[Hysterectomy to stop bleeding in obstetrics. Are there still indications for performing hysterectomy immediately post-partum? (author's transl)].

13 cases of hysterectomy carried out to stop bleeding immediately after delivery have been studied. Among these there were : 5 cases of placenta accreta which confirmed histologically. 4 cases of uterine inertia. 2 cases of failure of coagulation, one of which occurred after there had been a retro-placental haematoma and 2 ruptured uteri. After analysing the clinical circumstances surrounding these 13 cases and the technical details in carrying out hysterectomy on the gravid uterus, the authors studied conservative surgical measures that could be used to deal with post-partum uterine haemorrhage : the hypogastric arteries on the one hand the efferent arteries supplying the uterus on the other hand can be ligated. The technique for this last procedure is illustrated in one case.

[Dystocia at the onset of labour. An evaluation of the different treatments available (author's transl)].

The authors have carried out a random study on 75 cases in order to evaluate objectively the therapeutic methods usually employed in cases of dystocia in starting labour. These studies have shown the superiority of Syntocinon using an infusion together with epidural analgesia. This attitude goes against the methods of treatment usually used in France. The authors explain the reasons for their choice in the light of factors that they have observed using other therapeutic methods.

[Viral hepatitis: incidence and epidemiologic pattern in an urban area (author's transl)].

In order to describe the incidence and the epidemiologic pattern of viral hepatitis (V.H.) in a French urban area, with a special attention to ambulatory cases, an epidemiological information system has been developed during one year. The physicians (sample), the medical laboratories and, at a lesser degree, the hospital care units, have taken part in this work. The incidence rate of ambulatory cares has been estimated 85 cases/10(5) persons (69/110), less than previously assumed. Non-B V.H. remains more frequent (52 cases/10(5) persons), but B.V.H. are rather close to them (33 cases/10(5) persons). Large differences exist between geographic areas, without evident rational. Higher incidence rates characterize children (especially 5-9 years) and migrant people (especially from North Africa): those facts are completely account by very high rates within the subgroup of migrant children (473 and 260 cases/10(5) persons). The reported patterns allows us to point on this high risk characteristic of migrant's children and to assume an important modification in the pattern of local inhabitants V.H. in the region. These trends have to be followed by further studies.

[Contraception by progestational agents administered in sequential form (author's transl)].

On the basis of a study involving 2338 months of treatment with progestational agents administered in sequential form from the 10th to the 25th day of the cycle, in women during the premenopausal period or suffering from benign or malignant breast pathology, the authors evaluated the contraceptive efficacy of this treatment. This efficacy was found to be good (Pearl index: 1 to 1,6%) with norsteroid progestational agents and inadequate with hydroxyprogesterone derivatives. It would thus appear possible to ensure coincidence of therapeutic action and contraception when norsteroids are used after the age of 40 (lower fertility) and/or when other methods are contraindicated.

[Ophthalmologic pathology in the pregnant woman].

Literature review and report of personal cases concerning the ocular physiopathological problems of pregnant women. Are studied especially chorioretinal perturbations: surgical and medical retinal detachment, diabetic retinopathy, choroiditis with pseudohistoplasmosis and toxoplasmosis. Are also studied the variations of the visual acuity, ocular tumors and pituitary tumors.

[Acute upper genital tract infections. II. A. bacteriological study and the therapeutic consequences].

The authors have studied a prospective series of 62 cases of acute upper genital tract infections of different severities. Aerobic and anaerobic bacteriological examination were carried out from different sites: urine, blood, vagina, endocervix, intra-uterine devices and the pouch of Douglas. It is absolutely necessary to follow a strict technique according to the clinical examination in order to interpret the results according to a methodological plan. The results show that the gonococcus was found in 9 out of the 62 cases, which had a very varied flora. The value of the different smears and swabs is then discussed. In this way it is shown that specimens taken from the endocervix are definitely of great value although the ideal is to culture directly from intra-uterine devices or from specimens taken laparoscopically from the tubes. It is possible to give different courses of antibiotics: these should include however active substances against the group of anaerobes, which justifies adding metronidazole routinely. The use of corticoids which are often prescribed to limit the sequelae has not been proven to be of value.

[Lipoprotein-lipase-activity in human milk (author's transl)].

