[Olfactory neuroblastoma. A clinical and morphological study].

A clinical and morphological study about neuroblastoma with olfactory differentiation are made. This study includes: TAC, vanimil mandelic acid urine determination, optic microscope, immunohistochemical technique, neuropeptidal, in situ hybridization, DNA analysis by flow cytometry and electronic microscope.

[Normal values of nasal mucociliary clearance. Comparison of various techniques and substances].

We have study the nasal mucociliary clearance in 150 normal persons. Four methods have been used: vegetable charcoal powder, saccharin, seroalbumine-Tc99m and pertechnetate-Tc99m. The time and speed of mucociliary nasal transport are related to the solubility of the tracer. The insoluble substances have a faster transport. The mean transport time for charcoal powder is 8.3 +/- 2.6 min. and for the saccharin is 17.4 +/- 5.5 min. The mean velocity for seroalbumine is 8 +/- 1.8 mm/min. and for the pertechnetate is 5 +/- 0.2 mm/min.

[Flow cytometry. Its prognostic value in cancer of the head and neck].

The authors introduce an historical review and technical information on the management of this technique. The current condition of this matter is commented on, with special interest in head and neck tumors. Very few, contradictory papers have been published on laryngeal tumors concerning the prognostic value of aneuploid tumors.

[Neuromodulators in otorhinolaryngology].

Malignant fibrous histiocytoma of the head and neck. Case report.

Malignant fibrous histiocytoma (MFH) is a rare tumor included in the group of soft tissue sarcomas. Approximately 1-3% of these tumors affect the head and neck, with about 100 cases reported to date. A case affecting the retromolar trigone and involving the postero-lateral portion of the left hemi-tongue is reported. Etiological and pathological aspects are discussed. Diagnosis, treatment and prognosis of this neoplasm are reviewed.

[Pleomorphic adenoma of the minor salivary gland].

Minor salivary gland tumors represent about 10-15% of salivary gland tumors. The most frequently seen histological type are pleomorphic adenomas, which are usually located at the hard palate. Clinical and pathological features of 7 cases are reviewed.

[Pansinusitis: approach to its etiopathogenesis].

Local factors and external conditions are determinants for pansinusitis. We present 10 cases in which 20% are of unknown origin, 30% have allergic antecedents, 20% show hypersensitivity to aspirin, 20% have an immotile-cilia syndrome and 10% are caused by exogenous factors. Surgical treatment is performed in 70% of the cases, always followed by a long-term medical therapy. Forty per cent of the cases show a chronic progress despite the type of treatment applied.

[The histological study of adenopathies in laryngeal cancer with supraglottic localization].

A histopathologic study was made to establish the prognostic importance of lymph node involvement in operable supraglottic laryngeal carcinoma. A total of 51 cancer patients underwent supraglottic horizontal laryngectomy with bilateral cervical dissection. The number of adenopathies the presence of capsule rupture and type of corresponding infiltration were significantly related to survival (p < 0.01). Prognosis was poorer in patients with more than three metastatic adenopathies and/or capsule rupture of one or more of them. Patient survival in cases with capsule rupture affecting one or more nodes was 36%, versus 33% among patients with three or more metastatic nodes. It is concluded that (a) the poor survival rate observed among these patients points to the need for more aggressive treatment protocols; and (b) cervical dissection should be performed systematically not only for therapeutic purposes but also to improve patient prognosis.

[The effect of visual feedback exercises on balance in normal subjects].

The effects of visual feedback exercises on balance was studied in normal individuals, to evaluate the efficacy of visual feedback of postural oscillation in improving stability. A DINAS-CAN dynamometric platform designed by the Valencian Institute of Biomechanics (IBv) was used. Before to commencing training sessions, we determined the following parameters in each individual: (a) static stability, via posturography; and (b) individual ability to displace and voluntarily maintain the center of gravity within the limits of stability. All static measures were poorer after training, but the differences were not significant (p > 0.01). In contrast, the exercises performed via visual feedback of postural control improved significantly (p < 0.01). Training by visual feedback facilitates integration of visual, somatosensory, and vestibular information. In normal individuals, stability improves under excentric conditions, but no improvement is seen in the central resting position.

[Verifiable diagnosis of the puncture of the fine needle aspiration in head and neck neoplasms].

We have studied 103 fine needle aspiration (FNA) biopsies as diagnostic technique on patients with head and neck tumours: 55 FNA on thyroid gland, 35 in cervical lymph nodes and 13 on salivary glands. The results obtained by PAAF were coincident in 90.8% of the cases with histology and clinical evolution of the patient. Specificity was of 98%, sensibility to detect the tumour presence 84%, with a positive predictive value of 95%. No complications were found in any of the patients. We conclude that PAAF is an excellent first line method for diagnostic investigation of palpable masses in head and neck, and allows us to reduced the number of surgical manoeuvres.

[Primary ciliary dyskinesia: functional and morphological study].

We present 7 patients with a typical symptomatology of immotile cilia syndrome, three of them with complete situs inversus. Nasal mucociliary transport was studied by sero-albumin marked with technetium 99m. In all cases there was an absence of transport. The ultrastructure of the nasal cilia was studied. The findings were: In four cases alterations in the dynein arms, in three cases alterations in the central and peripheral microtubules associated por not to the defects in the dynein arms, and in one cases absence of cilia were observed. The ciliary complexes were common to all cases.

[Painful shoulder syndrome with drooping after cervical lymphatic dissection].

A study was made of 12 patients with head and neck cancer who underwent surgical neck dissection in 1993 to evaluate quantitatively the degree of postoperative shoulder dysfunction after surgical neck dissection. Nerve conduction studies were made of the accessory nerve and the range of motion, strength and position of the shoulder were evaluated. Patients were invited to complete a questionnaire about daily living activities, shoulder pain, shoulder movement and shoulder droop. Our results showed that abnormalities can be found in shoulder and arm function after any type of neck dissection and that these are evident when the accessory nerve is damaged. Subjective questionnaire findings generally coincided with objective postoperative dysfunction.
