sequencing programs written in Assembler and incorporated in the CYTOMIC 12 analyzer (4) are described. A sequence of 64 one parameter histograms can be automatically acquired and immediately displayed as a pseudo-two-parameter histogram. The acquisition time for each of the subsequent histograms can be selected between 1 and 32 seconds. Kinetics lasting up to 34 minutes are resolved into 64 time intervals. Two parameter kinetics can be resolved into 12 32 X 32 channel, two parameter histograms which are displayed and evaluated immediately on the analyzer screen in groups of 4 without using complicated list mode procedures. The standard CYTOMIC 12 software can be applied for processing and printing of the sequence distribution curves.
An investigation of the USP assay of chlorthalidone tablets showed that variable degradation of chlorthalidone occurred during assay preparation. The degradation products were isolated and identified. A stability-indicating high-performance liquid chromatographic (HPLC) assay which separates the degradation products from chlorthalidone was developed and used to examine the present USP preparation. The HPLC assay is suggested as an alternate.
A high-performance liquid chromatographic (HPLC) analysis for tetrahydrozoline and naphazoline in ophthalmic solutions is presented. The analysis allows a more reproducible, direct stability-indicating assay than the colorimetric methods generally employed. The HPLC system is so designed that a variety of ophthalmic solutions containing either naphazoline or tetrahydrozoline can be analyzed concomitantly.
Treatment with N-ethylmaleimide (NEM) is known to stimulate ouabain-insensitive, Cl- -dependent K+ transport in low K+ (LK) but not in high K+ (HK) sheep red cells (Lauf, P.K., and Theg, B.E., 1980, Biophys. Biochem. Res. Commun. 92:1422-1428). The dependence of this effect on the pH of pretreatment with NEM and/or iodoacetamide (IAA) was studied. Maximum stimulation of Cl- -dependent K+ transport in LK red cells was produced by prior treatment with 1-5 mM NEM at pH 6 at which only about 30-40% of the 10(7) SH groups present per membrane reacted. At pH 6 no NEM effect was seen on Nap K+ fluxes in HK red cells. Treatment with NEM below pH 6 enhanced Cl- -independent K+ transport in both LK and HK red cells. At higher pH values or higher concentrations the NEM-stimulation of K+ transport was reduced and absent at pH 8.7. Exposure of LK cells to 5 mM IAA prior to NEM abolished the stimulatory effect of NEM on K+ transport. Hence at least two different chemical groups were reacting with NEM, and more alkaline SH or NH2 groups whose reaction with NEM leads to an inhibition of the NEM effect brought about at pH 6.
Mucosal proctectomy with endorectal pull-through allows the removal of all colonic mucosa with preservation of continence. This operation was performed in 19 patients with familial polyposis coli and ulcerative colitis. A temporary loop ileostomy was used to defunctionalize the anastomosis. Intestinal continuity was restored in 17 of the 19 patients. Mean duration of follow-up was 29 months. All patients are continent, and the mean number of bowel movements per 24 hours is 6. Follow-up barium studies revealed a gradual dilatation of the terminal ileum within the rectal cuff which accounts for the decrease in the number of bowel movements. This operation eliminates the risk of carcinoma without compromising sphincter function.
In freezing experiments with isolated spinach thylakoids (Spinacia oleracea L. cv. Monatol) the cryoprotective efficiency of various low-molecular-weight polyols was determined. The activity of cyclic photophosphorylation was used as an assay for the functional integrity of the membranes. The results were compared with the osmotic behavior of the cryoprotectants at high concentrations. Equimolal concentrations of polyols which exhibit nearly comparable freezing point depressions even at high concentrations differed considerably in their protective capacity during a freeze-thaw cycle. This was particularly distinct when glucose, galactose, and ethylene glycol monomethyl ether were compared, but was also evident when various pentoses and deoxy-hexoses were used as cryoprotectants. Even in the absence of freezing, carbohydrates exerted a stabilizing influence on biomembranes. From the data it is suggested that in addition to colligative action of the compounds, a specific noncolligative mechanism contributes to membrane protection during freezing.