Lipoprotein-lipase-activity has been determined in human milk between the 3rd and the 10th post-partum days in women from different ethnic origins. This activity is low in the colostrum, it increases at the 4th and 5th days and remains stable between the 6th and the 10th day. There are important variations of the LPL-activity from one woman to another. Two variation-factors have been noticed. The first is an ethnic factor. It is apparent event if the diet is exactly the same in all women. In the milk, indeed, the activity of French women is much lower. It is about three times less than the activity of Yugoslavian of African women. The second is the galactogenic powder that has been estimated by the natural duration of lactation. We can see that the activity of women, who have to stop breast-feeding because of insufficient milk, is lower than the activity of women who can decide how long they will breast-feed their child. We took as a hypothesis that the milk is a means of elimination for the mammary-LPL. This enzyme is supposed to have done first its physiological action at the surface of mammary glandular cells. Ours results show more facts in favor of this hypothesis.

[Dietary behavior during pregnancy (author's transl)].

A study on dietary behavior during pregnancy was performed between June and December 1975 at the Saint Antoine Maternity Hospital in Paris. In addition to the routine clinical examinations at the 3rd, 6th, 8th and 9th month of pregnancy, the women were systematically questioned on their dietary and tobacco habits. Results show that: (1) the mean caloric intake is constant during the first trimester and then significantly decreases in the last two trimesters (2) the fatter the woman before pregnancy, the lower the caloric intake at the successive examinations (3) the total weight gain is positively related to the caloric intake during the first trimester, but not related to the dietary data observed at the 6th, 8th or 9th examinations (4) birth weight seems to be more related to body size before pregnancy than to weight gain.

Measurement of plasma human chorionic gonadotropin (hCG) and beta-hCG activities in the late luteal phase: evidence of the occurrence of spontaneous menstrual abortions in infertile women.

Plasma luteinizing hormone (LH-human chorionic gonadotropin (hCG) and beta-hCG activities were measured during the late luteal phase in 321 cycles of 147 infertile women. In 71 cycles the hCG measurement permitted the diagnosis of pregnancy between the 10th and 14th days after the thermal nadir. The slope of the regression line derived from hCG levels during the first 22 days of pregnancy was significantly lower in pregnancies which aborted before the 60th day than in normal pregnancies (P less than 0.01). Among 72 cycles ended by apparently normal menses which exhibited an LH-hCG activity at least equal to 7 mIU of hCG/ml during the late luteal phase, the beta-hCG activity was measured in 49 cycles during which hCG had not been given. Significant beta-hCG activity (greater than or equal to 4 mIU of hCG/ml) was detected in 19 cases. This finding supports the assumption that secretory trophoblastic tissue had been present and that spontaneous menstrual abortions had occurred in these women.

[The outcome in pregnancies resulting after the induction of ovulation 519 pregnancies (author's transl)].

Some geneticists have been against induction of ovulation because there is a greater incidence of chromosome abnormalities in spontaneous abortions that occur after induced pregnancies. We have found the number of early abortions (15.6 p. 100) and of malformed fetuses (4.8 p. 100) is not greater than average when we have studied a personal prospective series of 140 pregnancies and a series of 519 pregnancies collected following an enquiry made among some of our colleagues. The number of late abortions and of premature labours is much higher (3.4 p. 100 and 14.8 p. 100 respectively) and they correspond particularly with the higher incidence of multiple pregnancies (12.3 p. 100) which have been obtained especially after the use of HMG. This has a direct repercussion on perinatal mortality (4.3 p. 100).

[Severe acute genital infections (Based on 100 cases). A clinical study and the contribution of laparoscopy (author's transl)].

A continuous prospective series of 100 cases of severe acute genital infections which were collected over a period of 16 months at the Gynaecological and Obstetrical Clinic of the Saint Antoine Hospital has been studies using the same protocol. Analysing the history given by these patients before the onset of the illness shows how young they were and how frequently they were nulliparous, as well as the role of intra-uterine contraceptive devices. The correlation between the clinical features and those found on laparoscopy which was carried out almost routinely shows the great advantages of this examination. The authors in their discussion have tried, by analysing groups of symptoms, to show that to make an early correct diagnosis laparoscopy has to be used in a majority of cases. It allows treatment to be instituted quickly and unhesitatingly. Furthermore, it is justified because: It is necessary to find out all lesions that are present and which are unpredictable clinically. It renders it possible to carry out therapeutic manoeuvres (such as cutting adhesions and giving antibiotics locally) and to take swabs for bacteriological examination (this will be discussed in the second part of this article).
