Orally administered diacetoxyscirpenol (2 mg/kg of body weight) was rapidly absorbed into the blood serum of pigs; within 1 h, the highest amounts of diacetoxyscirpenol (9.6 to 21.9 ng/ml) were detected. Two metabolites of diacetoxyscirpenol were identified by gas chromatography-mass spectroscopy as monoacetoxyscirpenol and scirpenetriol. The three trichothecenes were present in the blood serum of pigs for only 24 h, indicating a rapid metabolism of these compounds.
We examined 100 alcoholics who had had no alcohol for more than 21 days, 100 controls, and 50 patients with essential tremor. Three percent of the controls and 47% of the alcoholics had a postural tremor. Alcoholic tremor was never severe, and functional disability occurred in only 17% of patients. There was no relation to age or duration of drinking, and only 1% of the alcoholics had a family history of tremor compared with 46% in essential tremor. Tremor frequency was significantly greater in the alcoholics than in essential tremor. Propranolol therapy decreased tremor more in the alcoholics than in essential tremor. The tremor of chronic alcoholism differs from essential tremor.

In 78 children (4 to 17 years of age) with moderate or severe asthma who were additionally treated with sustained-release theophylline preparations, different ways of drug monitoring were examined. Analysis of plasma and saliva theophylline was performed by means of high performance liquid chromatography. Saliva theophylline turned out to permit a reliable prediction of plasma theophylline, if an individual regression is calculated for each patient, basing on 3 simultaneously performed measurements of theophylline levels in saliva and plasma within the therapeutic range of 8 to 20 mg/l. In 25 patients theophylline levels were determined in venous and capillary blood. There was an excellent agreement (r = 0.97). Thus, a convenient monitoring of theophylline treatment in children is possible.
Mycotoxicoses have been recognized worldwide to cause problems in animal production. Trichothecene mycotoxins, for the most part produced by Fusarium spp., have obtained particular importance. Between 1982 and 1984, 295 samples of feedstuffs (cereals and mixed feeds) have been analyzed on natural occurrence of type-A trichothecenes. The skin toxicity test with guinea pigs was used as a screening method. Analysis of trichothecenes by capillary gaschromatography with flame ionization detection was complicated by interfering substances from the complex matrix of the sample-material. Definite results were only obtained by the use of gaschromatography-mass spectrometry. In comparison to the electron impact ionization the chemical ionization technique showed to be advantageous. Forty-two of the 295 samples analyzed were found to be positive in the biological assay. Nine of these positive samples contained trichothecenes as determined by mass spectrometry: T-2 toxin in mixed feed (65 micrograms/kg); oats (80 and 86 micrograms/kg) and wheat (100 micrograms/kg); diacetoxyscirpenol in mixed feed (125 micrograms/kg) and wheat (50 micrograms/kg); neosolaniol in oats (310 and 350 micrograms/kg); HT-2 toxin in oats (700 micrograms/kg). Oats proved to be contaminated more frequently as compared to the other cereals and the analyzed mixed feeds.

Between the end of 1979 and the beginning of 1984, the authors served as anaesthetists for Red Cross missions in surgical field hospitals on the Thai-Cambodian and the Afghan-Pakistan borders, in Lebanon, and in Indonesia. A total of 3665 civilian emergency patients were anaesthetised. 643 were operated on under local anaesthesia; 639, under regional anaesthesia, in most cases spinal anaesthesia. In spite of principal preference for local and regional anaesthetic techniques, 65% of the patients (2383 patients) were managed under general anaesthesia, which was maintained with halothane in 947 cases and ketamine in 1345 cases. 877 general anaesthesias with ketamine were performed with spontaneous breathing of ambient air; endotracheal anaesthesia was necessary in 1238 patients. In spite of the high risk for the patients and of the operative interventions and in the light of the simple and sometimes even primitive working conditions, no anaesthesia-related fatalities occurred. Postoperative lethality was 2%. In all cases, the duties of the anaesthesist involved more than performance of anaesthesia, which was sometimes assigned to paramedics. Other duties included preoperative examination, postoperative intensive care, consultation services in nonsurgical emergency cases, resuscitation, and training of local assistants.

A simple and reliable staining technique is described using the fluorescent brightener Blankophor BA which binds specifically to fungal cell wall components. Potential diagnostic applications are shown.
Turpentine injection into rats elicits enhanced secretion of acute phase proteins including alpha 2-macroglobulin (alpha 2M). Hypophysectomized rats, however, do not respond in this way unless dexamethasone is given together with turpentine. On the other hand, dexamethasone injection alone did not result in an induction of alpha 2M synthesis. When a medium of Kupffer cell cultures was added to hepatocytes, a dose-dependent stimulation of alpha 2M synthesis of up to 4-fold after 10-12 h was observed. However, the presence of low concentrations (10(-9)M) of dexamethasone was essential for the stimulatory effect. We conclude that the acute phase induction of alpha 2M in hepatocytes requires the synergistic action of glucocorticoids and a non-dialysable factor secreted by Kupffer cells.

We have determined in 22 volunteers the fibrinolytic activities of regular and of dextran sulfate precipitated euglobulin fractions in the presence and absence of flufenamate before and after 5 to 20 min of venous occlusion. Using the fibrin plate assay, Hageman factor Independent Intrinsic Fibrinolytic activity ( HIIF ) was measured in plasma euglobulins obtained from resting subjects in the morning. Hageman factor Dependent Intrinsic Fibrinolysis ( HDIF ) was calculated by subtracting the HIIF from the activity obtained after activation of the "resting plasma" by dextran sulfate. The activity of vascular Plasminogen Activator (v-PA) released after venous stasis was determined by subtraction of resting HIIF from post-occlusion total plasminogen activator activity. In normal subjects, the distribution of all activities followed a logarithmic scale. Twelve of 22 volunteers exhibited maximal v-PA activity after 20 min of venous occlusion. In three, it was maximal after 5 min, in three others after 10 min, and in four after 15 min of venous stasis. These results demonstrates that it may not be possible to clearly distinguish normal from abnormal responders using a stasis test of fixed duration.
Severe digoxin intoxications have a mortality rate of about 20%. Clinical course of intoxication, electrocardiographic changes as well as the behaviour of the plasma-digoxin concentration could closely be controlled on 4 patients after suicidal ingestion of potentially lethal dilanacin -new doses. In 3 patients a haemoperfusion treatment lasting 4 to 6 hours was performed. Clinical symptoms and existing disturbances of cardiac rhythm were clearly positively influenced under perfusion. The earlier the treatment was begun the more favourably appear the effects on the course of intoxication. After an at first benign picture of intoxication in the non-perfused female patient a comparatively distinct prolonged clinical and electrocardiographic symptomatology developed. Though the digoxin extraction established by computation fairly slightly imposes, this eliminated free digoxin fraction seems, nevertheless, to possess a considerable pharmacological and clinical importance. The adsorber material used showed well reproducible high clearance performances. During the haemoperfusion treatment in no case clinical complications were observed.
